Site: Potable Water-Far Point
Violation: The daily calibration check for the halogen analyzer was not recorded from October 3 to October 19, 2009.
Recommendation: Ensure that the daily, manual comparison test or calibration is recorded on the chart or in a log book.

Site: Potable Water-Cross-Connection Control Program
Violation: The location and system for the two backflow prevention devices in the garbage room were listed incorrectly on the cross-connection control program.
Recommendation: Ensure that the cross-connection control program includes at minimum: a complete listing of cross-connections and the backflow prevention method/device for each, so that there is a match to the plumbing system component and location, and an inspection frequency. Ensure that air-gaps are included in the listing.

Site: Potable Water-Potable Water Hose and Fitting Locker
Violation: There were no backflow prevention devices on the two threaded potable water taps. This was corrected.
Recommendation: Ensure that backflow preventers are installed when air gaps are impractical or when water under pressure is required.

Site: Galley-Deck 4 Soup Station
Violation: The backflow prevention device on the combination oven had a continuous drip of water at the intermediate vent.
Recommendation: Ensure that backflow preventers are maintained in good repair.

Site: Medical-Asymptomatic Crew Follow-up
Violation: According to the medical staff, follow-up for asymptomatic crew members is done for only 24 hours. In the STARDOCS database, the time and date stamp for zero and 24 hour follow-ups were the exact same date and time. This was the case for the past three asymptomatic crew members. In addition, the date and time stamped 48 hour follow-up was for the day following the 24 hour follow-up. However, according the medical staff, no follow-up was done at 48 hours and the box for this item was only checked to close out the case.
Recommendation: Ensure that medical staff conduct daily verbal interviews with asymptomatic crew until 48 hours after onset of the ill crew members symptoms.

Site: Pantry-Deck 10 Ice Station FSD 10-4-006
Violation: The handle of the ice scoop was in direct contact with the ice in the ice bin.
Recommendation: Ensure that the ice scoop is stored so that the handle does not touch the ice.

Violation: There were 3 trolleys of bread that did not have the top tray of bread covered.
Recommendation: Ensure that food is protected from cross-contamination by: (4) Storing the food in packages, covered containers, or wrappings.

Violation: There was peeling sealant on both sides of both fryers.
Recommendation: Remove the peeling sealant and replace only as necessary.

Violation: Some of the shields on the lights to the infrared warmers were rough and difficult to clean.
Recommendation: Replace the damaged light shields.

Violation: The front cover on the water/steam control panel on the left tilting kettle was bent, creating a large gap.
Recommendation: Repair the damage to the water/steam control panel.

Violation: The solution in the sanitizing bucket was measured at greater than 200 ppm. This was corrected.
Recommendation: Ensure that a chlorine solution used as a sanitizing solution has a concentration between 50 mg/L (ppm) and 200 mg/L (ppm).

Site: Pantry-Deck 8 Ice Station FSD 08-3-004
Violation: There was ice in the handwash sink.
Recommendation: Ensure that handwashing facilities are used for no other purpose and are accessible at all times.

Violation: There were multiple holes in the bulkhead where the plugs were missing. This was noted in the back area of the fruit room and in the dairy room.
Recommendation: Replace the missing plugs.

Violation: The lower right section of the entrance to the room was severely bent, making cleaning difficult.
Recommendation: Ensure that decks, bulkheads, and deckheads in food preparation, warewashing, pantries, and storage areas are constructed and maintained for easy cleaning.
