Site: Potable Water-Dialysis Machine
Violation: An atmospheric vacuum breaker backflow device was installed on the potable water supply where the dialysis machine connects in the medical center. Due to the higher health hazards associated with dialysis, a reduced pressure assembly or a pressure vacuum breaker device is required. No dialysis machines were connected or present during the inspection.
Recommendation: Install either a pressure vacuum breaker device or a reduced pressure assembly before the hemodialysis machine or before other related processes such as water treatment in order to protect the potable water system from contamination.

Site: Potable Water-Distallate and Permeate Water Lines
Violation: The distillate and permeate lines directed to the potable water system were not painted or striped in accordance with ISO 14726 (blue/gray/blue).
Recommendation: Stripe or paint distillate and permeate lines directed to the potable water system in accordance with ISO 14726 (blue/gray/blue). Ensure no other lines have the above color designations. Stripe or paint these lines at 5 meters (15 feet) intervals and on each side of partitions, decks, and bulkheads, except where decor would be marred by such markings, including potable water supply lines in technical lockers.

Site: Bar-Schooner Bar
Violation: Two carbonator backflow preventers had water leaking from the air relief holes. Rust had stained the chrome valves indicating this condition had existed for a long time. Maintenance was called immediately.
Recommendation: Maintain backflow prevention devices in good repair. Inspect backflow prevention devices periodically and replace any failed units.

Site: Recreational Water Facilities-Turnover Rates
Violation: Turnover rates for each of the recreational water facilities were not available.
Recommendation: For RWFs with skim gutters, ensure that the fill level of the RWF is to the level of the skim gutters. Design recirculation systems and equipment, including chemical control equipment, UV disinfection systems, filter, and pumps to maintain adequate water chemistry control while operating at the following minimum turnover rates: (1) swimming pool (VSP 2005 Construction Guidelines or earlier): 6 hours; (2) swimming pool (VSP 2011 Construction Guidelines or later): 4 hours; (3) children's pool: 0.5 hours; (4) wading pool: 1 hour; (5) whirlpool spa: 0.5 hours; (6) spa pool: 2 hours; (7) interactive RWF or activity pool less than 610 millimeters (24 inches) deep: 1 hour; (8) interactive RWF or activity pool greater than 610 millimeters (24 inches) deep: 2 hours; and (9) baby-only water facility: 0.5 hours. Ensure an RWF slide that is combined with a pool has a turnover rate that matches the rate for the pool.

Site: Recreational Water Facilities-Cartridge Filter Housing
Violation: Documentation was not available indicating that the cartridge filter housing for the recreational water facilities were cleaned, rinsed, and disinfected with a 50 ppm solution for one minute or equivalent CT value when new filter media is installed.
Recommendation: Clean, rinse, and disinfect the filter housing before the new filter media is placed in it. Ensure disinfection is accomplished with an appropriate halogen-based disinfectant. At a minimum, use a 50-ppm solution for 1 minute, or equivalent CT value. Maintain records on all inspection and cleaning procedures.

Site: Recreational Water Facilities-Bather Load
Violation: Documentation on maximum bather loads for each of the recreational water facilities were not available.
Recommendation: Maintain documentation on the maximum bather load for each RWF. Ensure the maximum bather load is based on the following factor: one person per five gallons (19 liters) per minute of recirculation flow.

Site: Recreational Water Facilities-Test Kit
Violation: A test kit capable of measuring total alkalinity was not available.
Recommendation: Ensure a test kit is available for testing water quality parameters, including free and total halogen levels (chlorine and bromine, where applicable), pH, and total alkalinity. Ensure that test kits are accurate to within 0.2 mg/L (ppm) for halogen and have a testing range of free residual halogen normally maintained in the RWF.

Site: Recreational Water Facilities-Halogen and pH Analyzer Probe Locations
Violation: The sample line locations for free halogen residual and pH monitoring for all recreational water facilities were located after the compensation tank.
Recommendation: Install chart recorders or electronic data loggers with security features that record pH and halogen measurements for each individual RWF. Ensure the sample line for the analyzer probe (monitoring) is either directly from the RWF or on the return line from each RWF and before the compensation tank. Install appropriate sample taps for analyzer calibration.

Site: Recreational Water Facilities-Shock Chlorination
Violation: The level of free chlorine residual was not recorded at the beginning and end of shock chlorination.
Recommendation: Increase the free residual halogen to at least 10.0 mg/L (ppm) and circulate for at least 1 hour every 24 hours. Test the free residual halogen both at the start and completion of shock halogenation. Superhalogenate the water in the entire RWF system to 10 ppm to include the whirlpool spa/spa pool tub, compensation tank, filter housing, and all associated piping before starting the 1-hour timing. Ensure a written or electronic record of shock halogenation (concentration in ppm at the start and completion and time) is available for review during inspections.

Site: Recreational Water Facilities-Safety Signage
Violation: The safety signage at all the recreational water facilities did not meet the current requirements of the 2011 VSP Operations Manual. New signs have been ordered.
Recommendation: Provide safety signs for all RWFs, except for baby-only water facilities. Ensure the signs, at a minimum, include the following words: (1) do not use these facilities if you are experiencing diarrhea, vomiting, or fever; (2) no children in diapers or who are not toilet trained; (3) shower before entering the facility; and (4) bather load #. For children's RWF signs, include the exact wording "TAKE CHILDREN ON FREQUENT BATHROOM BREAKS" or "TAKE CHILDREN ON FREQUENT TOILET BREAKS." In addition to the safety sign requirements in section 6.7.1.1.1, install a sign at each whirlpool spa and spa pool entrance listing precautions and risks associated with the use of these facilities. Include, at a minimum, cautions against use by the following: (1) individuals who are immunocompromised; (2) individuals on medication or who have underlying medical conditions such as cardiovascular disease, diabetes, or high or low blood pressure; and (3) pregnant women, elderly persons, and children. Additionally, caution against exceeding 15 minutes of exposure.

Site: Recreational Water Facilities-Antientrapment Drain Cover
Violation: The drain covers for the Main and Solarium swimming pools do not conform with ASME A112.19.8 standards. The drains were gravity, multiple, and greater than 3 feet apart with no alarm.
Recommendation: For RWFs with gravity drainage and multiple drains (2 or more drains greater than 3 feet apart), provide either a standard design (not compliant with ASME A112.19.8) drain cover and an alarm, or an ASME A112.19.8 compliant antientrapment/antientanglement drain cover. Ensure the alarm is audible and sounds in both a continuously manned space and at the RWF. Ensure this alarm is for all draining: accidental, routine, and emergency.

Site: Galley-Deck 4 - Potwashing
Violation: The 20 second final rinse cycle on the trolley potwash machine had steam emitting from the last part of the final rinse cycle rather than a hot water spray. Measurement by a thermolabel showed that the manifold temperature exceeded 180F. The final rinse gauge showed that the temperature exceeded 200F rather than a maximum of 194F as specified in the VSP Operations Manual.
Recommendation: In a mechanical operation, ensure the temperature of the fresh hot water sanitizing rinse as it enters the manifold is not more than 90C (194F) or less than 82C (180F).

Site: Galley-Deck 5 - Bakery
Violation: A wiping cloth sanitizing solution was found at 10 PPM. New 100 PPM solution replaced the solution in the bucket.
Recommendation: Ensure sanitizing solutions are used with the following concentrations: (1) A chlorine solution with a concentration between 50 mg/L (ppm) and 200 mg/L (ppm).

Site: Galley-Deck 4 - Pastry
Violation: A small floor mounted mixer that had been stored clean had dried batter on the wire type mixing blade.
Recommendation: Ensure food contact surfaces of equipment and utensils are clean to sight and touch.

Site: Preparation Room-Walk-In Cooler
Violation: A plastic trolley used for transporting containers of raw meat had meat residue splashed on the walls of the lower shelf compartment. The trolley was immediately removed for a more thorough cleaning and sanitization.
Recommendation: Ensure nonfood-contact surfaces of equipment are kept free of an accumulation of dust, dirt, food residue, and other debris.

Site: Galley-Deck 4 - Garden Manger
Violation: The hand soap dispenser valve was broken. Hand soap was seen in the unit through the open top. The entire dispenser was replaced before the inspection team left the area.
Recommendation: Ensure a handwashing facility includes a sink, soap dispenser, single-use towels dispenser, and waste receptacle.

Violation: A difficult to clean decorative coated covering was seen above the bar.
Recommendation: Ensure deckheads have smooth, hard finishes and light colored surfaces. Exception: bars may have decorative surfaces provided that they are easily cleanable.

Site: Other-Garbage Processing Room
Violation: The deck near the room's back bulkhead (under the large ducts and behind the cardboard bailing machine) had food debris and a large accumulation of dust indicating that it had been a long time since the area was cleaned. The ducts above were also soiled with old food spills.
Recommendation: Ensure decks, bulkheads, deckheads, and attached equipment in food preparation, warewashing, pantries, and storage areas are cleaned as often as necessary.

Site: Food Service General-Lighting
Violation: Ensure that there is at least 110 lux of light behind and around all counter and deck mounted equipment. In particular, lighting behind and around counter mounted equipment at the Windjammer beverage stations as well as behind and around deck mounted ice machines in the deck pantries.
Recommendation: Ensure the light intensity is at least 110 lux (10 foot candles) behind and around mounted equipment, including counter-mounted equipment.

Site: Galley-Windjammer Central Galley
Violation: The bain marie was at a high boil which caused steam to condense on the bottom of the shelf above it. Food in covered pans in the bain marie was not impacted.
Recommendation: Ensure ventilation hood systems and devices are sufficient in number and capacity to prevent grease or condensation from collecting on bulkheads and deckheads.

Site: Preparation Room-Centralized Potwashing
Violation: A mop was stored hanging on the edge of the mop sink with its handle extending across the adjacent handwashing sink. The mop was removed immediately and taken to the room's cleaning locker.
Recommendation: Ensure maintenance tools such as mops, brooms, and similar items are stored in a designated locker so they do not contaminate food, food-contact surfaces of utensils, and equipment, linens, and single-service and single-use articles. Ensure handwashing facilities are accessible at all times.

Site: Preparation Room-Commissary (Veg Prep) and Fish Prep
Violation: One small house fly was observed in the area of the prep table of the commissary (vegetable preparation). Another small fly was noted around the prep table in the fish prep room.
Recommendation: Effectively control the presence of insects, rodents, and other pests to minimize their presence in the food storage, preparation, and service areas and warewashing and utensil storage areas aboard a vessel.
