Site: Potable Water-Microbiological Analysis
Violation: A ultraviolet (UV) light, which is a required piece of equipment for the microbiological analysis testing, was not available. The ship showed an order for this equipment placed on 13 August.
Recommendation: Analyze samples using a method accepted in Standard Methods for the Examination of Water and Wastewater. Operate and maintain test kits, incubators, and associated equipment in accordance with the manufacturers? specifications.

Site: Potable Water-Cross Connection Control Log
Violation: The log listed a reduced pressure (RP) backflow preventer on the Reverse Osmosis (RO) unit, but upon inspection of the RP the machinery/plumbing connection was the distillate water supply to the boiler.
Recommendation: Ensure the cross-connection control program includes at a minimum: a complete listing of cross-connections and the backflow prevention method or device for each, so there is a match to the plumbing system component and location. Include air gaps on the listing. Ensure a schedule for inspection frequency is set. Maintain a log documenting the inspection and maintenance in written or electronic form, and ensure it is available for review during inspections.

Site: Potable Water-Bunker Hoses
Violation: Before the ship started actively bunkering, the hoses were lying directly on the pier.
Recommendation: Handle and store all hoses, fittings, water filters, buckets, equipment, and tools used for connection with the bunkering of potable water in a sanitary manner.

Site: Galley-Combination Ovens
Violation: Insulation debris was trapped between the outside glass and the inside glass on the sight window of both ovens. Staff reported a work order was submitted to repair this issue.
Recommendation: Ensure nonfood contact equipment is maintained in good repair and proper adjustment including: (1) Equipment maintained in a state of repair and condition that meets the materials, design, construction, and operation specifications of these guidelines; (2) Equipment components such as doors, seals, hinges, fasteners, and kick plates are kept intact, tight, and adjusted in accordance with manufacturer's specifications.

Site: Galley-Deck 3 Dishwash
Violation: A large amount of steam and condensation was exiting from the dishwasher, causing water to pool on the deck and collect on the deckhead. Staff reported this was due to a damaged piece of equipment inside the dishwasher, and stated parts were on order.
Recommendation: Maintain warewashing equipment in good repair and proper adjustment, including: (1) warewashing equipment is maintained in a state of repair and condition that meets the standards of the materials, design, and construction of these guidelines; (2) water pressure and water temperature-measuring devices are maintained in good repair and accurate within the intended range of use.

Site: Other-Engine Changing Room
Violation: An espresso machine and an upright refrigerator were located in the room, along with a washing machine, dryer, a toilet room, lockers, and a changing area. Staff stated the espresso machine and refrigerator was previously taken out of service and the espresso machine was labeled 'out of service.' Additionally, the paper coffee cups were stored in a locked undercounter cabinet.
Recommendation: Store cleaned equipment and utensils, laundered linens, and single-service and single-use articles: (1) in a clean, dry location; (2) in a location where they are not exposed to splash, dust, or other contamination; (3) at least 150 millimeters (6 inches) above the deck.

Site: Other-Pizza Pirate
Violation: Three stacks of plates were stored not inverted or protected on the port counter, and passengers were observed directly next to these plates.
Recommendation: Store clean equipment and utensils covered or inverted.

Site: Galley-Deck 9 Aft Starboard Grill
Violation: A void was noted, where the door latched on the bulkhead, in the pantry to the service area of the grill.
Recommendation: Ensure decks, bulkheads, and deckheads in food preparation, warewashing, pantries, bars, and food and equipment storage areas are constructed and maintained for easy cleaning.

Site: Bar-Port Side Pool
Violation: Water was pooled under the bar in this area, due to the leaking connection in the utility sink.
Recommendation: Ensure decks, bulkheads, deckheads, and attached equipment in food preparation, warewashing, pantries, and storage areas are cleaned as often as necessary.

Violation: The potable water connection to the utility sink in the bar was constantly leaking and water was pooling on the deck under the bar.

Violation: A large amount of ice was noted in the condensation pan of the evaporative condenser, inside the walk-in refrigerator; this was due to a clogged drain line.

Site: Galley-Deck 9 Conveyor Potwash
Violation: The light intensity at the handwash station at the dirty dish drop off area measured 46 lux. The area was in operation.
Recommendation: Ensure the light intensity is at least 220 lux (20 foot candles) on food preparation surfaces, and at a distance of 75 centimeters (30 inches) above the deck in food preparation areas, handwashing facilities, warewashing areas, equipment, and utensil storage, pantries, toilet rooms, and consumer self-service areas.

Violation: The light intensity at the handwash station at the pantry entry measured 145 lux. The area was in operation.
Recommendation: Ensure the light intensity is at least 220 lux (20 foot candles) on food preparation surfaces, and at a distance of 75 centimeters (30 inches) above the deck in food preparation areas, handwashing facilities, warewashing areas, equipment, and utensil storage, pantries, toilet rooms, and consumer self-service areas.

Violation: A dirty mop was stored in a mop bucket of water in the cleaning locker adjacent to the elevator. The area was in operation.
Recommendation: After use, ensure mops are placed in a position that allows them to air-dry without soiling walls, equipment, or supplies.
