Site: Bar-Pool Bar
Violation: A carbonator backflow preventer in the left beverage system cabinet in the pantry had failed. Carbon dioxide and syrup were leaking from the atmospheric vent. The vent nozzle was rusted indicating a longstanding failure of this unit. The carbonator in the right beverage system cabinet had a vent nozzle connected to a long plastic tube that disappeared in a hole in the rear bulkhead. This installation does not allow for detection of a possible malfunction of this device because the discharge end cannot be observed.
Recommendation: Inspect backflow prevention devices periodically and replace any failed units. Ensure backflow prevention devices and air gaps are accessible for inspection, service, and maintenance.

Site: Potable Water-Production System Piping
Violation: The distillation and the permeate lines were not striped blue-gray-blue prior to the chlorination point. Some of the piping had been striped appropriately since the last inspection, but additional work is required to complete the compliance work.
Recommendation: Stripe or paint potable water lines either in accordance with ISO 14726 (blue/green/blue) or blue only. Stripe or paint distillate and permeate lines directed to the potable water system in accordance with ISO 14726 (blue/gray/blue). Stripe or paint these lines at 5 meters (15 feet) intervals and on each side of partitions, decks, and bulkhead.

Site: Provisions-Milk/Eggs Walk-in Refrigerator 28519
Violation: The temperatures of two milk cartons were measured as 45F and 46.8F by the crew and inspector. The box containing the milk cartons was immediately discarded. The inspector was told the refrigerator was in a defrost cycle.
Recommendation: Maintain potentially hazardous foods at 5C (41F) or less.

Site: Galley-Ice Machines
Violation: Some of the ice sensor probes had slotted fasteners. Water splash was observed on the front exterior of one of the ice machine probes around the slotted fastener. Management informed the inspector a work order was in place to replace all the slotted fasteners.
Recommendation: Ensure multiuse food-contact surfaces are: (5) Accessible for cleaning and inspection by one of the following methods without being disassembled, by disassembling without the use of tools, or by easy disassembling with the use of handheld tools commonly available to maintenance and cleaning personnel. Use only low profile, nonslotted, noncorroding, and easy-to-clean fasteners on food-contact surfaces and in splash zones. Prohibit the use of exposed slotted screws, Phillips head screws, or pop rivets in these areas.

Site: Galley-Soup Station
Violation: A previously cleaned piece of the blender equipment was heavily soiled with food residue.
Recommendation: Ensure food contact surfaces of equipment and utensils are clean to sight and touch.

Site: Preparation Room-Handwash station
Violation: Over 20 small ants and one live fly were observed on and around a mixer adjacent to the handwash station. The mixer was heavily soiled in the crevices of the machine and ants were observed crawling in these crevices. The equipment was soiled with over a day's accumulation of debris. This area was in operation.
Recommendation: Ensure nonfood-contact surfaces of equipment are kept free of an accumulation of dust, dirt, food residue, and other debris.

Site: Galley-Potwash Machine
Violation: The water from the handwash station, adjacent to the potwash machine, was very hot and this temperature was not adjustable by the user. The inspector and crew measured the temperature to be 124F. This item was corrected on site.
Recommendation: For handwash sinks with electronic sensors, and other types of handwash sinks where the user cannot make temperature adjustments, ensure the temperature provided to the user after the mixing valve does not exceed 49C (120F).

Site: Bar-Pool Bar
Violation: The water at the handwash sink adjacent to the pantry dishwashing area measured 128F. This temperature was not adjustable by the user. This was repaired while the team was still in the area.
Recommendation: For handwash sinks with electronic sensors, and other types of handwash sinks where the user cannot make temperature adjustments, ensure the temperature provided to the user after the mixing valve does not exceed 49C (120F).

Violation: Water was observed dripping from the deckhead onto the deck adjacent to a light fixture in the middle of the room. It appeared to be dripping from the deckhead plug. No food was impacted.
Recommendation: Ensure decks and deckheads in food storage areas are cleaned as often as necessary.

Violation: Condensate was observed on the deckhead adjacent to the pastry preparation counter.
