Site: Medical-
Violation: The number of ill passengers and crew members on the top of the gastrointestinal illness log included non-reportable cases.
Recommendation: Ensure that the illness totals on the gastrointestinal illness log includes only reportable cases of gastrointestinal illnesses.

Violation: There was a leak at the vent on the reduced pressure assembly backflow prevention device inside the hood cleaning cabinet.

Site: Potable Water-
Violation: There was a listing of air-gaps for the vessel. This listing was several pages long and appeared to predominantly cover food areas. There was no information on the listing that specified what was connected to the potable water system. A few of these areas were visited and there were no connections that were protected with air gaps in those locations.
Recommendation: Ensure that the cross-connection control program includes at minimum: a complete listing of cross-connections and the backflow prevention method/device for each, so that there is a match to the plumbing system component and location, and an inspection frequency. Ensure that air-gaps are included in the listing.

Site: Potable Water-Engine Workshop
Violation: Only two out of a potential six backflow prevention devices were visible at the laundry machines. The four other devices that were said to be behind the machines could not be seen.
Recommendation: Ensure that backflow preventers are located so they may be inspected, serviced, and maintained.

Violation: The potable water lines to the sink were not painted or striped blue.
Recommendation: Ensure that potable water piping and fittings are painted or stripped auxiliary blue, or in accordance with ISO 14726, at 5 m (15 feet) intervals on each side of partitions, decks, and bulkheads in all spaces, except where the decor would be marred by such markings.

Site: Potable Water-Cross-Connection Control Program
Violation: Cross-connections on the list, such as those at the soda machines and some coffee machines, were said to have been inspected in December 2009. However, these connections were removed at least several months prior to that date.
Recommendation: Ensure that the cross-connection control program includes at minimum: a complete listing of cross-connections and the backflow prevention method/device for each, so that there is a match to the plumbing system component and location, and an inspection frequency. Ensure that air-gaps are included in the listing.

Violation: The deck material in all of the galley areas was in very poor condition. There was not a smooth transition between the deck material and the scuppers, the bulkhead, or equipment legs. The deck material was peeling at these junctures, and was also soiled in several of the areas where it was peeling.
Recommendation: Ensure that decks, bulkheads, and deckheads in food preparation, warewashing, pantries, and storage areas are constructed and maintained for easy cleaning. Site: Other-Outbreak Prevention and Response Plan
Violation: The triggers for going to medium or high levels was dependent on the number of cases in a 24 hour period. It was possible for the vessel to reach a 2% cumulative proportion of passengers or crew and not go to medium or high levels. High level was the level at which continuous disinfection was required of hand contact surfaces.
Recommendation: Ensure that when the cumulative proportion of cases of gastrointestinal illness among passengers or crew members is 2%, the infection control response includes cleaning and disinfecting all public areas, including handrails and restrooms, on a continuous basis until the proportion decreases to < 2%.
