Site: Room Service-Main Galley - Room Service Pantry
Violation: The passenger menu of foods had the consumer advisory near the bottom of the page in a very small text font. In addition, the advisory text was placed behind a service fee sentence without spaces between, and together with the letter sizes made it less likely to be noticed or read by the consumer.

Site: Medical-Acute Gastroenteritis (AGE) Surveillance Log
Violation: The AGE log for the 25 July-1 August voyagehad a crew member as a case, but the crew member position was missing on the log.
Recommendation: Ensure the AGE surveillance log entry for each passenger or crew member contain the following information in separate columns: (8) crew member position or job on the vessel, if applicable.

Site: Potable Water-Halogen Analyzer
Violation: The amount of halogen injected during bunkering was not controlled by a flow meter or a free halogen analyzer. The ship had an analyzer, but it was turned off while the ship was bunkering. Staff reported not using it during bunkering due to an unstable flow causing inaccurate readings between comparison tests and analyzer. When questioned, staff only used the anaylzer during production for monitoring and comparison of the free halogen residual testing, and the analyzer was not used to control the chlorine dosing pumps. Staff would monitor the free halogen readings from the analyzer in the engine control room, and manually control the dosing of the halogen when it needed to be adjusted. Staff informed the inspector a technician would be onboard in October, and they will address these analyzer issues.
Recommendation: Continuously halogenate potable water to at least 2.0 mg/L (ppm) free residual halogen at the time of bunkering or production with an automatic halogenation device that is controlled by a flow meter and adjust the pH so it does not exceed 7.8. Ensure the amount of halogen injected during bunkering or production is controlled by a flow meter or a free halogen analyzer.

Site: Medical-Crew Late Gastrointestinal Illness (GI) Reporting
Violation: A food handler experienced GI symptoms at 0400 on 11 August and did not report to medical until 1220 that day; the crew member participated in a boat drill from 1000-1200. A photographer experienced GI symptoms at 1500 on 20 July, and did not report to medical until 1900 that day; the crew member worked from 1700-1900. Both crew members were discplined for reporting late to medical and not being isolated while experiencing GI symptoms. A similar finding was noted on the last inspection.
Recommendation: When food employees meet the case definition for AGE, ensure the following actions are taken: (1) isolate in cabin or designated restricted area until symptom-free for a minimum of 48 hours; (2) follow-up with and receive approval by designated medical personnel before returning crew to work; (3) document date and time of last symptom and clearance to return to work. When nonfood employees meet the case definition for AGE, ensure the following actions are taken: (1) isolate in cabin or designated restricted area until symptom-free for a minimum of 24 hours; (2) follow-up with and receive approval by designated medical personnel before returning crew to work; (3) document the date and time of last symptom and clearance to return to work.

Site: Galley-Main Galley - Cold Galley Aft
Violation: The two door reach-in pass through refrigerator #R-06 had a door missing and was in disrepair. The out of order sign on the unit noted it has been in this condition since 20 February, 2015.
Recommendation: Food contact equipment must be maintained in good repair and proper adjustment: (1) Equipment must be maintained in a state of repair and condition that meets the materials, design, construction, and operating specifications of these guidelines; and (2) Cutting or piercing parts of can openers must be kept sharp to minimize the creation of metal fragments that can contaminate food when the container is opened.

Site: Galley-Crew Galley - Dishwash
Violation: The in-use conveyor glasswashing machine had a layer of mineral salts (scale) along the outer surfaces of the upper final sanitizing rinse spray arm, as well as along the upper metal panel just after the final rinse spray.
Recommendation: Ensure warewashing machines, drainboards, and the compartments of sinks, basins, or other receptacles used for washing and rinsing equipment, utensils, or raw foods, or laundering wiping cloths are cleaned: (1) before use; (2) throughout the day at a frequency necessary to prevent recontamination of equipment and utensils and to ensure that the equipment performs its intended function; (3) at least every 24 hours (if used).

Site: Galley-Garden Cafe Galley - Dishwash
Violation: The in-use flight-type conveyor dishwash machine had a heavy scale layer along the 100 mm metal panel installed along the top interior of the machine and just past the final sanitizing rinse compartment.
Recommendation: Ensure warewashing machines, drainboards, and the compartments of sinks, basins, or other receptacles used for washing and rinsing equipment, utensils, or raw foods, or laundering wiping cloths are cleaned: (1) before use; (2) throughout the day at a frequency necessary to prevent recontamination of equipment and utensils and to ensure that the equipment performs its intended function; (3) at least every 24 hours (if used).

Violation: The espresso machine used by staff in the medical center was soiled with coffee debris and dust beyond a day's accumulation.
Recommendation: Ensure nonfood-contact surfaces of equipment are kept free of an accumulation of dust, dirt, food residue, and other debris.

Site: Buffet-Officers Mess - Portable Buffet Table
Violation: The artificial light level over the buffet was below the minimum 220 lux; the two lights installed were not functional. In addition, the recessed light over the front center of the table was not covered and the bulb was not shatter-resistant. This was corrected.
Recommendation: Ensure the light intensity is at least 110 lux (10 foot candles) at handwashing stations in bars. In bars and dining room waiter stations, provide 220 lux (20 foot candles) light intensity during cleaning operations. Ensure light bulbs are shielded, coated, or otherwise shatter-resistant in areas where there is exposed food, clean equipment, utensils, and linens, or unwrapped single-service and single-use articles.

Site: Preparation Room-Provisions - Buffet Preparation Room
Violation: An active leak of a condensate water was observed from the center of the walk-in refrigerator near the door. The leak was from the juncture of two deckhead panels.
Recommendation: Ensure ventilation hood systems and devices operate effectively to prevent grease and condensate from collecting on the bulkheads and deckheads and remove contaminants generated by equipment located under them.

Site: Pantry-Sky High Bar - Pantry
Violation: During operation of the front bar the wooden swing entry door to the pantry was pinned open. The front bar is located on an open deck, so allowing the pantry door to stand open provides easy access to flying pests. Near the end of the inspection the inspector returned to this bar and once again the swing entry door was pinned open. There was an existing stainless steel fire door already pinned open inside the pantry.
Recommendation: Protect entry points where pests may enter the food areas.

Violation: Two toy cars for the 3-5 year old childern were soiled with dust and debris, These were previously cleaned.
Recommendation: Ensure toys used in the child-activity center are maintained in a clean condition.
