Site: Galley-Deck 2 Hot Galley
Violation: The backflow prevention device installed on the potable water line for the hood cleaning system was continuously leaking from the pressure relief valve.
Recommendation: Ensure that backflow preventers are maintained in good repair.

Violation: A backflow prevention device between the carbonation tank and the brass fitting was not able to be located on the carbonator.
Recommendation: Ensure an appropriate backflow prevention device is installed between the brass fitting and the carbonation tank.

Site: Potable Water-Cross-connection Control Program
Violation: The cross-connection control program listed "internal" for the type of backflow prevention devices for the Ecolab, Oasis and Scout dispensers. The cross-connection control program listed the backflow prevention devices for the undercounter glasswash machines at the Crow's Nest Bar and the Exploration Cafe as Watts N9. Upon inspection, the devices were a Watts 9D.
Recommendation: Ensure that the cross-connection control program includes at minimum: a complete listing of cross-connections and the backflow prevention method/device for each, so that there is a match to the plumbing system component and location, and an inspection frequency. Ensure that air-gaps are included in the listing.

Site: Potable Water-Engine Room
Violation: The potable water fill line for the pools and jacuzzis was not striped blue.
Recommendation: Ensure that potable water piping and fittings are painted or striped auxiliary blue, or in accordance with ISO 14726, at 5 m (15 feet) intervals on each side of partitions, decks, and bulkheads in all spaces, except where the decor would be marred by such markings.

Violation: The vent on the backflow prevention device for the undercounter glasswash machine was plugged.
Recommendation: Remove the plug from the vent on the backflow prevention device.

Violation: The original screw inserts were stripped so staff drilled additional holes into the anti-entrapment drain covers.
Recommendation: Ensure the drain covers are installed according to manufacturer's recommendations.

Violation: The suction fitting was not flush with the side of the jacuzzi. The threads on the suction fitting were visible.

Site: Housekeeping-Private Whirlpool Spas
Violation: The private whirlpool spas located in individual passenger cabins were disinfected on August 16th and not again until Aug 25th. Also, these whirlpool spas were disinfected on September 20, 2009 and not again until October 18th.
Recommendation: Ensure that private whirlpool spas located in individual passenger cabins are cleaned and disinfected, including associated recirculation systems, between occupancies or weekly, whichever is more frequent.

Site: Food Service General-Outer Sliding Doors/Provisions
Violation: The outer sliding doors in the provisions areas did not have inverted recessed nipples where the latching pins inserted. Also, the top of the right door frames were not sealed, both lead to a void space in the frame.
Recommendation: Close the opening at the latching pin insert and seal the top of the door frame.

Site: Galley-Deck 2 Potwash
Violation: The data plate for the mechanical potwash did not specify the wash cycle time. It only specified the final rinse cycle time.
Recommendation: Ensure that warewashing machines are provided with an easily accessible and readable data plate affixed to the machine by the manufacturer that indicates the machine's design and operating specifications including the: (1) Temperatures required for washing, rinsing, and sanitizing; (2) Pressure required for the fresh water sanitizing rinse; and (3) Conveyor speed for conveyor machines or cycle time for stationary rack machines.

Site: Galley-Pinnacle Grill Dishwash
Violation: The data plate for the hood type dishwash stated that the run time for cycle 1 was 7 minutes, cycle 2 was 3 minutes and cycle 3 was 5 minutes. The machine was in-use and using cycle 1 during the inspection. When timed, this cycle only ran for 1 minute. The plate surface temperature of the final rinse was 164 F when measured with the inspector's thermometer.
Recommendation: Ensure the appropriate cycle time for cycle 1 is listed on the data plate.

Violation: There was an opening in the bulkhead around the drain line behind the potato peeler.
Recommendation: Close the opening.

Violation: The poured deck at the entrance of the bakery was chipped and rough at the stainless steel juncture.
Recommendation: Ensure that decks, bulkheads, and deckheads in food preparation, warewashing, pantries, and storage areas are constructed and maintained for easy cleaning.

Violation: There was an opening in the bulkhead around the electrical lines in the technical space below the toaster.
Recommendation: Close the opening.

Violation: The flanges over the potable water lines supplying the tilting pans were loose, causing an unsealed opening in the bulkhead.
Recommendation: Seal the opening by reattaching the flanges.

Violation: The poured deck was chipped and worn in a few places.
Recommendation: Ensure that decks, bulkheads, and deckheads in food preparation, warewashing, pantries, and storage areas are constructed and maintained for easy cleaning.

Violation: A small amount of condensate had collected on the deckhead above the clean landing of the conveyor type dishwash machine.
Recommendation: Ensure that ventilation hood systems and devices are sufficient in number and capacity to prevent grease or condensation from collecting on bulkheads and deckheads.

Site: Provisions-Dry Stores
Violation: There were many small dead flies on the roach trap by the drain in the back right corner. Also, there were two live small flies in this area.
Recommendation: Ensure that the presence of insects, rodents, and other pests is effectively controlled to minimize their presence in the food storage, preparation, service areas, warewashing, and utensil storage areas aboard a vessel.
