Violation:
The cruise that ended 20 AUG had 5 AGE cases reported. Housekeeping records indicated that there were at least 14 PVI responses during the cruise. The voyage that ended 13 AUG had one passenger reporting AGE symptoms. Housekeeping records showed 5 PVI incidents from the disembarking passengers. Ensure active communication between these departments of these PVI's as a part of the overall ship's Outbreak Prevention and Response Plan.

Recommendation:
Ensure precautionary measures by housekeeping personnel are taken in consultation with the vessel?s medical staff to prevent the spread of AGE. Ensure each vessel has a written OPRP that details standard procedures and policies to specifically address AGE onboard. Include the following in the written OPRP: (1) Duties and responsibilities of each department and their staff for all the passenger and crew public areas.

Violation:
An opened and in-use carton of pina colada mix was set on the front bar counter without temperature or time control. The internal temperature of this potentially hazardous food was 55 F. The manager pointed out that the specific cartons of mix that are potentially hazardous are stored by procedure in an undercounter refrigerator, which he then opened to reveal several such quart cartons of mix. Those found did have internal temperatures below the required 41 F.

Recommendation:
Except during preparation, cooking, or cooling, or when time is used as the public health control, maintain potentially hazardous foods at 57C (135F) or above, except that roasts may be held at a temperature of 54C (130F); or 5C (41F) or less.

Violation:
The two previously cleaned small skillets at the induction cooker had the interior lower surface peeling of the non-stick coating. The fibers protruding from each skillet could be easily removed by hand pulling.

Recommendation:
Use only nonscoring or nonscratching utensils and cleaning aids on multiuse kitchenware such as frying pans, griddles, sauce pans, cookie sheets, and waffle bakers that have a perfluorocarbon resin coating. Ensure multiuse food-contact surfaces are: (1) Smooth; (2) Free of breaks, open seams greater than 0.8 mm (1/32 inch), cracks, chips, inclusions, pits, and similar imperfections; (3) Free of sharp internal angles, corners, and crevices; (4) Finished to have smooth welds and joints; and (5) Accessible for cleaning and inspection by one of the following methods without being disassembled, by disassembling without the use of tools, or by easy disassembling with the use of handheld tools commonly available to maintenance and cleaning personnel. Use only low profile, nonslotted, noncorroding, and easy-to-clean fasteners on food-contact surfaces and in splash zones. Prohibit the use of exposed slotted screws, Phillips head screws, or pop rivets in these areas.

Violation:
At the wine cellar service bars in both aft and forward pantries there were six slot-head screws fastened to each of six soda dispensing head plates on the underside of the tower soda dispenser attached to the exterior counters on each service bar. The screws were installed on the underside of the dispensing head plates, which were located directly above each soda dispensing head.

Recommendation:
Contact the manufacturer to either change the plate surrounding each dispensing head to one without screws or to find another solution that covers the existing slot screws to prevent splashing there.

Recommendation:
Ensure the light intensity is at least 220 lux (20 foot candles) on food preparation surfaces, and at a distance of 75 centimeters (30 inches) above the deck in food preparation areas, handwashing facilities, warewashing areas, equipment, and utensil storage, pantries, toilet rooms, and consumer self-service areas.

Recreational Water Facilities-New Double Slide
|

Violation:
A new double water slide was installed to replace a single water slide during the dry dock in November, 2015, This replacement installation triggered the 2011 Construction Guidelines requirements including the distance from the slide entrance to the closest passenger cleansing shower. The entrance is now 20 meters (60 feet) from the pool shower, which is double the allowed separation from an RWF entrance.

Recommendation:
12.13.3.3 Areas Not Identified - Procedures, systems, equipment, technology, processes, or activities that are not identified in the scope of this manual must not be tested or introduced operationally onboard any vessel until the concept is submitted in writing to the VSP Chief for review. If the review determines the concept is within the scope of the VSP Operations Manual, written procedures, control measures, or a complete variance submission may be required.
