Site: Potable Water-Testable Devices Records
Violation: Inspection records for all of the reduced pressure assembly devices indicated they were inspected and tested in October 2013. This was pointed out as an error and the correct date should have been in 2014.
Recommendation: Ensure records have the correct inspection date.

Site: Potable Water-Bunker Station Starboard
Violation: One of the chains on filling line caps was not attached.
Recommendation: Ensure the potable water filling line has a screw cap fastened by a noncorroding cable or chain to an adjacent bulkhead or surface in such a manner that the cap cannot touch the deck when hanging free. Ensure the hose connections are unique and fit only the potable water hoses.

Site: Potable Water-Tank Maintenance Records
Violation: The tank painting record on 13 August did not have a recorded cure time after the paint application. The staff provided a new form which included a space for this information and stated that future records will be complete.
Recommendation: Ensure the interior coatings on potable water tanks are approved for potable water contact by a certification organization. Follow all manufacturers' recommendations for application, drying, and curing. For the tank coatings used, maintain the following onboard: (1) written documentation of approval from the certification organization (independent of the coating manufacturer); (2) manufacturers' recommendations for application, drying, and curing; and (3) written documentation that the manufacturers' recommendations have been followed for application, drying, and curing.

Site: Recreational Water Facilities-Port Whirlpool Spa
Violation: The bromine level was measured three times at greater than 10 ppm with the vessel's and inspector's test kits. The analyzer for the whirlpool in the machinery room indicated a level of between 5 and 6 ppm bromine. Samples taken at the analyzer were also greater than 10 ppm bromine. The whirlpool was open at the time of the inspection.
Recommendation: Maintain a free residual halogen in recirculated RWFs in the following ranges: (6) whirlpool spas and spa pools, between 3.0 and 10.0 mg/L (ppm) free chlorine or between 4.0 and 10.0 mg/L (ppm) bromine.

Site: Recreational Water Facilities-Swimming Pool
Violation: The swimming pool had a decorative fountain with spraying jets that overflowed onto the beach level of the pool. The water for the fountain came directly from the pool. This RWF was considered an activity pool, but the turnover rate was 2.5 hours instead of 2 hours.
Recommendation: For RWFs with skim gutters, ensure that the fill level of the RWF is to the level of the skim gutters. Design recirculation systems and equipment, including chemical control equipment, UV disinfection systems, filter, and pumps to maintain adequate water chemistry control while operating at the following minimum turnover rates: (1) swimming pool (VSP 2005 Construction Guidelines or earlier): 6 hours; (2) swimming pool (VSP 2011 Construction Guidelines or later): 4 hours; (3) children's pool: 0.5 hours; (4) wading pool: 1 hour; (5) whirlpool spa: 0.5 hours; (6) spa pool: 2 hours; (7) interactive RWF or activity pool less than 610 millimeters (24 inches) deep: 1 hour; (8) interactive RWF or activity pool greater than 610 millimeters (24 inches) deep: 2 hours; and (9) baby-only water facility: 0.5 hours. Ensure an RWF slide that is combined with a pool has a turnover rate that matches the rate for the pool.

Site: Recreational Water Facilities-Pool/Filter Cleaning Records
Violation: There were no records stating the results of the monthly core samples for the sand filters for all recreational water facilities.
Recommendation: Examine granular filter media for channels, mounds, or holes. Inspect a core sample of the filter media for excessive organic material accumulation using a recommended sedimentation method. For whirlpool spas and spa pools, ensure inspections and sedimentation tests are done monthly. For all other RWFs, ensure inspections and sedimentation tests are conducted quarterly. Change granular filter media for whirlpool spas and spa pools based on the inspection and sedimentation test results or every 12 months, whichever is more frequent. For all other RWFs, change granular filter media based on the inspection and sedimentation results or per the manufacturer's recommendations, whichever is more frequent. Record the results of both the filter inspection and sedimentation test.

Site: Bar-Waves - Pool Bar
Violation: There was a dead fruit fly floating in the bottom of a partly filled bottle of a melon liquor on the back bar counter. Staff immediately discarded the contents of the bottle.
Recommendation: Ensure food is safe and unadulterated.

Site: Galley-Main Galley - Hot Service Line
Violation: There was a filled portable dispenser of waffle batter set in ice inside a loose pan on the serving line counter between uses. There was no discard time sticker affixed to the pan. Staff pointed out a discard label nearby on the bain marie overshelf as the label for this batter, but it was pointed out that the requirement is to label the food when applied to loose pans on counters.
Recommendation: Containers of potentially hazardous food under time control and placed on preparation counters must be labeled with the discard time, even if the outlet is open for less than 4 hours. (Clarifications to the VSP 2011 Operations Manual section 7.3.5.3.6)

Site: Galley-Main Galley - Dishwash
Violation: The in-use #1 and #2 conveyor dishwashing machines displayed problems within the final rinse compartment and the final rinse spray. There was a weak spray pattern in all the upper spray arm nozzles in dishwasher #1. In the #2 conveyor dishwasher the far left upper spray arm final rinse nozzle was mostly clogged, so there was only the smallest stream of water which could be seen spraying to the left or right side of the nozzle. Staff repaired both machines during the inspection.
Recommendation: Ensure a warewashing machine and its auxiliary components are operated in accordance with the machine's data plate and other manufacturer's instructions. Ensure a warewashing machine's conveyor speed or automatic cycle times is maintained accurately timed in accordance with manufacturer's specifications.

Site: Buffet-Staff Mess - Beverage Counter
Violation: There were visible milk solids on the interior surfaces of the small milk transfer tube inside the milk refrigerator of the previously cleaned espresso/cappuccino machine at the self-service beverage counter.
Recommendation: Ensure food only contacts surfaces of equipment and utensils that are cleaned and sanitized. Ensure food contact surfaces of equipment and utensils are clean to sight and touch.

Site: Other-Waves - Grill Section
Violation: The back grill counter had a portable panini shell grill set-up for service and very hot. The two grooved hot surfaces were heavily soiled with a layer of encrusted carbon material from past cooking.
Recommendation: Ensure food-contact surfaces of cooking equipment and pans are kept free of encrusted grease deposits and other soil accumulations. Ensure food only contacts surfaces of equipment and utensils that are cleaned and sanitized.

Site: Other-Waves - Ice Cream Section and Grill Section
Violation: There were some deckhead panels which were not fitted completely in the hanger system in place in the ice cream section so one end was not tight and had an open corner where the panel joined another. There were many more deckhead panels with the same condition in the grill section, where the panels were mis-aligned and a gap was present along a corner where two deckhead panels joined.
Recommendation: Ensure decks, bulkheads, and deckheads in food preparation, warewashing, pantries, and storage areas are maintained in good repair.

Violation: Some gaps were noted at panel junctures where some of the deckhead metal panels were not aligned together.

Site: Other-Waves - Ice Cream Section
Violation: There was a live filth fly circulating in this area during start of service and one live fruit fly on the center deckhead panel. There was also one dead filth fly loosely hanging from a deckhead panel directly above the ice cream display freezer at the front counter. Staff immediately removed the dead fly from the deckhead.
Recommendation: Effectively control the presence of insects, rodents, and other pests to minimize their presence in the food storage, preparation, and service areas and warewashing and utensil storage areas aboard a vessel.
