Site: Food Service General-Carbonators
Violation: Carbonators had a single check valve installed between the carbonation tank and brass fittings of the carbonator pump. Carbonated water, which has a lower pH could reach the brass pump fittings.
Recommendation: Install an approved backflow prevention assembly rated for continuous pressure between the carbonator tank and the brass fitting of the pump. Typically, this assembly would be a double check valve with an intermediate atmospheric vent.

Site: Potable Water-Medical - Sluice Machine
Violation: The air gap for the RPZ relief valve on the cold water line was not twice the diameter of the discharge pipe.
Recommendation: Ensure air gaps are at least twice the diameter of the delivery fixture opening and a minimum of 25 millimeters (1 inch).

Site: Potable Water-Bunker Stations
Violation: Several keeper chains to the potable water caps on the filling lines were not completely noncorroding. Plastic zip ties were added as links in several areas of the chains.
Recommendation: Ensure the potable water filling line has a screw cap fastened by a noncorroding cable or chain to an adjacent bulkhead or surface in such a manner that the cap cannot touch the deck when hanging free.

Site: Potable Water-Permeate and Distillate Water Lines
Violation: The distillate and permeate water lines directed to the potable water system were not painted or striped in accordance with ISO 14726 (blue/gray/blue) at 5 meter intervals.
Recommendation: Stripe or paint potable water lines either in accordance with ISO 14726 (blue/green/blue) or blue only. Stripe or paint distillate and permeate lines directed to the potable water system in accordance with ISO 14726 (blue/gray/blue). Ensure no other lines have the above color designations. Stripe or paint these lines at 5 meters (15 feet) intervals and on each side of partitions, decks, and bulkheads, except where decor would be marred by such markings, including potable water supply lines in technical lockers. Do not stripe or paint potable water lines after reduced pressure assemblies as potable water. Uniquely identify all refrigerant brine lines in all galleys, pantries, and cold rooms to prevent cross-connections.

Violation: The filling line to the Terrace Pool compensation tank was striped blue after the RPZ.

Site: Potable Water-Deck 8 Central Pantry
Violation: Blue tape covered the air relief vents on the continuous pressure backflow device installed on the potable water line to the ice machine. This was corrected.
Recommendation: Maintain backflow prevention devices in good repair.

Site: Recreational Water Facilities-Safety Signs
Violation: The safety signs at both the pools and the whirlpools did not comply with the new 2011 VSP Operations Manual. According to staff, new signs have been ordered.
Recommendation: Provide safety signs for all RWFs, except for baby-only water facilities. Ensure the signs, at a minimum, include the following words: (1) do not use these facilities if you are experiencing diarrhea, vomiting, or fever; (2) no children in diapers or who are not toilet trained; (3) shower before entering the facility; and (4) bather load #. For children?s RWF signs, include the exact wording ?TAKE CHILDREN ON FREQUENT BATHROOM BREAKS? or ?TAKE CHILDREN ON FREQUENT TOILET BREAKS.? In addition to the safety sign requirements in section 6.7.1.1.1, install a sign at each whirlpool spa and spa pool entrance listing precautions and risks associated with the use of these facilities. Include, at a minimum, cautions against use by the following: (1) individuals who are immunocompromised; (2) individuals on medication or who have underlying medical conditions such as cardiovascular disease, diabetes, or high or low blood pressure; and (3) pregnant women, elderly persons, and children. Additionally, caution against exceeding 15 minutes of exposure.

Site: Recreational Water Facilities-Depth Markers
Violation: The depth markers in feet at the swimming pools were not prominently displayed so that they could be seen from the deck and from the body of the pool.
Recommendation: Prominently display the depth of each RWF that is deeper than 1 meter (3 feet) so that it can be seen from the deck and in the pool. Label depth markers in both feet and meters. Additionally, install depth markers for every 1 meter (3 feet) change in depth.

Site: Recreational Water Facilities-Shepherd's Hooks
Violation: The poles of the shepherd's hooks at the Neptune, Calypso, and Terrace swimming pools were telescoping and not long enough to reach the center of the deepest portion of the pool from the side plus two feet.
Recommendation: Provide a rescue or shepherd?s hook and an approved flotation device at a prominent location (visible from the full perimeter of the pool) at each RWF that has a depth of 1 meter (3 feet) or greater. Mount these devices in a manner that allows for easy access during an emergency. Ensure the pole of the rescue or shepherd?s hook is long enough to reach the center of the deepest portion of the pool from the side plus 2 feet. Ensure it is a light, strong, nontelescoping material with rounded, nonsharp ends. Ensure the approved flotation device includes an attached rope that is at least 2/3 of the maximum pool width.

Site: Recreational Water Facilities-Antientrapment Drains
Violation: The Neptune pool drain covers did not meet the antientrapment requirements given in the 2011 VSP Operations Manual. The drains were gravity, multiple, and greater than 3 feet apart.
Recommendation: For RWFs with gravity drainage and multiple drains (2 or more drains greater than 3 feet apart), provide either a standard design (not compliant with ASME A112.19.8) drain cover and an alarm, or an ASME A112.19.8 compliant antientrapment/antientanglement drain cover. Ensure the alarm is audible and sounds in both a continuously manned space and at the RWF. Ensure this alarm is for all draining: accidental, routine, and emergency.

Site: Recreational Water Facilities-Whirlpool Antientrapment
Violation: The suction drains at the whirlpools were less than 3 feet apart and did not meet the secondary antientrapment requirements. According to staff, safety vacuum release systems are to be installed.
Recommendation: For RWFs with suction fittings and a single blockable drain or multiple drains (less than 3 feet apart), provide an ASME A112.19.8 compliant antientrapment/antientanglement drain cover and a safety vacuum release system or automatic pump shut-off system.

Site: Recreational Water Facilities-Whirlpool Monitoring
Violation: The sample lines for the analyzer chart recorders for all of the whirlpools were located after the compensation tanks.
Recommendation: Install chart recorders or electronic data loggers with security features that record pH and halogen measurements for each individual RWF. Ensure the sample line for the analyzer probe (monitoring) is either directly from the RWF or on the return line from each RWF and before the compensation tank. Install appropriate sample taps for analyzer calibration. In the event of equipment failure, measure free residual halogen and pH by a manual test kit at the RWF or return line at least hourly for whirlpool spas, spa pools, children?s pools, and wading pools and every 4 hours for all other RWFs. Record manual readings on a chart or log, retain for at least 12 months, and ensure that they are available for review during inspections. Complete repairs on malfunctioning halogen analyzer-chart recorders within 30 days of equipment failure. Provide an audible alarm in a continuously occupied watch station to indicate low and high free halogen and pH readings in each RWF.

Violation: The turnover rates and bather load numbers for the RWFs were calculated off of theoretical pump curves and not from actual flow rates as determined from flow meters.
Recommendation: Calculate turnover rates based off of flow meters not theoretical pump curves.

Violation: The black painted finish was chipping from the diamond shaped metal valve that was located on the inside of the plastic inserts of the liquid concentrate based, leg mounted coffee machines.
Recommendation: Materials used in the construction of multiuse utensils and food contact surfaces of equipment must be: (1) durable, corrosion resistant, and nonabsorbent; (2) sufficient in weight and thickness to withstand repeated warewashing; (3) finished to have a smooth, easily cleanable surface; and (4) resistant to pitting, chipping, crazing, scratching, scoring, distortion and decomposition.

Violation: Steam escaped from the bottom of the in-use combination ovens.
Recommendation: Ensure nonfood contact equipment is maintained in good repair and proper adjustment including: (1) Equipment maintained in a state of repair and condition that meets the materials, design, construction, and operation specifications of these guidelines; (2) Equipment components such as doors, seals, hinges, fasteners, and kick plates are kept intact, tight, and adjusted in accordance with manufacturer's specifications.

Violation: Food debris was not effectively removed from plates at the warewash station. Large pieces of lemon and single service articles were found along the clean landing and final rinse tank of the in-use flight-type warewash machine.
Recommendation: Ensure food debris on equipment and utensils is scrapped over a waste disposal unit, pulper, or garbage receptacle or removed in a warewashing machine with a prewash cycle.

Violation: A wet wiping cloth was stored in a dry bucket on the deck.
Recommendation: Restrict wiping cloths to the following: (1) Cloths used for wiping food spills are used for no other purpose; (2) Cloths used for wiping food spills are dry and used for wiping food spills from tableware and single service articles or wet and cleaned, stored in a chemical sanitizer, and used for wiping spills from food-contact and nonfood-contact surfaces of equipment; (3) Dry or wet cloths used with raw animal foods are kept separate from cloths used for other purposes. Wet cloths used with raw animal foods must be kept in a separate sanitizing solution; (4) Wet wiping cloths used with a freshly made sanitizing solution and dry wiping cloths are free of food debris and visible soil.

Violation: The single check valve installed between the carbonator tank and the brass fittings of the pump would not be sufficient to prevent carbonated water from contacting the brass fittings of the pump, which could lead to copper being leached from the brass.
Recommendation: Ensure copper and copper alloys such as brass are not used in contact with a food that has a pH below 6 such as vinegar, fruit juice, or wine or for a fitting or tubing installed between a backflow prevention device and a carbonator.

Violation: Water was turned off at the handwash station on the show-galley side. According to staff, water was turned off after each service and turned back on when the area returned to operation.
Recommendation: Ensure handwashing sinks are equipped to provide water at a temperature of at least 38°C (100°F) through a mixing valve or combination faucet. For handwash sinks with electronic sensors, and other types of handwash sinks where the user cannot make temperature adjustments, ensure the temperature provided to the user after the mixing valve does not exceed 49°C (120°F).

Violation: Four live fruit flies were found along the bulkhead adjacent to the bulk milk dispenser and juice machines near the Gazebo. In addition, at least four ants were found along the deck/bulkhead juncture coming out of a small opening where the coving tile met the bulkhead.
Recommendation: Effectively control the presence of insects, rodents, and other pests to minimize their presence in the food storage, preparation, and service areas and warewashing and utensil storage areas aboard a vessel.
