Site: Medical-Immediate Contacts of Symptomatic Crew
Violation: The review of the acute gastroenteritis (AGE) logs found that there was no documentation if the symptomatic crew members had immediate contacts or significant others, other than cabin mates.
Recommendation: An immediate contact means sharing a cabin either full or part time or sharing its bathroom facilities. This includes boyfriends, girlfriends, spouses, or other significant others. If the symptomatic crew member has no cabin mate or other immediate contact, this must be documented. When any crew member (food or nonfood employee) meets the case definition for AGE, take the following actions: (1) restrict exposure to symptomatic crew member(s); (2) undergo a verbal interview with medical or supervisory staff, who will confirm their condition, provide facts and a written fact sheet about hygiene and handwashing, and instruct them to report immediately to medical if they develop illness symptoms; (3) complete a verbal interview daily with medical or supervisory staff until 48 hours after the ill crew members? symptoms began. Ensure that the first verbal interview is conducted within 8 hours from the time the ill crew member initially reported to the medical staff. If the asymptomatic immediate contact or cabin mate is at work, ensure that he or she must is contacted by medical or supervisory staff as soon as possible. Document the date and time of verbal interviews.

Site: Medical-Acute Gastroenteritis (AGE) Case Definition
Violation: On 28 August, a passenger reported having AGE symptoms with no underlying medical conditions, was prescribed anti-diarrheal medication by the medical staff, and was deemed nonreportable. There was no documentation as if the AGE symptoms were normal or not for the passenger. The passenger was isolated as a precaution.
Recommendation: Ensure a reportable case of AGE is a case reported to the master of the vessel, the medical staff, or other designated staff by a passenger or a crew member and meets the case definition based on the following symptoms: (1) Diarrhea (three or more episodes of loose stools in a 24 hour period or what is above normal for the individual. See section 4.1.2.1.3, illness symptoms); or (2) vomiting and one additional symptom including one or more episodes of loose stools in a 24-hour period, or abdominal cramps, or headache, or muscle aches, or fever (temperature of greater than or equal to 38C [100.4F]).

Site: Potable Water-Cross-connection Control Program
Violation: The cross-connection control log listed two testable devices with a plumbing connection of 'boiler potable water'. Upon inspection it was determined that the devices were for the high salinity overboard discharge lines. This was written in the previous inspection report.
Recommendation: Update the cross-connection control log.

Site: Potable Water-Port Evaporator
Violation: The reduced pressure zone (RPz) assembly for the evaporator high salinity overboard discharge line was continuously leaking.
Recommendation: Maintain backflow prevention devices in good repair.

Site: Potable Water-Starboard Evaporator
Violation: The previous inspection found that a double check backflow preventer with no intermediate vent was installed in the evaporator high salinity overboard discharge line. The current inspection found that a reduced pressure zone (RPz) assembly was installed replacing the previous device. However, the new RPz was not tested after installation.
Recommendation: Inspect and test backflow prevention devices requiring testing with a test kit after installation and at least annually. Maintain the test results showing the pressure differences on both sides of the valves for each device.

Site: Galley-Main Galley - Garde' Manger
Violation: The previously cleaned counter model slicer had a significantly damaged surface along the grey pad that made direct food contact in sliding the food back and forth against the blade. The surface material was torn away. Staff immediately removed this pad and replaced it.
Recommendation: Food contact equipment must be maintained in good repair and proper adjustment: (1) Equipment must be maintained in a state of repair and condition that meets the materials, design, construction, and operating specifications of these guidelines; and (2) Cutting or piercing parts of can openers must be kept sharp to minimize the creation of metal fragments that can contaminate food when the container is opened.

Site: Buffet-Oceanview Caf - Forward Beverage Station
Violation: The port ice and water dispenser had a difficult to clean gap at the top exterior panel which exposed an open void between that panel and the plastic liner for the upper ice bin. Also exposed was a sponge-type insulation between the plastic liner and stainless steel exterior panel. Staff evaluated the pieces and re-attached to correct this during the inspection.
Recommendation: Ensure nonfood-contact surfaces are free of unnecessary ledges, projections, and crevices, and designed and constructed to allow easy cleaning and to facilitate maintenance.

Site: Pantry-Deck 6 Forward
Violation: The undercounter warewasher did not have a data plate.
Recommendation: Ensure a warewashing machine is provided with an easily accessible and readable data plate affixed to or posted adjacent to the machine that indicates the machine?s design and operating specifications including the: (1) wash tank, rinse tank(s) if present, and final sanitizing rinse temperatures; (2) pressure required for the fresh water sanitizing rinse unless the machine is designed to use only a pumped sanitizing rinse; (3) conveyor speed in feet per minute or minimum transit time for belt conveyor machines, minimum transit time for rack conveyor machines, and wash and final sanitizing rinse times as specified by the manufacturer for stationary rack machines.

Site: Galley-Oceanview Caf Galley - Potwash
Violation: The in-use stationary rack potwash machine had a wash temperature of 140-141F. The manufacturer's data plate specified a minimum wash temperature of 150F. Staff stopped the unit and repaired it during the inspection.
Recommendation: Ensure the temperature of the wash solution in spray type warewashers that use hot water to sanitize is not less than: (1) 74C (165F) for a stationary-rack, single-temperature machine; (2) 66C (150F) for a stationary-rack, dual-temperature machine; (3) 71C (160F) for a single-tank, conveyor, dual-temperature machine; (4) 66C (150F) for a multi-tank, conveyor, multi-temperature machine.

Site: Buffet-Oceanview Caf - Forward Beverage Station
Violation: Three previously cleaned plastic self-service dispensers of milk, skim milk, and half & half were set on the beverage station awaiting filling. When the dispensing nozzles were removed by the inspector, each of these had residual product still remaining in the head. Staff took all back to be re-washed.
Recommendation: Ensure food only contacts surfaces of equipment and utensils that are cleaned and sanitized.

Site: Buffet-Oceanview Caf - Starboard Service Line
Violation: Over the workers edge of the serving line there was a heavy, black dirt and dust debris mixture on the deckhead surrounding what was a slot-type air vent. This debris was noted along the deckhead surface over a 5 meter length on the starboard side service line and almost directly above the edge of the stainless steel counter.
Recommendation: Ensure decks, bulkheads, deckheads, and attached equipment in food preparation, warewashing, pantries, and storage areas are cleaned as often as necessary.

Site: Children Area-Diaper Changing Facility
Violation: The sign advising handwashing after diaper changing was not specific to advise handwashing after each diaper change.
Recommendation: Post signs in the diaper changing area advising handwashing after each diaper change.
