Site: Medical-CLINICAL SPECIMEN SUBMISSION
Violation: CLINICAL SPECIMEN COLLECTION CONTAINERS FOR BOTH VIRAL AND BACTERIAL AGENTS, AS WELL AS THE PROPER SHIPPING CONTAINERS AND LABELS WERE NOT ONBOARD THE VESSEL. NEW REQUIREMENT
Recommendation: Ensure the proper clinical specimen collection containers, as well as the proper shipping containers and labels are onboard the vessel.

Site: Potable Water-
Violation: THE VESSEL IS ONLY RECORDING THE DAILY COMPARISON OF CALIBRATION OF THE HALOGEN ANALYZER ON THE DISTRIBUTION SYSTEM IF CALIBRATION IS CONDUCTED. THERE IS NO DOCUMENTATION OF THE DAILY COMPARISON IF CALIBRATION IS NOT CONDUCTED.
Recommendation: Ensure that the halogen analyzer-chart recorder is properly maintained, operated, and calibrated daily in accordance with the manufacturer's instructions.

Violation: THE POTABLE WATER HOSES USED FOR BUNKERING ARE NOT LABELED "POTABLE WATER ONLY". THIS IS A NEW REQUIREMENT IN THE AUGUST 2005 VSP OPERATIONS MANUAL.
Recommendation: Ensure the bunkering hoses for potable water are labeled "POTABLE WATER ONLY."

Violation: THERE WAS NO WRITTEN DOCUMENTATION OF THE COATINGS USED IN POTABLE WATER TANKS. THIS IS A NEW REQUIREMENT IN THE AUGUST 2005 VSP OPERATIONS MANUAL.
Recommendation: Ensure the documentation for the coatings used in the potable water tanks is onboard the vessel.

Site: Buffet-DISH WASHING
Violation: THERE IS NO DATA PLATE ON THE FLIGHT-TYPE DISHWASHING MACHINE.
Recommendation: Ensure that warewashing machines are provided with an easily accessible and readable data plate affixed to the machine by the manufacturer that indicates the machine's design and operating specifications including the: (1) Temperatures required for washing, rinsing, and sanitizing; (2) Pressure required for the fresh water sanitizing rinse; and (3) Conveyor speed for conveyor machines or cycle time for stationary rack machines.

Violation: THE TECHNICAL SPACE OF THE JUICE MACHINE WAS SLIGHTLY SOILED WITH A BLACK MATERIAL.
Recommendation: Ensure that nonfood-contact surfaces of equipment are kept free of an accumulation of dust, dirt, food residue, and other debris.

Violation: THE LIGHTS IN THE FOOD PREPARATION AREA ARE NOT SHIELDED OR SHATTER RESISTANT.
Recommendation: Ensure that light bulbs are shielded, coated, or otherwise shatter-resistant in areas where there is exposed food, clean equipment, utensils, and linens, or unwrapped single-service and single-use articles.

Violation: THERE WAS NO COMPREHENSIVE WRITTEN INSPECTION, CLEANING, AND MAINTENANCE PLAN FOR THE HEATING, VENTILATION AND AIR-CONDITIONING SYSTEM. THE CURRENT PLAN ONLY COVERS THE FILTERS. THIS IS A NEW REQUIREMENT IN THE AUGUST 2005 VSP OPERATIONS MANUAL.
Recommendation: Ensure that vessels have a plan to inspect and maintain heating, ventilation, and air conditioning systems in accordance with manufactures recommendations and industry standards.

Site: Housekeeping-PUBLIC RESTROOMS
Violation: PERSONS EXITING THE TOILET ROOM ARE REQUIRED TO HANDLE THE DOOR WITH BARE HANDS. THIS IS A NEW REQUIREMENT IN THE AUGUST 2005 VSP OPERATIONS MANUAL.
Recommendation: Public toilet facilities shall be equipped so that persons exiting the toilet room are not required to handle the door with bare hands. This may be accomplished by methods such as locating paper towel dispensers at sinks and waste container near the door, installing mechanically operated door, door removal, or other effective means.
