Site: Recreational Water Facilities-Fecal Accident Plan
Violation: The disinfection policy and procedure (section 5.3.2) stated that staff were to use a 50 ppm halogen concentration solution for 4 hours contact time. This calculates to a CT value of 12000. The new CT value is 15300.
Recommendation: Ensure a contact time (CT value) of 15300 in the fecal accident plan.

Site: Potable Water-Cross-connection Control Program
Violation: There were four backflow prevention devices on the washing machines (2 hot/2 cold) in the deck 4 officers laundry. Two of the devices were Watts 9D and two were Watts N9. The backflow prevention device listing indicated that all four devices were Watts 9D.
Recommendation: Ensure that the cross-connection control program includes at minimum: a complete listing of cross-connections and the backflow prevention method/device for each, so that there is a match to the plumbing system component and location, and an inspection frequency. Ensure that air-gaps are included in the listing.

Violation: The backflow prevention device on the potable water line serving the decorative fountains on deck 15 were not on the cross-connection control program list.

Violation: Watts S8 type backflow prevention devices were listed as the backflow prevention devices for the spa steam generator aroma therapy units. There were Watts N9 devices installed on these units.

Site: Galley-Deck 14 - Galley Hot Preparation
Violation: There was a wire whisk that had a handle with an open end cap. The inside of the handle was rough and not easily cleanable.
Recommendation: Ensure that materials used in the construction of multiuse utensils and food-contact surfaces of equipment are: (1) Durable, corrosion-resistant, and nonabsorbent; (2) Sufficient in weight and thickness to withstand repeated warewashing; (3) Finished to have a smooth, easily cleanable surface; and (4) Resistant to pitting, chipping, crazing, scratching, scoring, distortion, and decomposition. Ensure that food contact surfaces of equipment and utensils are designed and constructed to be durable and to retain their characteristic qualities under normal use conditions.

Site: Galley-Deck 5 - Soup Station
Violation: The stainless steel wire strainer had missing/separating pieces and the decorative center piece had rough surfaces. These features made cleaning difficult.
Recommendation: Ensure that materials used in the construction of multiuse utensils and food-contact surfaces of equipment are: (1) Durable, corrosion-resistant, and nonabsorbent; (2) Sufficient in weight and thickness to withstand repeated warewashing; (3) Finished to have a smooth, easily cleanable surface; and (4) Resistant to pitting, chipping, crazing, scratching, scoring, distortion, and decomposition. Ensure that multiuse food-contact surfaces are: (1) Smooth; (2) Free of breaks, open seams >1 mm (1/32 inch), cracks, chips, inclusions, pits, and similar imperfections; (3) Free of sharp internal angles, corners, and crevices; (4) Finished to have smooth welds and joints; and (5) Accessible for cleaning and inspection by one of the following methods without being disassembled, by disassembling without the use of tools, or by easy disassembling with the use of handheld tools commonly available to maintenance and cleaning personnel such as screwdrivers, pliers, open-end wrenches, and Allen wrenches.

Violation: There were two white cuttings boards that were heavily scored.
Recommendation: Ensure that surfaces such as cutting blocks and boards that are subject to scratching and scoring are resurfaced if they no longer can be effectively cleaned and sanitized, or discarded if they are not capable of being resurfaced.

Violation: The water supply lines and power cables for the two juice machines were draped across the countertop, making cleaning difficult. The counter surface below the utility lines was soiled.
Recommendation: Ensure that nonfood-contact surfaces are free of unnecessary ledges, projections, and crevices, and designed and constructed to allow easy cleaning and to facilitate maintenance.

Violation: There was grease seeping from the seams of the deckhead, immediately outside the gazebo exhaust hood and in front of the rolling shutters protecting the grill and deep fat fryer. The area was covered with plastic wrap to contain the grease. There was also grease seeping from the deckhead access hatch for the rolling shutters. Additionally, there was an accumulation of grease along the rolling shutter door tracks, the bulkhead inside and outside the gazebo, and along the top of the gazebo entry door.
Recommendation: Ensure that decks, bulkheads, deckheads, and attached equipment in food preparation, warewashing, pantries, and storage areas, are cleaned as often as necessary.

Violation: There was grey water pooling on the deck and in some locations was approximately 3 inches deep. It appeared that the scupper drains for the flight-type warewash machine were plugged.
