Violation: A REVIEW OF SOME OF THE GASTROINTESTINAL ILLNESS SURVEILLANCE LOGS AND THE 24-HOUR AND 4-HOUR REPORTS INDICATED THAT THE MEDICAL STAFF REPORTED REPORTABLE AND NON-REPORTABLE CASES OF ILLNESS. IN ONE ROUTINE REPORT, THE NUMBER OF CASES AMONG PASSENGERS INCLUDED A CASE THAT WAS NOT GASTROINTESTINAL ILLNESS AND THEREFORE, WAS NOT A REPORTABLE CASE TO THE VSP.
Recommendation: Include case counts for reportable cases of gastrointestinal illness only in the 24-hour and 4-hour surveillance reports.

Site: Potable Water-MICROBIOLOGICAL TESTING
Violation: THE SHIP BUNKERS POTABLE WATER REGULARLY IN NEW ORLEANS, BUT THE MOST RECENT MICROBIOLOGICAL ANALYSIS REPORT ONBOARD WAS AN ANNUAL SUMMARY OF WATER QUALITY FROM THE SEWERAGE AND WATER BOARD OF NEW ORLEANS FOR THE YEAR 2003. THE SHIP HAS AN ONBOARD MICROBIOLOGICAL TEST KIT WHICH IS APPROVED AND WILL PROVIDE ANALYSIS RESULT IN 24 HOURS, BUT IT IS ONLY USED FOR ANALYZING DISTRIBUTION SAMPLES FROM AREAS ON THE SHIP.
Recommendation: Where available, ensure that the vessel has a copy of the most recent microbiologic report from each port before bunkering potable water to verify that the water meets potable standards. In the case of unavailable recent reports for New Orleans or other ports, the ship should use the secondary option of conducting the E. coli test onboard with their existing test kit. For ports where the ship makes repeated sailings, the VSP considers one test per month, per port as a recent test.

Site: Potable Water-DECK C ENGINE ROOM - AC COMPRESSOR ROOM
Violation: THE HIGH SALINE OVERBOARD DISCHARGE LINES TO THE TWO EVAPORATORS WERE DOUBLE CHECK VALVE ASSEMBLY BACKFLOW PREVENTERS WITHOUT ATMOSPHERIC VENTS.
Recommendation: Replace the double check valve backflow preventers with reduced pressure assembly backflow preventers.

Violation: THE 4 CLOTHES WASHERS ON THE FORWARD BULKHEAD IN THE LAUNDERETTE HAD NON-CONTINUOUS PRESSURE (8S MODEL) BACKFLOW PREVENTERS INSTALLED. IT WAS ALSO NOTED THAT AT LEAST SOME OF THE BACKFLOW PREVENTERS FROM THE ORIGINAL INSTALLATION MAY ALSO HAVE BEEN OF A NON-CONTINUOUS PRESSURE TYPE (NLF9 MODEL) ON THE OTHER WASHING MACHINES.
Recommendation: Replace the backflow preventers with continuous pressure backflow preventers.

Site: Potable Water-DECK 11 BEAUTY SALON, CREW AND PASSENGER LAUNDERETTES
Violation: SOME OF THE BACKFLOW PREVENTION DEVICES INSTALLED ON THE SPRAY RINSE SINKS, WHIRPOOL FOOT BATH, AND CLOTHES WASHING MACHINES WERE NON-CONTINUOUS PRESSURE DEVICES (MODEL NLF9). IN EACH CASE THESE PREVENTERS WERE INSTALLED IN CONTINUOUS PRESSURE POSITIONS OF THE POTABLE WATER SUPPLY LINES. ADDITIONALLY, SOME OF THE DEVICES IDENTIFIED IN THE INSPECTION AND THE AIR-GAP PREVENTION PRESENTED IN VARIOUS APPLICATIONS (MINERALIZERS AND FILL LINE FROM EVAPORATORS TO POTABLE WATER).
Recommendation: The vessel shall provide a comprehensive cross- connection control program that provides safe connections to the potable water system through air gaps or appropriate backflow devices at the following locations, if present: (1) Potable water supply lines to swimming pools, whirlpool spas, hot tubs, bathtubs, showers, and similar facilities; (2) Photographic laboratory developing machines and utility sinks; (3) Beauty and barber shop spray-rinse hoses; (4) Potable water faucets where hoses are connected or can be connected by threaded or quick-connect outlets such as those serving tanks containing chlorine and other chemicals, and deck taps; (5) Garbage grinders and pulpers; (6) Mechanical warewashing machines; (7) Hospital and laundry equipment; (8) Air conditioning expansion tanks; (9) Boiler feed water tanks; (10) Fire systems; (11) Toilets; (12) Potable water, bilge, and sanitary pumps that require priming; (13) Freshwater or saltwater ballast systems; (14) Bilge or other waste water locations; (15) International shore connection; and (16) Any other connection between potable and non-potable water systems.

Site: Buffet-LIDO - CEZANNE GALLEY - COLD PANTRY
Violation: WALK-IN REFRIGERATOR #49 WAS POSTED WITH A TIME CONTROL PLAN AND STAFF STATED THAT ALL ITEMS HELD WITHIN WERE ON TIME ONLY AS A PUBLIC HEALTH CONTROL. THE TIME CONTROL PLAN SHEETS WERE COMBINATION PLAN AND TIME/TEMPERATURE LOG FORMS, BUT IN THE DESCRIPTION OF ITEMS UNDER CONTROL THE CREAM CHEESE, DELI MEATS, CUBED MELONS, AND WEDGE MELONS WERE LISTED WITH PLAN ELEMENTS. IN EACH CASE THE FOOD WAS LISTED AS IN STORAGE AT 41 °F UNTIL REMOVED TO BE PLACED ON THE SELF-SERVICE BUFFET LINE (APPROXIMATELY 15 MINUTES PRIOR TO SERVICE). WHEN REMOVED THE ITEMS WOULD BE PLACED ON TIME CONTROL. THE LOG SECTION OF THE TIME CONTROL PLAN LISTED A TIME CONTROL PERIOD OF 6 AM TO DISCARD AT 10 AM. ANOTHER PLAN FOR HACCP ON THE SAME ITEMS REVEALED A SIMILAR DESIGN, WHERE AFTER RECEIPT (TEMPERATURE CONTROLLED) FOODS WOULD BE PREPARED IN SMALL BATCHES, AND HELD UNDER TEMPERATURE CONTROL UNTIL PLACED OUT FOR SERVICE (TIME CONTROL BEGINS AT THE TIME FOODS WERE PLACED FOR SERVICE). THE WALK-IN CONTAINED 4 TROLLEYS FILLED WITH WEDGE CUT CANTALOUPE MELONS, A LARGE PLASTIC BIN HALF-FILLED WITH CREAM CHEESE, 4 TRAYS OF CUT CHEESE AND VARIOUS DELI MEATS, AND APPROXIMATELY 8 LARGE PANS FILLED WITH CUBE CUT HONEYDEW AND CANTALOUPE MELONS. ALL THE INDIVIDUAL CONTAINERS OF FOOD INSIDE WALK-IN REFRIGERATOR #49 HAD TIME LABELS AFFIXED WHICH LISTED PREPARATION DATE OF 16 APRIL AND DISCARD DATES OF 23 APRIL.
Recommendation: Using a walk-in refrigerator for a time only control system requires a detailed, written time control plan, with clearly detailed steps from preparation, cooling, transport, storage, and service. Temperature control must be documented and the point of time control should be labeled on the containers, as well as their discard time. Revise the written plan and all labels and log forms to be very specific about the potentially hazardous foods which will be stored in walk-in refrigerator #49.

Violation: THE SMALL, SELF-SERVICE SOUP KETTLE WAS OUTSIDE THE SNEEZE GUARD AND THE SOUP WAS UNPROTECTED FROM POTENTIAL CONTAMINATION FROM STAFF DURING SERVICE.
Recommendation: Provide a protective covering or sneeze guard for the soup kettle.

Violation: THREE LARGE PANS FILLED WITH CUBE CUT CANTALOUPE AND HONEYDEW MELONS HAD INTERNAL TEMPERATURES BETWEEN 45 AND 50 °F. THE PANS WERE REMOVED AND BLAST CHILLED. SEVERAL OTHER TROLLEYS AND PANS OF MELON, CREAM CHEESE, AND DELI MEATS/CHEESES WERE AT PROPER TEMPERATURE.
Recommendation: Ensure that equipment for cooling and heating food, and holding cold and hot food, is sufficient in number and capacity to maintain specified potentially hazardous food temperatures.

Violation: THE INTERIOR OF THE BLADE COVER TO THE BUFFALO CHOPPER WAS SCORED AND GOUGED, MAKING CLEANING DIFFICULT.
Recommendation: Adjust the blade on the buffalo chopper to prevent it from striking the lid when it is in operation. Smooth down the surfaces of the lid to return it to its easily cleanable characteristic.

ADDITIONALLY, THE FINAL UPPER SPRAY ARM HAD ONLY 5 OF THE 7 SPRAY NOZZLES PRODUCING WATER SPRAY.
Recommendation: Ensure that warewashing equipment is maintained in good repair and proper adjustment including: (1) Being maintained in a state of repair and condition that meets the standards of the materials, design, and construction of these guidelines; and (2) Maintaining water pressure, and water temperature measuring devices in good repair and ensuring their accuracy within the intended range of use.

Recommendation: Ensure that light bulbs are shielded, coated, or otherwise shatter-resistant in areas where there is exposed food, clean equipment, utensils, and linens, or unwrapped single-service and single-use articles.
