Violation: SHIPPING CONTAINERS AND LABELS WERE NOT AVAILABLE FOR SHIPPING CLINICAL SPECIMENS.
Recommendation: 4.3.1.1-A supply of shipping containers and labels shall be maintained on the vessel for shipping clinical specimens.

Violation: AN ASSISTANT WAITER PRESENTED TO THE MEDICAL FACILITY ON 09/14/2005 WITH SYMPTOMS OF GASTROINTESTINAL ILLNESS. THIS INDIVIDUAL WAS EXCLUDED FOR 24 HOURS.
Recommendation: 7.2.2.1.1-A waiter is considered a food employee as defined by the 2005 VSP Operations Manual and as such must be excluded from work and must be symptom free for 48 hours before returning to work.

Violation: THE COOLING LOGS WERE NOT KEPT FOR 30 DAYS.
Recommendation: 7.3.5.2.6-Logs documenting cooling of potentially hazardous foods shall document the cooling steps outlined in 7.3.5.2.1 and 7.3.5.2.2. Logs documenting cooling shall be kept for 30 days.

Violation: THE FACE PLATE WAS MISSING ON THE BUFFET MOUNTED THERMOMETER, MAKING CLEANING DIFFICULT.
Recommendation: Replace the face plate on this unit. Ensure that nonfood-contact surfaces of equipment that are exposed to splash, spillage, or other food soiling or that require frequent cleaning are constructed of a corrosion-resistant, nonabsorbent, and smooth material. Ensure that nonfood-contact surfaces are free of unnecessary ledges, projections, and crevices, and designed and constructed to allow easy cleaning and to facilitate maintenance.

Violation: A PAPER LABEL WAS NOTED ON THE NON-FOOD CONTACT SIDE OF THE WHITE PLASTIC PANEL IN THE UPPER COMPARTMENT OF THE SODA MACHINE ICE MAKERS. THESE LABELS WERE WET AND DETERIORATED AND MADE CLEANING DIFFICULT.
Recommendation: Ensure that nonfood-contact surfaces of equipment that are exposed to splash, spillage, or other food soiling or that require frequent cleaning are constructed of a corrosion-resistant, nonabsorbent, and smooth material.

Violation: THE FRYER COILS WERE SLIGHTLY SOILED AT THE FRYER NEAR THE COMBINATION OVENS.

Violation: THE LIGHT BULB OVER THE COFFEE MACHINE WAS NOT SHIELDED OR SHATTER-RESISTANT.
Recommendation: Ensure that light bulbs are shielded, coated, or otherwise shatter-resistant in areas where there is exposed food, clean equipment, utensils, and linens, or unwrapped single-service and single-use articles.

Site: Housekeeping-PUBLIC TOILET ROOMS
Violation: SOME OF THE PUBLIC TOILETS ROOMS WERE NOT EQUIPPED TO ALLOW FOR HANDS FREE EXIT.
Recommendation: 9.1.1.1.3-Equip public toilet rooms to allow for hands free exit. Providing paper towel dispensers in the rooms with a waste receptacle near the toilet room doors would be acceptable to meet this requirement.
