Site: Medical-Gastrointestinal Illness Log
Violation: On the cruise 3/27-4/10, the log had a column for number of diarrhea and that column was filled out with the word "several". That passenger was not listed as a reportable case. In addition, there were a few passengers that had vomiting and one additional symptom and they were not listed as reportable cases.
Recommendation: A reportable case of gastrointestinal illness shall be defined as: (1) Diarrhea (three or more episodes of loose stools in a 24 hour period); or (2) Vomiting and one additional symptom including one or more episodes of loose stools in a 24-hour period, or abdominal cramps, or headache, or muscle aches, or fever (temperature >38 C (100.4 F)); and (3) Reported to the master of the vessel, the medical staff, or other designated staff by a passenger or a crew member.

Violation: There were no sample valves on the potable water tanks.
Recommendation: Ensure that potable water tanks have sample valves which are turned down.

Violation: Per staff, the initial reading for the chlorine residual was taken at the start of production. However, the logs indicated that the first reading was taken between an hour and 3 hours after the start of production.
Recommendation: Ensure the first recorded reading is within 30 minutes from the start of production.

Violation: There was no documentation of the manufacturer's recommendation for application, drying, and curing for the potable water tank coatings.
Recommendation: Ensure that interior coatings on potable water tanks are approved for potable water contact, and all manufacturer's recommendations for application, drying, or curing are followed. Ensure written documentation for the coating used and recommendations followed are available for review during inspections.

Violation: Per staff, the asymptomatic cabin mates were not interviewed daily until 48 hours after the onset of the ill crew member's symptoms.

Violation: The back shelving unit did not have a few shelf locks. White tape was used instead of the locks to hold the unit in place. This made cleaning difficult.
Recommendation: Ensure that nonfood-contact surfaces of equipment that are exposed to splash, spillage, or other food soiling or that require frequent cleaning are constructed of a corrosion-resistant, nonabsorbent, and smooth material.

Recommendation: Ensure that warewashing machines are provided with an easily accessible and readable data plate affixed to the machine by the manufacturer that indicates the machine's design and operating specifications including the: (1) Temperatures required for washing, rinsing, and sanitizing; (2) Pressure required for the fresh water sanitizing rinse; and (3) Conveyor speed for conveyor machines or cycle time for stationary rack machines.

Violation: The room used to store liquor was unfinished. The deck/bulkhead junctures were not coved. The room had many hard to reach and clean spaces. The bulkheads and decks were not easy to clean and in disrepair. In addition, there were exposed utility lines and pipes that made cleaning difficult.
Recommendation: Ensure that decks, bulkheads, and deckheads in food preparation, warewashing, pantries, and storage areas are constructed and maintained for easy cleaning. Ensure that bulkhead and deck junctures are coved (including galleys, pantries, deck/counter junctures at buffets, bars, waiter stations, and dining room work counters). Ensure that bulkheads and deckheads have smooth, hard finishes, and light colored surfaces. Ensure that decks, bulkheads, and deckheads in food preparation, warewashing, pantries, and storage areas are corrosion-resistant. Ensure that exposed utility service lines and pipes, including lines for fire detection and protection systems, are installed so they do not obstruct or prevent cleaning. Ensure that decks, bulkheads, and deckheads in food preparation, warewashing, pantries, and storage areas, are maintained in good repair.
