Item No.: 33
Site: Dining Room-Main Restaurant Locker A60.4.139
Violation: The deck under the shelves with galley utensils was heavily soiled with dust and debris.
Recommendation: Ensure decks, bulkheads, deckheads, and attached equipment in food preparation, warewashing, pantries, and storage areas are cleaned as often as necessary.

Site: Galley-Hot Line
Violation: The hot cabinets under the bains marie were used as plate warmers after breakfast in preparation for lunch service. These cabinets had approximately 100 previously cleaned and sanitized plates. Some of these cabinets were heavily soiled with food residues. It was discussed that the cabinets were used to keep foods hot during breakfast. The plates were immediately removed to be rewashed and the cabinets started to get cleaned.
Recommendation: Store cleaned equipment and utensils, laundered linens, and single-service and single-use articles: (1) in a clean, dry location; (2) in a location where they are not exposed to splash, dust, or other contamination; (3) at least 150 millimeters (6 inches) above the deck.

Site: Provisions-Consumable and Durable Lockers
Violation: The deck under the pallets and shelves with boxed single-service, single-use items, galley utensils, and chinaware were heavily soiled with dust and debris. Also, the cables and pipes above these articles had accumulations of dust.
Recommendation: Ensure decks, bulkheads, deckheads, and attached equipment in food preparation, warewashing, pantries, and storage areas are cleaned as often as necessary.

Site: Housekeeping-Outbreak Prevention and Response Plan (OPRP)
Violation: The OPRP listed the disinfectant Multi Quat Mega 1 as the product used during elevations of acute gastroenteritis for disinfecting public areas. The OPRP only listed dilutions and not concentrations for this disinfectant. The product's label indicated its effectiveness against norovirus at a certain dilution and a dispenser diluted the product to that specific dilution. However, there was no method to check that the actual concentration needed for the product to be effective against norovirus was achieved.
Recommendation: Ensure each vessel has a written OPRP that details standard procedures and policies to specifically address AGE onboard. Include the following in the written OPRP: (3) Disinfectant products or systems used, including the surfaces or items the disinfectants will be applied to, concentrations, and required contact times. Ensure to have a test method that measures the concentration in milligrams per liter (parts per million) of this quaternary ammonium chlorides sanitizing solution.

Site: Potable Water-Production Piping
Violation: The evaporator production line and the reverse osmosis production lines were not striped blue/gray/blue prior to chlorine injection point.
Recommendation: Stripe or paint distillate and permeate lines directed to the potable water system in accordance with ISO 14726 (blue/gray/blue). Ensure no other lines have the above color designations. Stripe or paint these lines at 5 meters (15 feet) intervals and on each side of partitions, decks, and bulkheads.

Site: Integrated Pest Management-Commendation - IPM Team
Violation: The Silver Shadow staff and crew are to be commended for their detection and elimination of a stored pest problem during July. The findings and follow-up actions documented by the ship's Environmental Officer were some of the most thorough IPM records examined on any ship in recent history of USPHS inspections.
Recommendation:
*Inspections scores of 85 or lower are NOT satisfactory
