Site: Potable Water-Salon
Violation: The backflow prevention device at the cold water pipe line to the salon was continuously leaking.
Recommendation: Maintain backflow prevention devices in good repair.

Site: Galley-Deck A - Potwashing
Violation: The water supply line to the automatic potwashing machine (JC/A 4061) did not have a backflow preventer installed. Staff confirmed the machine did not have an internal backflow preventer installed on it.
Recommendation: Provide backflow prevention devices on all fixtures using potable water.

Site: Recreational Water Facilities-Crew Whirlpool
Violation: There was no safety sign. Staff stated that it was ordered.
Recommendation: Provide safety signs for all RWFs, except for baby-only water facilities. Ensure the signs, at a minimum, include the following words: (1) do not use these facilities if you are experiencing diarrhea, vomiting, or fever; (2) no children in diapers or who are not toilet trained; (3) shower before entering the facility; and (4) bather load #. For children's RWF signs, include the exact wording "TAKE CHILDREN ON FREQUENT BATHROOM BREAKS" or "TAKE CHILDREN ON FREQUENT TOILET BREAKS." In addition to the safety sign requirements in section 6.7.1.1.1, install a sign at each whirlpool spa and spa pool entrance listing precautions and risks associated with the use of these facilities. Include, at a minimum, cautions against use by the following: (1) individuals who are immunocompromised; (2) individuals on medication or who have underlying medical conditions such as cardiovascular disease, diabetes, or high or low blood pressure; and (3) pregnant women, elderly persons, and children. Additionally, caution against exceeding 15 minutes of exposure.

Site: Medical-Crew Acute Gastroenteritis (AGE) Reporting
Violation: The review of the AGE logs for the last 5 cruises revealed that a beauty therapist had an onset of AGE symptoms on 23 November at 0700 but reported to the medical center at 1050 that day. She did not work while symptomatic but walked around the ship and stayed in another room with a close contact before reporting to the medical center. She received written counseling and disciplinary action.
Recommendation: When food employees meet the case definition for AGE, ensure the following actions are taken: (1) isolate in cabin or designated restricted area until symptom-free for a minimum of 48 hours; (2) follow-up with and receive approval by designated medical personnel before returning crew to work; (3) document date and time of last symptom and clearance to return to work. When nonfood employees meet the case definition for AGE, ensure the following actions are taken: (1) isolate in cabin or designated restricted area until symptom-free for a minimum of 24 hours; (2) follow-up with and receive approval by designated medical personnel before returning crew to work; (3) document the date and time of last symptom and clearance to return to work.

Site: Other-Koppert Cress Room
Violation: The nonremovable drain pipe from the Cressomatic cabinets was longer than 1 meter and with corners, making it difficult to clean.
Recommendation: Ensure that the drain line is less than 1,000 millimeters (40 inches) in length and free of sharp angles or corners if designed to be cleaned in place by a brush. Otherwise, make the drain line readily removable for cleaning if longer than 1,000 millimeters (40 inches).

Violation: Containers of feed solution and a cleaner/disinfectant solution were stored a few millimeters apart inside the technical cabinet.
Recommendation: Protect food from contamination by storing the food: (1) Covered or otherwise protected; (2) In a clean, dry location; (3) Where it is not exposed to splash, dust, or other contamination; and (4) At least 15 centimeters (6 inches) above the deck.

Site: Galley-Deck A - Potwashing
Violation: Six red plastic cutting boards in the clean utensil area were severely scored and difficult to clean.
Recommendation: Ensure surfaces such as cutting boards that are subject to scratching and scoring are resurfaced if they no longer can be effectively cleaned and sanitized, or discarded if they are not capable of being resurfaced.

Site: Food Service General-Handheld Radios
Violation: The handheld radios used by food employees to communicate during operations were not easy to clean.
Recommendation: Ensure nonfood-contact surfaces of equipment that are exposed to splash, spillage, or other food soiling or that require frequent cleaning are constructed of a corrosion-resistant, nonabsorbent, and smooth material.

Violation: The glasswasher consistently allowed one rack of dishware to pass without a final rinse spray whenever it restarted.
Recommendation: Maintain warewashing equipment in good repair and proper adjustment, including: (1) warewashing equipment is maintained in a state of repair and condition that meets the standards of the materials, design, and construction of these guidelines.

Violation: The wiping cloth solution in a bucket next to undercounter cooler 2.14A was heavily soiled with grease. The solution was immediately changed.
Recommendation: Ensure the sanitize solutions are maintained clean.

Recommendation: Restrict wiping cloths to the following: (4) Wet wiping cloths used with a freshly made sanitizing solution and dry wiping cloths are free of food debris and visible soil.

Violation: The juice dispenser nozzle stored in the clean utensil area had juice residue in it. The nozzle was stored assembled. The soiled nozzle was sent for rewashing.
Recommendation: Ensure food contact surfaces of equipment and utensils are clean to sight and touch.

Violation: The previously cleaned grease pan housing was soiled with grease on the cooking range. Cleaning started while the inspection team was in the area.
Recommendation: Ensure nonfood-contact surfaces of equipment are kept free of an accumulation of grease residue.

Violation: Containers of feed solution and a cleaner/disinfectant solution were stored a few millimeters apart inside the technical cabinet.
Recommendation: Store poisonous or toxic materials used in the cleaning and maintenance of food areas in a cleaning materials locker so they cannot contaminate food, equipment, utensils, linens, and single-service and single-use articles.

Violation: The light intensity could not be raised to 220 lux for cleaning operations. Efforts to make the area brighter started immediately.
Recommendation: Ensure the light intensity is at least 110 lux (10 foot candles) at handwashing stations in bars. In bars and dining room waiter stations, provide 220 lux (20 foot candles) light intensity during cleaning operations.

Violation: The potwash clean storage rack area had 89-100 lux of light even with top shelf of pans removed.
Recommendation: Ensure the light intensity is at least 220 lux (20 foot candles) on surfaces, and at a distance of 75 centimeters (30 inches) above the deck in warewashing areas, and in equipment and utensil storage areas.

Violation: At least 23 dead small flies were on top of the Cressomatric cabinets. This area was not yet commissioned.
Recommendation: Effectively control the presence of insects, rodents, and other pests to minimize their presence in the food storage, preparation, and service areas and warewashing and utensil storage areas aboard a vessel.

Violation: The toilet room shared by the reception area and the ICU room had three bath towels next to the handwashing station.
Recommendation: Remove the bath towels and provide only single-use paper towels for the handwashing station.

Site: Housekeeping-Outbreak Prevention and Response Plan (OPRP)
Violation: The review of the AGE logs for the last 5 cruises revealed that one of the crew AGE reportable cases had a public vomit incident (PVI) in the crew mess. It was unclear if a cleaning and disinfection response occurred per OPRP procedures. A log book for all PVI responses was reviewed and there was no indication of a response following this specific PVI.
Recommendation: Ensure to follow the OPRP.

Violation: There were 6 microsuede chairs and 16 fabric pillows that were not easy to clean.
Recommendation: Ensure surfaces of tables, chairs, and other furnishings that children touch with their hands are cleanable.
