1/30/11-2/6/11 - An additional case reported to medical after the 24 hour report, but more than 4 hours before entering a US port and no 4-hr report was submitted.

Provide a system in Medical to ensure that the correct number of gastrointestinal illness cases are reported.

Potable Water-Cabins with Whirlpool Tubs

Violation:
In the cabins with whirlpool tubs, there were two shower hoses with backflow prevention devices installed. Some of these hoses had ORAS vacuum breakers installed. It was unclear if all of the additional shower hoses were included in the cross-connection control program, but the program listed all shower hoses as having Watts S-8 devices.

Recommendation:
Update the cross-connection control program to include the correct backflow devices installed for the cabins with whirlpool tubs.

During the 2/6/11 - 2/16/11 cruise, there were two reportable gastrointestinal cases in the crew where the close contacts (cabin mates) were not monitored for 48 hours. The cabin mates were initially evaluated and isolated for 24 hours, but not checked again at 48 hours.

Recommendation:
Ensure that medical staff conduct daily verbal interviews with asymptomatic crew until 48 hours after onset of the ill crew members symptoms.

Except during preparation, cooking, or cooling, or when time is used as the public health control, ensure that potentially hazardous food is maintained: (1) At 60C (140F) [roasts 54C (130F)] or above, or (2) At 5C (41F) or less.
