Site: Medical-Acute Gastroenteritis Surveillance Log
Violation: A review of the 2-9 September 2012 acute gastroenteritis surveillance log indicated some of the dates varied between the American method (month/day/year) and the European method (day/month/year).
Recommendation: Use only one calendar date format for the acute gastroenteritis surveillance log.

Site: Food Service General-Chilled Water Lines
Violation: The chilled water lines for the ice machines were not uniquely identified.
Recommendation: Uniquely identify all refrigerant brine lines in all galleys, pantries, and cold rooms to prevent cross-connections.

Violation: The depth markers for the swimming pools were only in meters.

Site: Recreational Water Facilities-Safety Signs
Violation: The signs for the recreational water facilities did not include the bather load. The whirlpool signs did not include a caution against use by children and exceeding 15 minutes of exposure. There was no safety sign at the children's pool. Staff showed documentation that new safety signs would be installed around 5 December 2012.
Recommendation: Ensure the signs, at a minimum, include the following words: (1) do not use these facilities if you are experiencing diarrhea, vomiting, or fever; (2) no children in diapers or who are not toilet trained; (3) shower before entering the facility; and (4) bather load #. For children's RWF signs, include the exact wording "TAKE CHILDREN ON FREQUENT BATHROOM BREAKS" or "TAKE CHILDREN ON FREQUENT TOILET BREAKS." In addition to the safety sign requirements in section 6.7.1.1.1, install a sign at each whirlpool spa and spa pool entrance listing precautions and risks associated with the use of these facilities. Include, at a minimum, cautions against use by the following: (1) individuals who are immunocompromised; (2) individuals on medication or who have underlying medical conditions such as cardiovascular disease, diabetes, or high or low blood pressure; and (3) pregnant women, elderly persons, and children. Additionally, caution against exceeding 15 minutes of exposure.

Site: Recreational Water Facilities-Bather Load and Turnover Rates
Violation: There were no flow meters installed for the recreational water facilities. The bather loads and turnover rates were calculated, but these were based on the pump rates. Staff showed documentation which indicated flow meters would be installed by 6 October 2012 and stated the bather loads and turnover rates would be reassessed following the installation.
Recommendation: Install flow meters at all recreational water facilities. Recalculate the bather loads and turnover rates for all of the recreational water facilities based on the actual flow rates measured by the flow meters. If necessary, update the recreational water facility safety signs to indicate the new bather loads.

Site: Recreational Water Facilities-Antientrapment Drain Covers
Violation: All of the drain covers in the recreational water facilities were field fabricated. Staff stated they were having difficulty locating a certifying official for the drain covers. Staff showed documentation which indicated new antientrapment drain covers would be installed 2-6 October 2012.
Recommendation: For RWFs with gravity drainage and multiple drains (2 or more drains greater than 3 feet apart), provide either a standard design (not compliant with ASME A112.19.8) drain cover and an alarm, or an ASME A112.19.8 compliant antientrapment/antientanglement drain cover. Ensure the alarm is audible and sounds in both a continuously manned space and at the RWF. Ensure this alarm is for all draining: accidental, routine, and emergency. For RWFs with gravity drainage and a single blockable drain or multiple drains less than 3 feet apart, provide an ASME A112.19.8 compliant antientrapment/antientanglement drain cover and a gravity drainage system.

Site: Recreational Water Facilities-Sample Lines
Violation: The sample lines for the analyzer-chart recorders at all of the recreational water facilities were located after the compensation tank. Staff stated the sample lines would be relocated 2-6 October 2012.
Recommendation: Ensure the sample line for the analyzer probe (monitoring) is either directly from the RWF or on the return line from each RWF and before the compensation tank.

Site: Recreational Water Facilities-Whirlpool Water Dumping
Violation: The records indicating the date and time for dumping the whirlpools could not be located.
Recommendation: Ensure a written or electronic log of RWF filter inspection results, granular filter sedimentation test results, backwashing frequency and length of backwashing, and date and time of water dumping is available for review during inspections.

Site: Recreational Water Facilities-Pump Suction Filters
Violation: Records indicated that the pump suction filters (hair and lint strainers) were cleaned and disinfected monthly. There was no indication of the concentration and contact time for disinfection.
Recommendation: Clean, rinse, and disinfect the hair and lint strainer and hair and lint strainer housing on all RWFs weekly. Ensure disinfection is accomplished with an appropriate halogen-based disinfectant. At a minimum, use a 50-ppm solution for 1 minute, or equivalent CT value. Maintain records on all inspection and cleaning procedures.

Site: Buffet-Orangery Hot Line
Violation: A consumer advisory for undercooked eggs was posted in a location where not all consumers could see the advisory when self-serving the poached eggs.
Recommendation: If an animal food such as beef, eggs, fish, lamb, milk, pork, poultry, or shellfish that is raw, undercooked, or not otherwise processed to eliminate pathogens is offered in a ready-to-eat form or as a raw ingredient in another ready-to-eat food, ensure the consumer is informed by way of disclosure using menu advisories, placards, or other easily visible written means of the significantly increased risk to certain especially vulnerable consumers eating such foods in raw or undercooked form. Locate the advisory at the outlets where these types of food are served. Ensure that disclosure is made by one of the two following methods: (1) On a sign describing the animal-derived foods (e.g., "oysters on the half-shell," "hamburgers," "steaks," or "eggs"); AND that they can be cooked to order and may be served raw or undercooked; AND a statement indicating that consuming raw or undercooked meats, seafood, shellfish, eggs, milk, poultry may increase your risk for foodborne illness, especially if you have certain medical conditions. Ensure the advisory is posted at the specific station where the food is served raw, undercooked, or cooked to order.

Violation: A hotel pan of tomatoes cut on the day of the inspection had internal temperatures of 46F to 58F. The tomatoes were moved to the blast chiller.
Recommendation: Except during preparation, cooking, or cooling, or when time is used as the public health control, maintain potentially hazardous foods at 57C (135F) or above, except that roasts may be held at a temperature of 54C (130F); or 5C (41F) or less.

Violation: The crew member at the sanitizing compartment of the three-compartment sink was holding the pots by the corners while immersing them in the sanitizing solution. This prevented these small areas of the pots from being properly sanitized.
Recommendation: Ensure items are completely immersed in the sanitizing solution.

Violation: Trays were placed on the conveyor of the in-use dishwash machine in a way that all of their surface areas could not be properly cleaned and sanitized.

Violation: The light intensity was below 110 lux behind and around the combination ovens and the ice machine.
Recommendation: Ensure the light intensity is at least 110 lux (10 foot candles) behind and around mounted equipment, including counter-mounted equipment.

Violation: The light intensity was below 220 lux in the self-service areas.
Recommendation: Ensure the light intensity is at least 220 lux (20 foot candles) on food preparation surfaces, and at a distance of 75 centimeters (30 inches) above the deck in food preparation areas, handwashing facilities, warewashing areas, equipment, and utensil storage, pantries, toilet rooms, and consumer self-service areas.

Site: Housekeeping-Outbreak Prevention and Response Plan
Violation: The outbreak prevention and response plan indicated one of the trigger points for red level was a cumulative number of cases greater than or equal to 2% 'within 72 hours at any point in the cruise.'
Recommendation: Remove "in 72 hours" from this trigger point so that the red level will begin when the cumulative number of cases is greater than or equal to 2% at any time during the cruise.
