Site: Medical-Bacterial Specimen Collection Containers
Violation: The bacterial specimen collection containers expired 6/2010. According to the ordering records, new containers had been ordered on 5/14/2010, and again on 9/13/2010.
Recommendation: Ensure that at least 10 clinical specimen collection containers for both viral and bacteriological agents (10 each), as well as the proper shipping containers and labels for same, are maintained by the medical staff.

Site: Potable Water-Mid-Ship Pool Machinery Room
Violation: There was a leak from the vent on the reduced pressure assembly on the hot water line.
Recommendation: Ensure that backflow preventers are maintained in good repair.

Site: Potable Water-Cross-Connection Control Program
Violation: The internal air gap on the 3M chemical dispenser in the deck 2 chemical locker and the two backflow prevention devices on the handwash sink by the laundry entrance were not listed on the cross-connection control program. These were added to the list.
Recommendation: Ensure that the cross-connection control program includes at minimum: a complete listing of cross-connections and the backflow prevention method/device for each, so that there is a match to the plumbing system component and location, and an inspection frequency. Ensure that air-gaps are included in the listing.

Site: Room Service-
Violation: The bulk milk container dispensing tube was greater than one inch in length. This was corrected.
Recommendation: Ensure that the bulk milk container dispensing tube is cut on the diagonal leaving no more than 25mm (1 inch) protruding from the chilled dispensing head.

Site: Galley-Pastry
Violation: There was missing and peeling sealant around the edge of the marble work-top. This was corrected.
Recommendation: Ensure that nonfood-contact surfaces are free of unnecessary ledges, projections, and crevices, and designed and constructed to allow easy cleaning and to facilitate maintenance.

Violation: The flange around the pulper drain line was loose at the bulkhead.
Recommendation: Secure the flange to the bulkhead.

Site: Food Service General-Dishwash Areas
Violation: The conveyor dishwash machines had Opti-Rinse nozzles on the final rinse spray arms. The key could not be found to activate the over-ride to view the spray pattern on these nozzles.
Recommendation: Ensure that the proper operation of the spray heads can be viewed for inspection.

Violation: The small bowl of sanitizer on the counter was measured as greater than 200 ppm chlorine.
Recommendation: Ensure that a chlorine solution used as a sanitizing solution has a concentration between 50 mg/L (ppm) and 200 mg/L (ppm).

Violation: The inside of both beer taps was soiled.
Recommendation: Ensure that food only contacts surfaces of equipment and utensils that are cleaned and sanitized.

Violation: The large pipe in the left technical compartment below the bone crusher was heavily soiled with a dried material. This was corrected.
Recommendation: Ensure that nonfood-contact surfaces of equipment are kept free of an accumulation of dust, dirt, food residue, and other debris.

Violation: The deck/bulkhead juncture was soiled behind the tilting kettles closest to the tilting pans. It appeared to be a rust-like substance dripping from behind the profile strip.
Recommendation: Ensure that decks, bulkheads, deckheads, and attached equipment in food preparation, warewashing, pantries, and storage areas, are cleaned as often as necessary.

Violation: There was no coving at the deck/cabinet juncture at this waiter station in Rosie's Restaurant.
Recommendation: Ensure that bulkhead and deck junctures are coved (including galleys, pantries, deck/counter junctures at buffets, bars, waiter stations, and dining room work counters).

Violation: The handwash station had a low flow of water and when the water was turned on, the bucket fill station faucet would leak water.
Recommendation: Ensure that a plumbing system in a food area is maintained in good repair.
