Site: Potable Water-Pipe Disinfection Records
Violation: A small potable water pipe was replaced and disinfected correctly, according to a 8/11/15 record. The record stated the pipe was flushed to achieve a halogen residual below 5.0 ppm, but no halogen residual value was recorded on the document.
Recommendation: Document the free halogen residual level.

Site: Potable Water-Mineralizer Backwash Line
Violation: The water line was labeled 'potable water' after the reduced pressure backflow preventer on the mineralizer backwash line. This was corrected.
Recommendation: Stripe or paint potable water lines either in accordance with ISO 14726 (blue/green/blue) or blue only. Stripe or paint distillate and permeate lines directed to the potable water system in accordance with ISO 14726 (blue/gray/blue). Ensure no other lines have the above color designations. Stripe or paint these lines at 5 meters (15 feet) intervals and on each side of partitions, decks, and bulkheads, except where decor would be marred by such markings, including potable water supply lines in technical lockers. Do not stripe or paint potable water lines after reduced pressure assemblies as potable water. Uniquely identify all refrigerant brine lines in all galleys, pantries, and cold rooms to prevent cross-connections.

Site: Potable Water-Halogen Injection Point - Static Mixer
Violation: A static mixer was installed between the halogen injection point and sampling point, A copy of all the manufacturers' literature for the installation, operation, and maintenance of the static mixer was not available to confirm configuration.
Recommendation: Obtain water samples for halogen and pH testing from a sample cock and/or a halogen analyzer probe located on the bunker or production water line at least 3 meters (10 feet) after the halogen injection point and before the storage tank. A static mixer may be used to reduce the distance between the halogen injection point and the sample cock or halogen analyzer sample point. If used, the mixer must be installed per the manufacturer's recommendations. A copy of all manufacturers' literature for installation, operation, and maintenance must be maintained.

Site: Other-Noodle Bar - Dining Room Menu's
Violation: The paper menus were printed in a light ink which was apparently copied. The consumer advisory for undercooked and raw animal foods appeared on the bottom of each menu and was not readable. Staff stated they would order new menu's.
Recommendation: Ensure the text for the menu's is printed clearly so it can be read and understood.

Site: Galley-Room Service Preparation
Violation: One previously cleaned green and brown cutting board had crevices in the surface that made cleaning difficult.
Recommendation: Ensure surfaces such as cutting blocks and boards that are subject to scratching and scoring are resurfaced if they no longer can be effectively cleaned and sanitized, or discarded if they are not capable of being resurfaced.

Violation: A seam in the counter juncture with the fryer was not tight allowing grease to seap through it.
Recommendation: Ensure equipment that is fixed because it is not easily movable is installed so that it is: (3) Sealed to adjoining equipment or bulkhead. Ensure nonfood contact equipment is maintained in good repair and proper adjustment including: (1) Equipment maintained in a state of repair and condition that meets the materials, design, construction, and operation specifications of these guidelines; (2) Equipment components such as doors, seals, hinges, fasteners, and kick plates are kept intact, tight, and adjusted in accordance with manufacturer's specifications.

Violation: The gasket on the undercounter refrigerator was in disrepair causing condensate to collect on the cabinet and puddle on the deck.
Recommendation: Ensure nonfood contact equipment is maintained in good repair and proper adjustment including: (1) Equipment maintained in a state of repair and condition that meets the materials, design, construction, and operation specifications of these guidelines; (2) Equipment components such as doors, seals, hinges, fasteners, and kick plates are kept intact, tight, and adjusted in accordance with manufacturer's specifications.

Violation: The gasket on the sliding door refrigerator on the far right was damaged allowing condensate to collect around the door.

Violation: The technical compartment and deck under this fryer had a greater than one days accumulation of grease. The grease had wicked through a seam in the counter juncture.
Recommendation: Ensure decks in food preparation areas are cleaned as often as necessary.

Violation: During operation the spray pattern for the final sanitizing rinse upper spray arms of both the flight-type conveyor dishwasher and the conveyor glasswash machines was weak and nearer to a single water stream than a fan pattern,
Recommendation: Maintain warewashing equipment in good repair and proper adjustment, including: (1) warewashing equipment is maintained in a state of repair and condition that meets the standards of the materials, design, and construction of these guidelines; (2) water pressure and water temperature-measuring devices are maintained in good repair and accurate within the intended range of use.

Violation: The solution in the sanitizing bucket, located adjacent to the hand wash station, was dirty, The bucket was removed.
Recommendation: Ensure the sanitize solutions are maintained clean.

Violation: The single ice machine was in use and had an accumulation of sediment inside the water bath supplying the upper cuber panel.

Violation: Two of the starboard side draft beer taps and one on the port side had an old brown soil inside the serving nozzle. Staff immediately took these out of service for cleaning.
Recommendation: Ensure food only contacts surfaces of equipment and utensils that are cleaned and sanitized. Ensure food contact surfaces of equipment and utensils are clean to sight and touch.
