Site: Medical-24-Hour Report Confirmation
Violation: The ship had documentation that the 24-hour report was sent via email to VSP before arrival in Ketchikan, AK (24 May) and Sitka, AK (14 May), but no confirmation that the report was received by VSP.
Recommendation: Submit routine 24-hour and 4-hour update reports electronically and ensure that the vessel maintains proof onboard that the report was successfully received by VSP.

Site: Potable Water-Garbage Room
Violation: The potable water supply lines to the food waste system were not striped blue or blue/green/blue before both reduced pressure backflow prevention assemblies.
Recommendation: Stripe or paint potable water lines either in accordance with ISO 14726 (blue/green/blue) or blue only.

Site: Potable Water-Portside Evaporator
Violation: A double-check valve backflow prevention device was installed on the high salinity discharge line. This device did not have a relief vent and was not accepted by VSP.
Recommendation: Provide a comprehensive cross-connection control program. Protect the following connections to the potable water system against backflow with air gaps or mechanical backflow prevention devices: (24) high saline discharge line from evaporators (install an air gap or a reduced pressure assembly).

Site: Potable Water-Cross-Connection Control Program
Violation: The cross-connection control log listed two testable devices with a plumbing connection of 'boiler potable water.' Upon inspection it was determined the devices were for the high salinity overboard discharge lines.
Recommendation: Ensure the cross-connection control program includes at a minimum: a complete listing of cross-connections and the backflow prevention method or device for each, so there is a match to the plumbing system component and location. Include air gaps on the listing. Ensure a schedule for inspection frequency is set. Maintain a log documenting the inspection and maintenance in written or electronic form, and ensure it is available for review during inspections.

Violation: On all of the lunch and dinner menus, the consumer advisory statement was written in a small font that was difficult to read.
Recommendation: Increase the font of the consumer advisory statement so it is easier to read.

Violation: At the salad station, some breads out for passenger self service were in front of the sneeze shield and not protected from contamination.

Violation: An unused deck stand was resting on its side with the feet in contact with a box of beef.

Violation: A cloth was stored on the top opening of a box of lettuce. The cloth was in contact with the lettuce. The cloth was immediately removed from the room.
Recommendation: Protect food from contamination by storing the food: (3) Where it is not exposed to splash, dust, or other contamination.

Site: Food Service General-Ice Machines
Violation: A slotted fastener was in the splash area of the ice thickness sensor inside all ice machines. Staff showed the inspector a purchase order placed on 12 May for many nonslotted fasteners to be delivered on 5 June.
Recommendation: Use only low profile, nonslotted, noncorroding, and easy-to-clean fasteners on food-contact surfaces and in splash zones. Prohibit the use of exposed slotted screws, Phillips head screws, or pop rivets in these areas.

Violation: Electrical cords for the two coffee machines were draped across the counter, making cleaning difficult.
Recommendation: Elevate the electrical cords above the counter.

Violation: At the ice cream station, the bucket of sanitizing solution had an oily film on the top.
Recommendation: Ensure the wash, rinse, and sanitize solutions are maintained clean.

Violation: A self-serve water dispenser was located in the dining area. Old juice soil was inside the water chute. Staff stated the machine was last used during the breakfast service, about three hours before the area was inspected.
Recommendation: Ensure food contact surfaces of equipment and utensils are clean to sight and touch.

Site: Other-Food and Beverage Replaceables (Deck 2 Forward)
Violation: Some food and beverage equipment items were stored on the deck. This room was used to store new, unused food and beverage equipment received onboard prior to issuing to their designated area.
Recommendation: Store cleaned equipment and utensils, laundered linens, and single-service and single-use articles: (3) at least 150 millimeters (6 inches) above the deck.

Violation: A green scrub pad was inside the handwash station. The area was not in operation during the inspection.
Recommendation: Ensure handwashing facilities are used for no other purpose and are accessible at all times.

Violation: Outside the main galley, the entrance door to the food worker men's toilet room was not self-closing.
Recommendation: Ensure toilet rooms are completely enclosed and have tight-fitting, self-closing doors which are kept closed except during cleaning or maintenance.

Violation: At the clean end of the dishwash machine, the deck grouting was missing and recessed along the deck/bulkhead juncture.
