Site: Medical-Acute Gastroenteritis (AGE) Log
Violation: For the voyage ending on 15 March, the submitted 24-hour electronic medical report listed six reportable AGE cases instead of seven.
Recommendation: Ensure the number of AGE cases is accurately submitted.

Site: Medical-Immediate Contact Follow-up
Violation: The 48-hour follow-up interview was not documented for the cabin mate of a reportable acute gastroenteritis case crew member that reported to the medical center on 16 March.
Recommendation: When any crew member (food or nonfood employee) meets the case definition for AGE, take the following actions: (3) complete a verbal interview daily with medical or supervisory staff until 48 hours after the ill crew members' symptoms began. Ensure that the first verbal interview is conducted within 8 hours from the time the ill crew member initially reported to the medical staff. If the asymptomatic immediate contact or cabin mate is at work, ensure that he or she must is contacted by medical or supervisory staff as soon as possible. Document the date and time of verbal interviews.

Violation: A crew member reported to the medical center on 14 March after experiencing abdominal cramps and three episodes of vomiting. The crew member's medical records identified her as a reportable AGE case, but she was documented on the AGE log as nonreportable. Subsequently, the submitted 24-hour electronic medical report for the voyage listed six reportable AGE cases instead of seven.
Recommendation: Ensure the number of AGE cases on the AGE surveillance log is accurate.

Violation: The AGE log for the voyage ending on 1 March listed seven reportable AGE case passengers. The 4-hour electronic medical report submitted to VSP listed five reportable AGE case passengers. An additional passenger reported to the medical center with AGE symptoms after the 4-hour report was submitted, which brought the total number of reportable passenger cases to six. One passenger entry on the AGE log was listed as a nonreportable case, but appeared to be counted as a reportable case on the AGE log.
Recommendation: Ensure the number of reportable AGE case passengers and crew members are accurate on the AGE log and the electronic medical reports submitted to VSP.

Site: Medical-Acute Gastroenteritis (AGE) Log
Violation: The total number of passengers and/or crew members listed on the AGE logs did not match the numbers indicated on the electronic medical reports submitted to VSP. For the voyage ending on 15 March, the AGE log stated 3,000 passengers and 1,176 crew members, but the submitted 24-hour electronic medical report listed 3,348 passengers and 1,191 crew. According to medical staff, the 24-hour report contained the correct number of passengers and crew members.
Recommendation: Report the highest number of passengers and crew members that were onboard at any time during the cruise. Include those who signed on and signed off during the cruise, to include contractors, crew members joining/leaving during the cruise, and passengers who depart or join the cruise other than on turn-around day. In the event of an outbreak, the highest number of passengers and crew members onboard during the cruise make up the cohort who may have been exposed to the pathogen.

Site: Medical-Immediate Contacts
Violation: Medical staff was asking reportable AGE case crew members if they had additional immediate contacts (i.e. boyfriend, girlfriend, spouse, etc.) besides cabin mates, but it was not documented if they did not.
Recommendation: Document if there are no additional immediate contacts.

Site: Potable Water-Bunker Pretest
Violation: According to staff, the initial shore side chlorine and pH values were taken before bunkering commenced, but were not documented.
Recommendation: Conduct a free halogen residual and pH test on the shore-side water supply before starting the potable water bunkering process to establish the correct halogen dosage. Record the results of the pretest and ensure they are available for review during inspections.

Site: Potable Water-Hot Water Laundry Line
Violation: Water was continuously leaking from the relief vent on the reduced pressure assembly backflow prevention device installed on hot water laundry fill line.
Recommendation: Maintain backflow prevention devices in good repair.

Site: Potable Water-Deck 4 Zone 7 Starboard Cart Wash
Violation: The list of testable backflow prevention devices indicated one testable device installed on the two chemical dispensing connections in the cart wash room. Upon inspection the two devices installed were atmospheric vacuum breakers. The atmospheric vacuum breaker on the left connection was connected to a hose with a spray nozzle on the end.
Recommendation: Update the list of backflow prevention devices. Ensure the devices are appropriate for any installed connections.

Site: Recreational Water Facilities-Hair and Lint Strainer Disinfection
Violation: The hair and lint strainer and housing for all RWFs were not cleaned and disinfected weekly dating back to 14 January. The recorded dates were 11 March, 3 March, 23 February, 11 February, 1 February, 23 January, and 14 January.
Recommendation: Clean, rinse, and disinfect the hair and lint strainer and hair and lint strainer housing on all RWFs weekly. Ensure disinfection is accomplished with an appropriate halogen-based disinfectant. At a minimum, use a 50-ppm solution for 1 minute, or equivalent CT value. Maintain records on all inspection and cleaning procedures.

Site: Other-Officer's Mess Pantry
Violation: A crew member was observed rinsing his sanitizing cloth from the handwash sink faucet at the same time another crew member was filling a sanitizing container with water from the bucket fill tap.
Recommendation: Ensure the handwash sink is used for no other purpose.

Site: Galley-Deck 5 - Hot Galley
Violation: The cooling log documented beef stock was cooled starting at 14:01 with a temperature of 146F, followed by 15:56 with a temperature of 68F. No further temperatures were documented to indicate the food was cooled to 41F or below and no notes for this item were on the log. The preparer explained the food was reheated for immediate service.
Recommendation: Ensure the cooling temperatures of cooked potentially hazardous foods are documented. Potentially hazardous foods shall be cooled from 57C (135F) to 21C (70F) or less within 2 hours and from 21C (70F) to 5C (41F) or less within an additional 4 hours.

Site: Galley-Deck 15 - Cafe Caribe
Violation: Inside the food equipment locker, the teflon on a skillet was peeling along the edges and in the center. The pan was removed to be taken to the incinerator.
Recommendation: Materials used in the construction of multiuse utensils and food contact surfaces of equipment must be: (2) sufficient in weight and thickness to withstand repeated warewashing; (3) finished to have a smooth, easily cleanable surface.

Site: Galley-Deck 6 - Midship Soup Station
Violation: The food splash area of the buffalo chopper lid was cracked. Staff showed the inspector a new lid was available to replace immediately.
Recommendation: Design and construct food contact surfaces of equipment and utensils to be durable and to retain their characteristic qualities under normal use conditions.

Site: Galley-Crown Grill Show Galley
Violation: Inside the grease tray housing of the flat and grooved grills, excess silicone was installed around the chute. Staff immediately replaced the silicone.
Recommendation: Ensure nonfood-contact surfaces of equipment that are exposed to splash, spillage, or other food soiling or that require frequent cleaning are constructed of a corrosion-resistant, nonabsorbent, and smooth material.

Site: Galley-Horizon Court - Dishwash Area
Violation: The far right final rinse sanitizing spray nozzle was blocked. A crew member removed the final rinse spray arm and high pressure sprayed the nozzles followed by sanitizing the spray arm and nozzles. This corrected the issue.
Recommendation: Maintain warewashing equipment in good repair and proper adjustment, including: (1) warewashing equipment is maintained in a state of repair and condition that meets the standards of the materials, design, and construction of these guidelines.

Site: Buffet-Horizon Court Gazebo
Violation: During the active lunch service, the container of sanitizing solution had an oily residue on the surface. The solution was immediately remade.
Recommendation: Restrict wiping cloths to the following: (4) Wet wiping cloths used with a freshly made sanitizing solution and dry wiping cloths are free of food debris and visible soil.

Violation: In the food splash area of the iced tea machine, residue was inside the dispensing ports and on the underside panel. The machine had been previously cleaned and had not been in operation since the previous evening.

Violation: Aft of the cold pantry, inside the lower in-use ice machine compartment, a layer of brown material was on the water collection tray. Staff explained this was rust from the small pump. The machine was cleaned and sanitized in January 2015.

Violation: Between most of the deckhead panels, condensation droplets formed and some were observed dripping onto the deck. Staff stated that insulation and materials for the condensation and cooling lines were scheduled to be delivered in a few months. As a temporary solution, cardboard sheets were placed on top of the boxes to protect the beverages. A few small wet areas of cardboard were observed, indicating leaking condensation.
Recommendation: Repair the insulation on the condensation and cooling lines to prevent leaking through the deckhead panels.

Violation: Eight bottles of facial misting spray were stored in the walk-in refrigerator. The bottles were immediately removed.
