Violation:
On Voyages 373 (5/22-28/16), 372 (5/15-22/16), and 371 (5/8-15/16), over 12 passengers and crew members had temperatures recorded in the AGE log from 206.60F to 212.72F. The prior logs examined had normal temperature ranges recorded. The medical staff discovered that the problem had occurred but had not been recognized in the log reviews when the temperatures had been taken and recorded in Celsius and improperly converted to Fahrenheit.

Recommendation:
Ensure the AGE surveillance log entry for each passenger or crew member contain the following information in separate columns: ignation as passenger or crew member; (8) crew member position or job on the vessel, if applicable; (9) cabin number; (10) meal seating information; (11) date of illness onset; (12) time of illness onset; (13) illness symptoms, including: (d) recorded temperature.
