Site: Bar-Deck 9 Red Frog Rum
Violation: On the left end of the worker side of the bar, the backflow preventer in the undercounter technical compartment was leaking onto the deck and the device was heavily soiled with biofilm.
Recommendation: Maintain backflow prevention devices in good repair.

Site: Medical-Crew Member Reporting
Violation: On 11 March, a Galley Steward had an onset of AGE symptoms sometime between 10:00 - 12:00 and reported to the medical center around 17:00. He experienced his last symptom at 18:00. He went to the crew mess at 11:00. According to his time card, he clocked in to work at 17:04 and clocked out at 17:38. The crew member received a written warning for late reporting his AGE symptoms.
Recommendation: When food employees meet the case definition for AGE, ensure the following actions are taken: (1) isolate in cabin or designated restricted area until symptom-free for a minimum of 48 hours.

Site: Medical-Crew Member Reporting
Violation: On 20 March, an Assistant Waiter had an onset of acute gastroenteritis (AGE) symptoms at 07:45 and reported to the medical center at 10:58. He stated he only experienced AGE symptoms from 08:00 to 09:00, but wanted to finish his duties and was aware of the AGE reporting criteria. The crew member received a written warning for working while symptomatic.
Recommendation: When food employees meet the case definition for AGE, ensure the following actions are taken: (1) isolate in cabin or designated restricted area until symptom-free for a minimum of 48 hours.

Violation: An Assistant Waiter continued to work while experiencing AGE symptoms. He explained to the medical staff that he wanted to finish his duties although he was aware of the AGE reporting criteria.
Recommendation: Ensure crew members report to the medical center immediately upon experiencing AGE symptoms.

Violation: The door gaskets of combination oven #1 lower and #2 upper were in disrepair. Condensate from inside the ovens was leaking onto the deck.
Recommendation: Design and construct nonfood-contact surfaces of equipment and utensils to be durable and to retain their characteristic qualities under normal use conditions.

Site: Galley-Deck 3 Dishwash
Violation: The alarm for low detergent was sounding intermittently on one of the dishwash machines, however the detergent reservoir was full.
Recommendation: Maintain warewashing equipment in good repair and proper adjustment, including: (1) warewashing equipment is maintained in a state of repair and condition that meets the standards of the materials, design, and construction of these guidelines.

Site: Galley-Deck 3 Main Dishwash
Violation: A back-up of over 200 previously cleaned dish plates in at least three trolleys were stored in the passageway entrance to the dishwash area instead of a proper soiled storage area. The trolleys were marked with signs stating the plates were to be rewashed.
Recommendation: Ensure drainboards, utensils, racks, or tables large enough to accommodate all soiled and cleaned items that may accumulate during hours of operation are provided for necessary utensil holding before cleaning and after sanitizing.

Violation: On the clean landing table, one previously cleaned plate contained food debris. The plate was removed from service and rewashed.

Recommendation: Ensure a handwashing facility includes a sink, soap dispenser, single-use towels dispenser, and waste receptacle.

Violation: The area above the preparation and service areas had a one inch (25 mm) gap directly leading to an air handling plenum. This area was soiled with excess dust and debris, potentially falling onto the bar area.
Recommendation: Ensure decks, bulkheads, deckheads, and attached equipment in food preparation, warewashing, pantries, and storage areas are cleaned as often as necessary.

Violation: Condensate from inside combination oven #1 lower and #2 upper was leaking onto the deck.
Recommendation: Ensure black and gray water is discharged to the vessel's wastewater disposal system and does not pool on the deck.

Violation: One live house fly was observed on the in-use bar.
Recommendation: Effectively control the presence of insects, rodents, and other pests to minimize their presence in the food storage, preparation, and service areas and warewashing and utensil storage areas aboard a vessel.

Site: Children Area-Camp Ocean
Violation: The previously cleaned diaper changing table was soiled at the juncture where the bulkhead-mounted section met the fold-out table. The table was immediately dismantled and the area was cleaned.
Recommendation: Clean and disinfect diaper changing, handwashing facilities, and toilet rooms daily and when soiled during use.

Site: Buffet-Deck 9 Guy's Burger Joint
Violation: No passenger handwash station was installed in this self service area as per the 2011 VSP Construction Guidelines. The inspector was provided an e-mail sent to VSP on 9 January 2015 specifically identifying a timeline and resolution by September 2015.
Recommendation: In accordance with section 12.13.3.3 of the 2011 VSP Operations Manual, "procedures, systems, equipment, technology, processes, or activities that are not identified in the scope of this manual must not be tested or introduced operationally onboard any vessel until the concept is submitted in writing to the VSP Chief for review. If the review determines the concept is within the scope of the VSP Operations Manual, written procedures, control measures, or a complete variance submission may be required."
