Recommendation:
The underlying illness column for each AGE log entry must always been completed. If no underlying illness is present then "none" or an equivalent term must be written in this column. Any additional information can be place in this column only after the underlying illness question is addressed.

The patient case file and the summary page did not always match regarding isolation times. For example, a crew member's file stated the last symptom was on 9/30 at 17:30 and he/she was released from isolation on 10/2 at 17:30. This crew member was a food worker and met the 48-hour isolation requirement. However, the summary page for this case indicated the isolation ended on 10/2 at 7:29, which is less than 48-hours from last symptom. Medical staff was aware of this issue and had already contacted corporate to correct.

Recommendation:
Ensure all sections of the electronic reporting system contain the summary information for each case.

A test case was entered into the AGE log for the voyage from 9/13-9/23 by the corporate office. This case was never removed from the AGE log and was counted as a reportable case in the submitted 24 hour report. The AGE log listed 17 ill passengers for the voyage when the total was actually 16.

Recommendation:
Ensure only actual reportable AGE cases are counted for the total number of ill passengers and/or crew on the AGE log and 24 hour report.

A passenger reported to medical on 9/3 with one loose stool and informed staff this was above normal for him. 'Above normal for individual' was listed in the underlying illness column. This case was listed as nonreportable on the AGE log for the voyage.

Recommendation:
Ensure a reportable case of AGE is a case reported to the master of the vessel, the medical staff, or other designated staff by a passenger or a crew member and meets the case definition based on the following symptoms: (1) Diarrhea (three or more episodes of loose stools in a 24 hour period or what is above normal for the individual. See section 4.1.2.1.3, illness symptoms); or (2) vomiting and one additional symptom including one or more episodes of loose stools in a 24-hour period, or abdominal cramps, or headache, or muscle aches, or fever (temperature of greater than or equal to 38°C [100.4°F]).

The newer style juice machines had old juice residue in most of the small tight areas to the right and left of the closed technical compartments at the front of the machine.

Recommendation:
Ensure nonfood-contact surfaces of equipment are kept free of an accumulation of dirt, food residue, and other debris. Ask the company service technicians to open these closed compartments for the ship staff to observe for possible cleaning problems in these areas and provide the ship with recommendations on how to prevent the buildup of soil in the areas and clean these areas when soil is found.

The preparation area handwashing sink automatic water valve control would not activate when the inspection team entered the area. The sink started working immediately when the engineer examined it.
Another handwashing sink was available around the corner in the pizza preparation station. A crew member at the waffle station was using the adjacent preparation sink for handwashing. No food preparation activities which required the use of this sink were occurring at this time.

The rat guards on the aft mooring lines were not properly installed and were ineffective. One rat guard was used to cover multiple mooring lines which left multiple openings were rats could enter. Other mooring lines did not have any rat guards.
