Site: Medical-Crew Member Late Gastrointestial Illness (GI) Reporting
Violation: A youth activity counselor experienced a GI onset at 0700 on 16 March, and did not report to medical until 1552 that day. The crew member worked from 0900 to 1300 this day, and ate breakfast in the crew mess. Staff showed documentation that the crew member received disciplinary action.
Recommendation: When nonfood employees meet the case definition for AGE, ensure the following actions are taken: (1) isolate in cabin or designated restricted area until symptom-free for a minimum of 24 hours; (2) follow-up with and receive approval by designated medical personnel before returning crew to work; (3) document the date and time of last symptom and clearance to return to work.

Violation: A rolling bin of rice had a pan for a bin lid that did not provide complete protection to the uncooked rice. Plastic wrap was placed over the opening.
Recommendation: Protect food from contamination by storing the food: (1) Covered or otherwise protected.

Violation: The ice bin was left open on the front bar with the ice scoop stored in a manner that prevented the sliding bin to be closed.

Site: Preparation Room-
Violation: Plastic blade guide for the meat saw was worn and difficult to clean. A new blade guide was installed before the inspection team left the butcher shop.
Recommendation: Food contact equipment must be maintained in good repair and proper adjustment: (1) Equipment must be maintained in a state of repair and condition that meets the materials, design, construction, and operating specifications of these guidelines.

Site: Galley-Officer Galley
Violation: A half full garbage can with food waste was found in the area that was not in service at the time of the inspection. The lid in place had holes in it for the waste disposal, but did not provide complete isolation of the waste from flies and other vermin.
Recommendation: Cover receptacles and waste-handling containers when not in continuous use and after they are filled.

Site: Galley-Cold Galley
Violation: Walk-in Cooler #29 door gasket was loose. This was repaired while the inspection team was in the Cold Galley.
Recommendation: Ensure bulkheads in food storage areas are maintained in good repair.

Violation: The drain lines on two evaporator units in the walk-in coolers had gaps in the bulkheads where the lines passed through.

Site: Food Service General-Galleys and Pantrys
Violation: Substantial work has been completed to make the decks in food preparation areas of the galleys and pantries smooth and easily cleanable. Work is quickly progressing to eliminate these problem decks and scuppers in the crew galley, and other food and beverage areas.
Recommendation: Ensure decks in food areas are constructed and maintained for easy cleaning.

Site: Provisions-Dry Stores
Violation: Two light shields had been removed for light maintenance and had not been replaced. The shields, which were still in the room, were installed before the inspection team moved to the next area.
Recommendation: Ensure light bulbs are shielded, coated, or otherwise shatter-resistant in areas where there is exposed food.

Violation: One fruit fly was observed in the operating food preparation area. The fruit fly was eliminated.
