Item No.: 01
Site: Medical-Acute Gastrroenteritis (AGE) Reporting
Violation: The 24-hour report sent before arrival to San Diego on 28 April was sent on 25 April, more than 36 hours prior arrival.
Recommendation: Ensure the master, medical staff, or other designated staff of a vessel destined for a U.S. port from a foreign port submits at least one standardized AGE report based on the number of reportable cases in the AGE log to VSP no less than 24 hours-but not more than 36 hours-before the vessel's expected arrival at the U.S. port.

Site: Potable Water-Microbiological Testing
Violation: The color comparator for analyzing results of the potable water testing for microbiological monitoring expired on November 2013.
Recommendation: If water samples are collected and analyzed by the vessel for the presence of E. coli, analyze the samples using a method accepted in Standard Methods for the Examination of Water and Wastewater. Ensure test kits, incubators, and associated equipment are operated and maintained in accordance with the manufacturers' specifications.

Site: Potable Water-Cross-Connection Control Program
Violation: The atmospheric vacuum breaker next to the far point analyzer was not installed at least 150 millimeters (6 inches) above the fixture.
Recommendation: Install the device according to manufacturer's instructions.

Site: Potable Water-Cross-Connection Control Program
Violation: The list of backflow preventers had the hot and cold lines supplying potable water to the infirmary as double check valve assemblies Wilkins 950. Inspection found that the actual backflow preventers installed in this location were reduced pressure assemblies Wilkins 975 XL.
Recommendation: Update the cross-connection control program.

Site: Potable Water-Cross-Connection Control Program
Violation: The list of backflow preventers documented annual testing that did not seem accurate for the reduced pressure assemblies (RPz) installed on the supply lines to the infirmary. The list documented annual testing as Test 1: 5 psi and Test 2: 5 psi. The same pressure differentials were documented for the tests on November 2013 and December 2012. Upon review of the manufacturer's instructions for testing these RPz, actually three tests had to be performed and documented: Test 1: tightness against reverse flow, Test 2: record pressure differential, and Test 3: recorded pressure differential had to be at least 2 psi less than the supply pressure.
Recommendation: Test the devices according to the manufacturer's instructions.

Site: Potable Water-Disinfection
Violation: Records documented that during the October 2013 dry dock, potable water pipelines were replaced for decks 1, 2, 6, and 7 and that from December 2013 to February 2014 technical water lines to the AC Stations were converted to potable water lines. The pipelines were disinfected and flushed as required, but the actual value of chlorine residual before the system was placed back in service was not documented.
Recommendation: Flush the disinfected parts of the system with potable water or otherwise dechlorinate until the free residual halogen is less than or equal to 5.00 mg/L (ppm). Document the free halogen test result.

Violation: Potable water tank 50C was disinfected and flushed on July 2013 as required, but the actual value of chlorine residual before the system was placed back in service was not documented.

Violation: The backflow prevention devices on both pre-rinse sprayers were heavily soiled with food residue that clogged the atmospheric vents.
Recommendation: Maintain backflow prevention devices in good repair.

Site: Galley-Unrestrained Mustaches
Violation: Approximately 4 food workers had pronounced mustaches and were not wearing hair restraints.
Recommendation: Ensure food employees wear hair restraints and clothing that covers body hair. Ensure these items are designed and worn effectively to prevent hair from contacting exposed food; clean equipment, utensils, and linens; and unwrapped single-service and single-use articles.

Site: Galley-Pinnacle Grill
Violation: The microwave oven was mounted to the counter with less than a 4 inch clearance.
Recommendation: Ensure table-mounted equipment that is not easily movable is elevated on legs that provide at least a 100 mm (4-inch) clearance between the table and the equipment.

Site: Galley-Clean Plate Storage Area
Violation: Clean plates stored on the main shelves were not covered or inverted.
Recommendation: Store clean equipment and utensils in a self-draining position that allows air drying, and covered or inverted.

Site: Galley-Starboard Bistro Entry
Violation: A dispensing station for Oxivir Five 16 was connected to the handwash station and mounted on the adjacent cleaning materials locker.
Recommendation: Ensure only poisonous or toxic materials that are required for the operation and maintenance of a food area of the vessel, such as for the cleaning and sanitizing of equipment and utensils and the control of insects and rodents, are allowed in the food areas of the vessel.

Site: Galley-Scupper Drains
Violation: The grouting around most of the scupper lines were either recessed or missing. Standing water was present in these areas. Specific areas included: the hot galley, around both ice machines, the pot wash area, and the breakfast pantry.
Recommendation: Ensure decks, bulkheads, and deckheads in food preparation, warewashing, pantries, and storage areas are maintained in good repair.

Site: Dining Room-Rotterdam Storeroom P4.19
Violation: Food service equipment was stored in this room with a deckhead that had exposed wiring and ventilation ducts.
Recommendation: Ensure decks, bulkheads, and deckheads in food preparation, warewashing, pantries, bars, and food and equipment storage areas are constructed and maintained for easy cleaning. Do not use carpet in these areas.

Violation: The sanitizing sink of the 3-compartment sink had a small leak that caused a significant amount of water to accumulate on the deck.
Recommendation: Ensure a plumbing system in a food area is maintained in good repair.

Site: Buffet-Neptune Lounge
Violation: The light intensity at the refrigerated dome used for customer self-service was less than 220 lux.
Recommendation: Ensure the light intensity is at least 220 lux (20 foot candles) on food preparation surfaces, and at a distance of 75 centimeters (30 inches) above the deck in food preparation areas, handwashing facilities, warewashing areas, equipment, and utensil storage, pantries, toilet rooms, and consumer self-service areas.

Site: Buffet-Officer's Mess Beverage Station
Violation: The light intensity behind and around the counter-mounted ice machine, coffee machine, and juice machine was less than 110 lux.
Recommendation: Ensure the light intensity is at least 110 lux (10 foot candles) at a distance of 75 centimeters (30 inches) above the deck when in use, in walk-in refrigerator units and dry storage areas, and in other areas and rooms during periods of cleaning. Ensure the light intensity is at least 110 lux (10 foot candles) behind and around mounted equipment, including counter-mounted equipment.

Violation: A jacket was hanging on a hook on the bulkhead.
Recommendation: Ensure only articles necessary for the food service operation are stored in food preparation, food storage, and warewashing areas.
