Site: Potable Water-Port Side Hi Fog Connection
Violation: Water was leaking from the top of the reduced pressure assembly backflow prevention device by one of the large bolts. Crew tightened the bolt and the water leak stopped.
Recommendation: Maintain backflow prevention devices in good repair.

Site: Potable Water-Garbage Room
Violation: The hot water line supplying the hand spray sink was not striped blue or blue/green/blue to indicate potable water.
Recommendation: Stripe or paint potable water lines either in accordance with ISO 14726 (blue/green/blue) or blue only. Stripe or paint these lines at 5 meters (15 feet) intervals and on each side of partitions, decks, and bulkheads, except where decor would be marred by such markings, including potable water supply lines in technical lockers.

Site: Potable Water-Bunker Pretest
Violation: The ship had multiple occasions where the bunkering process started and stopped throughout the time the bunker line was connected to the shore connection. When this occurred the bunker pretest of the shore side was not documented. For example, bunkering started on 1 March at 7:05 and the initial chlorine and pH values were properly recorded. Bunkering then stopped at 13:30 and restarted at 16:20. No shore side chlorine or pH values were taken at the restart. Bunkering continued until 21:25, stopped, and then restarted again at 1:45 on 2 March. No shore side chlorine or pH values were taken at the restart This bunkering segment stopped at 8:05 and restarted again at 14:40. No shore side chlorine or pH values were taken at the restart.
Recommendation: Conduct a free halogen residual and pH test on the shore-side water supply before starting the potable water bunkering process to establish the correct halogen dosage. Record the results of the pretest and ensure they are available for review during inspections.

Site: Potable Water-Potable Water Tank Maintenance
Violation: Touch-up paint was applied to potable water tanks 2P, 2S, 3P, 3S, and 5P when they were inspected since the beginning of 2015 but it was not documented if the paint was applied and dried in accordance with the manufacturer's recommendations.
Recommendation: Ensure the interior coatings on potable water tanks are approved for potable water contact by a certification organization. Follow all manufacturers? recommendations for application, drying, and curing. For the tank coatings used, maintain the following onboard: (1) written documentation of approval from the certification organization (independent of the coating manufacturer); (2) manufacturers? recommendations for application, drying, and curing; and (3) written documentation that the manufacturers? recommendations have been followed for application, drying, and curing.

Site: Galley-Pastry
Violation: Only one of the four chilled water lines behind the ice cream machines were uniquely identified. This was corrected.
Recommendation: Uniquely identify all refrigerant brine lines in all galleys, pantries, and cold rooms to prevent cross-connections.

Site: Pantry-Staff
Violation: Two slotted fasteners were in the food-contact zone of the coffee strainer on the espresso machine.
Recommendation: Use only low profile, nonslotted, noncorroding, and easy-to-clean fasteners on food-contact surfaces and in splash zones. Prohibit the use of exposed slotted screws, Phillips head screws, or pop rivets in these areas.

Site: Buffet-Staff Mess
Violation: A red bucket in the worker side of the buffet was had an Oxivir Five16 label affixed to the outside of the bucket. It was determined that the liquid in the bucket was soapy water and not Oxivir. The Oxivir label was removed immediately.
Recommendation: Ensure the bucket is not improperly labeled.

Site: Bar-Staff
Violation: The bulkhead-mounted menu board above the back bar counter was made of a soft cloth material and was not hard or nonabsorbent.
Recommendation: Ensure bulkheads and deckheads have smooth, hard finishes and light colored surfaces. Exception: bars may have decorative surfaces provided that they are easily cleanable.

Site: Preparation Room-Deck 0
Violation: Four bulkhead plugs were missing, exposing holes. This was corrected.
Recommendation: Ensure decks, bulkheads, and deckheads in food preparation, warewashing, pantries, and storage areas are maintained in good repair.

Site: Other-Engine Control Room Toilets
Violation: Both toilet rooms did not have a sign prompting users to use a paper towel to open the door upon exiting.
Recommendation: Post a sign advising users of toilet facilities to use hand towel, paper towel, or tissue to open the door unless the exit is hands free.

Site: Other-Deck 10 Men's Changing Room and Sauna
Violation: No handwash sign or pictograph was posted at the handwash sink located inside the handicap accessible stall.
Recommendation: Provide the passenger and crew public toilets with a handwashing station that includes: (4) A sign advising users to wash hands.
