Violation:
A nonfood worker had four vomiting episodes, muscle aches, and a headache on July 26, 2013 at 2030, but did not report until July 27 at 0840. According to the documentation, this individual did not work while sick.

Recommendation:
Crew members should report to the medical center immediately after their first acute gastroenteritis (AGE) symptom. When any crew member (food or nonfood employee) meets the case definition for AGE, take the following actions: (1) restrict exposure to symptomatic crew member(s); (2) undergo a verbal interview with medical or supervisory staff, who will confirm their condition, provide facts and a written fact sheet about hygiene and handwashing, and instruct them to report immediately to medical if they develop illness symptoms; (3) complete a verbal interview daily with medical or supervisory staff until 48 hours after the ill crew members? symptoms began. Ensure that the first verbal interview is conducted within 8 hours from the time the ill crew member initially reported to the medical staff. If the asymptomatic immediate contact or cabin mate is at work, ensure that he or she must is contacted by medical or supervisory staff as soon as possible. Document the date and time of verbal interviews.

A nonfood worker had five episodes of diarrhea and vomiting on August 3 and was documented as a reportable case, but there were no close contact interviews recorded. According to the staff, this individual did not share a room and did not have any close contacts.

Recommendation:

Violation:
On August 6, a nonfood worker had two episodes of diarrhea and stomach cramps, but was recorded as a nonreportable case on the AGE log. This individual was given antidiarrheal medication and was isolated for 24 hours. The underlying illness column on the AGE surveillance log stated 'nil of note'. The medical records indicated one loose stool for this individual was normal, but according to staff, two loose stools was above normal for this individual.

Recommendation:
Ensure the AGE surveillance log entry for each crew member contain the following information in separate columns: (13) illness symptoms, including the presence or absence of the following selected signs and symptoms, with a separate column for each of the following: (a) number of episodes of diarrhea in a 24-hour period; (b) number of episodes of vomiting in a 24-hour period; (c) bloody stools; (d) recorded temperature; (e) abdominal cramps; (f) headaches; (g) muscle aches; (14) entry (yes/no) for whether this was a reportable case; (15) entry (yes/no) for antidiarrheal medications sold or dispensed by designated medical staff; (16) presence of underlying medical conditions that may affect interpretation of AGE.

The posted time control plan specified that color dots were used for labeling food being held under the ship's time control procedures. The operation had shifted to a set-up time and discard time label for the pizza ingredients and finished pizzas being held for service to passengers. The old plan was removed and changed to reflect the current practice being used in this area.

Maintain a written time control plan(s) that ensures compliance with these guidelines on the vessel and make it available for review during inspections. Post a time control plan at each outlet where time control is used. There were two separate undercounter refrigerators in each of these lines and only one of the two refrigerators in each area had a label. The time control plans for these areas only listed the one number for two units on each line, but according to the technical staff, all of the undercounter refrigerators were separate units. All of the units were labeled for time control.

The inspections of the incoming food supplies between 6/21-8/23 could not be found in the ship's integrated pest management records. Deck and hotel supplies delivered weekly on these dates were in the binder. The missing records were later delivered to the inspection team.

Recommendation:
Routinely inspect incoming shipments of food supplies for evidence of insects, rodents, and other pests. Maintain a record of these inspections onboard the vessel and make the record available for review during inspections.

Violation:
The small toys and pieces of toys that are cleaned in the dishwashers were very clean. The larger toys that are cleaned by wiping have many areas where soil and dust are accumulating in the crevices and rough surfaces, such as between and around the letters where the alphabet letters are embossed and slider pieces are mounted.

Recommendation:
Ensure toys used in the child-activity center are maintained in a clean condition.
