Site: Medical-Medical Records
Violation: A musician reported an onset of signs and symptoms of Acute Gastrointestinal Illness (AGE) on 13 July 2011 at 12:01 AM on the Gastrointestinal Illness Surveillance Questionnaire. The musician met the case definition for AGE with three episodes of diarrhea, cramps, fever and muscle aches. The self-report form also indicated the musician ate in the crew mess on 13 July 2011 at 18:00 hours, suggesting the musician was not in isolation. According to medical records, the musician had an on-set of signs and symptoms on 15 July 2011 at 09:30.
Recommendation: Ensure that symptomatic non-food employees meeting the case definition for AGE are isolated in cabin or designated, restricted area until symptom-free for a minimum of 24 hours.

Violation: A seasonal youth staff member reported an on-set of signs and symptoms of AGE on 15 July 2011 at 0900. The seasonal youth staff member met the case definition for AGE with four instances of diarrhea, abdominal cramps, fever and muscle aches. The youth staff indicated on the Gastrointestinal Illness Surveillance Questionnaire that he ate meals between 15 July 2011 and 16 July in the crew mess. The youth staff member reported to medical on 16 July 2011 at noon.
Recommendation: Ensure that symptomatic non-food employees meeting the case definition for AGE are isolated in cabin or designated, restricted area until symptom-free for a minimum of 24 hours.

Site: Buffet-Horizon Court
Violation: Some of the platters of food on both the port and starboard buffet lines were missing the serving utensil.
Recommendation: Ensure that food-dispensing utensils are available for each container displayed at a consumer self-service unit such as a buffet or salad bar.

Site: Buffet-Horizon Court (Starboard)
Violation: The round plastic panel in the upper interior of the microwave near the utility sink was not secured. Staff replaced the missing fasteners, securing the panel flush with the top of the microwave.
Recommendation: Ensure the upper interior panel of the microwave is properly secure.

Violation: A gap was present along the length of the marble counter/stainless steel counter juncture on the port side, which made cleaning difficult.
Recommendation: Ensure that nonfood-contact surfaces are free of unnecessary ledges, projections, and crevices, and designed and constructed to allow easy cleaning and to facilitate maintenance.

Site: Food Service General-Dining Rooms
Violation: The wood panel on the front of some of the waiter station doors was cracked or broken off, exposing raw wood. This was noted in the Island, Coral, and Palm Dining Rooms.
Recommendation: Ensure that nonfood-contact surfaces are free of unnecessary ledges, projections, and crevices, and designed and constructed to allow easy cleaning and to facilitate maintenance.

Violation: The refrigerant lines in the technical compartment were wrapped with duct tape. There were ridges in the tape, making cleaning difficult.
Recommendation: Ensure that nonfood-contact surfaces of equipment that are exposed to splash, spillage, or other food soiling or that require frequent cleaning are constructed of a corrosion-resistant, nonabsorbent, and smooth material.

Violation: The electrical cord for the carving station was split open, exposing the wires.

Violation: The wood panel laminate on the doors at some of the beverage stations was peeling, making cleaning difficult.

Site: Buffet-Horizon Court (Starboard)
Violation: The inside of the technical compartment below the carving station was soiled with a sticky brown substance at the top of the compartment. The green electrical cord on the left of the compartment was also soiled with a dried brown substance.
Recommendation: Ensure that nonfood-contact surfaces of equipment are kept free of an accumulation of dust, dirt, food residue, and other debris.

Site: Provisions-Dry Store #3703
Violation: There was a gap along the back bulkhead at the deck/bulkhead profile strip juncture below the evaporator. A work order was placed the day of the inspection.
Recommendation: Ensure that decks, bulkheads, and deckheads in food preparation, warewashing, pantries, and storage areas, are maintained in good repair.

Violation: Sailcloth was used as a deckhead material immediately above the worker side of the bar.
Recommendation: Ensure that decks, bulkheads, and deckheads in food preparation, warewashing, pantries, and storage areas are constructed and maintained for easy cleaning.
