Item No.: 02
Site: Medical-Crew Close Contacts
Violation: A bar waiter that reported to the medical center on 27 September with GI illness symptoms had three cabin mates and a significant other (immediate contact). The 48-hour follow-up was documented for the three cabin mates but not for the significant other. It could not be determined if one of the cabin mates was the significant other.
Recommendation: Ensure the record of crew GI illness cases includes information on presence or absence of any cabin mates and immediate contacts.

Site: Medical-Crew Close Contacts
Violation: The significant other category on the GI paperwork for two of the reportable crew cases during the voyage from 17 September to 21 September was not filled out.
Recommendation: Ensure the notes on crew GI illness cases include both cabin mate and immediate contact completed.

Site: Potable Water-Pipe Replacement
Violation: Since 9 August, eight sections of potable water piping, all 6 meters in length or less, were replaced on the ship. The documentation showed the pipes were disinfected with 50 ppm of chlorine for 4 hours, but the chlorine residual after the pipe was flushed was not documented.
Recommendation: Maintain documentation of all inspections, maintenance, cleaning, and disinfection for 12 months and ensure it is available for review during inspections. Ensure records include a method of disinfection, concentration and contact time of the disinfectant, and a halogen value of less than or equal to 5 ppm before the tank is put back into service.

Site: Recreational Water Facilities-Fecal and Vomit Accident Log
Violation: The log for the fecal accident recorded on 19 September did not indicate if it was loose or formed stool. The disinfection procedures followed by the ship were for a formed stool accident.
Recommendation: Ensure a written or electronic record is made of all accidents involving fecal material or vomit. Ensure the record includes the name of the RWF, date and time of the accident, type of accident, response steps taken, and free residual halogen level and contact time reached during disinfection. For a fecal accident, ensure the record also includes whether the fecal material was formed or loose.

Site: Housekeeping-Private Cabin Whirlpool Spas
Violation: The disinfection record was missing for 26 July. The records went from 19 July to 2 August.
Recommendation: Clean and disinfect private whirlpool spas located in individual passenger cabins, including associated recirculation systems, between occupancies or weekly, whichever is more frequent. Ensure disinfection is accomplished with an appropriate halogen-based disinfectant at 10 ppm for 60 minutes, or an equivalent CT value. Maintain a record outlining the frequency of cleaning and disinfection. Ensure the record includes the type, concentration, and contact time of the disinfectant. Retain records on the vessel for 12 months.

Site: Recreational Water Facilities-Crew Port Jacuzzi
Violation: No safety sign advising users of the precautions and risks of using the whirlpool spas (Section 6.7.1.1.3) was posted by the entrance to the port crew whirlpool spa.
Recommendation: In addition to the safety sign requirements in section 6.7.1.1.1, install a sign at each whirlpool spa and spa pool entrance listing precautions and risks associated with the use of these facilities. Include, at a minimum, cautions against use by the following: (1) individuals who are immunocompromised; (2) individuals on medication or who have underlying medical conditions such as cardiovascular disease, diabetes, or high or low blood pressure; and (3) pregnant women, elderly persons, and children. Additionally, caution against exceeding 15 minutes of exposure.

Site: Buffet-Crew Mess
Violation: Some of the rolls displayed in imitation wicker baskets for crew self-service were in contact with the wicker, which was woven and difficult to clean. Paper liners were inside the baskets, but not draped over the top edge to prevent contact. Use of these baskets in all buffets should be reviewed to ensure proper foods are inside and protection is provided.
Recommendation: Do not use wood and wood wicker as a food-contact surface.

Site: Galley-Deck 5 Galley - Potwash
Violation: There was a large skillet on the clean rack with peeling non-stick coating material around the entire perimeter of the interior surface. Staff discarded the pan during the inspection.
Recommendation: Materials used in the construction of multiuse utensils and food contact surfaces of equipment must be: (1) durable, corrosion resistant, and nonabsorbent; (2) sufficient in weight and thickness to withstand repeated warewashing; (3) finished to have a smooth, easily cleanable surface; and (4) resistant to pitting, chipping, crazing, scratching, scoring, distortion and decomposition.

Site: Bar-International Cafe
Violation: The counter under the espresso machine had a square opening for the power cables and water line that was not sealed. The opening had an exposed ledge that accumulated coffee debris.
Recommendation: Ensure nonfood-contact surfaces are free of unnecessary ledges, projections, and crevices, and designed and constructed to allow easy cleaning and to facilitate maintenance.

Site: Galley-International Cafe Warewash
Violation: The curtains on the clean end of the in-use rack-type machine were soiled with food debris, especially on the clean side. When the curtains were removed to observe the final rinse spray pattern, both interior side panels of the machine were soiled with a heavy accumulation of food debris right around where the curtains hook into place.
Recommendation: Ensure warewashing machines, drainboards, and the compartments of sinks, basins, or other receptacles used for washing and rinsing equipment, utensils, or raw foods, or laundering wiping cloths are cleaned: (1) before use; (2) throughout the day at a frequency necessary to prevent recontamination of equipment and utensils and to ensure that the equipment performs its intended function; (3) at least every 24 hours (if used).

Violation: Coffee debris was accumulated on the ledge that was exposed in the square opening in the counter under the espresso machine.
Recommendation: Ensure nonfood-contact surfaces of equipment are kept free of an accumulation of dust, dirt, food residue, and other debris.

Site: Galley-Deck 5 Galley - Potwash
Violation: There were cracked and broken deck tiles thru the short corridor between the manual sinks and potwash machine. Some of the tiles would release water when stepped on.
Recommendation: Ensure decks, bulkheads, and deckheads in food preparation, warewashing, pantries, bars, and food and equipment storage areas are constructed and maintained for easy cleaning. Do not use carpet in these areas.

Site: Galley-Deck 5 Galley - Corridor Starboard
Violation: A large deckhead mounted air vent with box like grate surrounding was leaking water to the deck in the corrirdor beside the Garde' Manger and the blast chiller opposite. Water was pooling on the deck but no food or food surfaces were affected.
Recommendation: Ensure decks, bulkheads, and deckheads in food preparation, warewashing, pantries, and storage areas are maintained in good repair.

Violation: The exhaust vent above the back counter with the panini press was heavily soiled with a layer of dust and grease.
Recommendation: Ensure decks, bulkheads, deckheads, and attached equipment in food preparation, warewashing, pantries, and storage areas are cleaned as often as necessary.

Site: Galley-International Cafe Warewash
Violation: A significant amount of condensation was accumulating on the deckhead inside the hood system for the rack-type dish machine. Condensation was observed around the deckhead exhaust vent above the opening on the clean side of the machine, but no condensation was observed falling below.
Recommendation: Ensure ventilation hood systems and devices are sufficient in number and capacity to prevent grease or condensation from collecting on bulkheads and deckheads.

Violation: One small fruit fly was observed by the door of the walk-in refrigerator.

Site: Food Service General-Horizon Court Buffet and Galley
Violation: There were three live fruit flies observed during the lunch service in the two working side areas of the cooking section in the horizon cout buffet. The perforated deckhead panels did have small food splash on the surfaces and the many holes in each panel allowed easy access for these flies above the deckhead throughout. In the Horizon Court galley there was one live filth fly on the clean landing counter of the in-use conveyor dishwashing machine. There were two live fruit flies near the clean end of the machine on the deckhead near an access hatch, and one additional fruit fly was observed on the deckhead over the clean glass racks.
Recommendation: Effectively control the presence of insects, rodents, and other pests to minimize their presence in the food storage, preparation, and service areas and warewashing and utensil storage areas aboard a vessel.

Site: Food Service General-Sabbatini's Galley and Trident Grill Pantry
Violation: In the galley one live fruit fly was observed in the seam between two bulkhead panels starboard at the cold galley preparation counter center. In the combined pantry for the bar and trident grill there was one live filth fly on a bulkhead over the preparation counter (which was destroyed) and one live fruit fly near the entry to the grill walk-in refrigerator.
Recommendation: Effectively control the presence of insects, rodents, and other pests to minimize their presence in the food storage, preparation, and service areas and warewashing and utensil storage areas aboard a vessel.

Site: Galley-Deck 5 Galley - Wine Pantry
Violation: One live fruit fly was observed on a seam between deckhead panels above the overshelf where filled liquor bottles were stored.

Violation: Three live fruit flies were observed on the deckhead in various areas of this space during active preparation. Two of the three flies were destroyed during the inspection.
