The backflow preventers installed on the potable water lines supplying the boilers, chemical feeds, and showers in two different rooms in the thermal suite were not on the backflow preventer listing. This was corrected during the inspection.

Recommendation:
Ensure that the cross-connection control program includes at minimum: a complete listing of cross-connections and the backflow prevention method/device for each, so that there is a match to the plumbing system component and location, and an inspection frequency. Ensure that air-gaps are included in the listing.

The in-use conveyor dishwash and glasswash machines had partially blocked upper spray arm nozzles in the final rinse. The spray pattern from the left two upper arm spray nozzles was weak, producing only a narrow water stream.

Recommendation:
Ensure that warewashing equipment is maintained in good repair and proper adjustment including: (1) Being maintained in a state of repair and condition that meets the standards of the materials, design, and construction of these guidelines; and (2) Maintaining water pressure, and water temperature measuring devices in good repair and ensuring their accuracy within the intended range of use.

The clean dish racks were stacked on a deck stand positioned directly below the flying insect glue lamp. The dish racks were moved away during the inspection.

Recommendation:
Ensure that cleaned equipment and utensils, laundered linens, and single-service and single-use articles are stored: (1) In a clean, dry location; (2) Where they are not exposed to splash, dust, or other contamination; and (3) At least 150 mm (6 inches) above the deck.

The "wash hands after using toilet" sign was over the toilet and not on the bulkhead near the exit door. Also, when fully open, the exit door would stay open. When closing, it would not close completely.

Profile closed those door frames where locking pins and latches insert.
