Site: Potable Water-Tank Maintenance Records
Violation: Potable water storage tanks 3C, 2S, 1S, 1P, and 1C were cleaned and disinfected in April 2012. The records indicated chlorine values of less than 5 ppm before they were put back into service. However, the actual chlorine values were not recorded.
Recommendation: Maintain documentation of all inspections, maintenance, cleaning, and disinfection for 12 months and ensure it is available for review during inspections. Ensure records include a method of disinfection, concentration and contact time of the disinfectant, and a halogen value of less than or equal to 5 ppm before the tank is put back into service.

Site: Recreational Water Facilities-Whirlpool Safety Signs
Violation: The whirlpool safety signs stated that 'children under 14 require adult supervision.' However, the clarifications page to the 2011 Operations Manual indicated that those under 16 years of age are considered children. This was also noted in the previous inspection.
Recommendation: Update the whirlpool safety signs to state that children under 16 require adult supervision.

Site: Medical-Crew Acute Gastroenteritis (AGE) Reporting
Violation: During the 2-9 December cruise, a cadet had an onset of AGE symptoms on 2 December at 1200, but did not report to the medical center until 3 December at 0730. The time card for this crew member indicated that he worked on 2 December from 1300 to 1400 while symptomatic.
Recommendation: Ensure that employees having AGE symptoms are restricted from work and report to the medical center as soon as having their first AGE symptom.

Site: Other-Neptune Lounge
Violation: Fridge #2 contained milk and yogurt containers. The fridge was not marked in accordance with the time control plan. The fridge was on time control, but had a yellow dot (setup 0800 - discard 1200) and an orange dot (setup 1700 - discard 2100) indicating different setup and discard times. The staff removed the orange dot from the fridge explaining that the yellow dot indicated the correct setup and discard times for the potentially hazardous foods in the fridge.
Recommendation: Label the refrigerator for time control. Follow the time control plan and ensure that that the correct discard dot is used.

Site: Buffet-Lido
Violation: In the continental breakfast section, there was no consumer advisory for the cold smoked salmon. This was corrected during the inspection.
Recommendation: If an animal food such as beef, eggs, fish, lamb, milk, pork, poultry, or shellfish that is raw, undercooked, or not otherwise processed to eliminate pathogens is offered in a ready-to-eat form or as a raw ingredient in another ready-to-eat food, ensure the consumer is informed by way of disclosure using menu advisories, placards, or other easily visible written means of the significantly increased risk to certain especially vulnerable consumers eating such foods in raw or undercooked form. Locate the advisory at the outlets where these types of food are served. Ensure that disclosure is made on a sign describing the animal-derived foods (e.g., "oysters on the half-shell," "hamburgers," "steaks," or "eggs"); AND that they can be cooked to order and may be served raw or undercooked; AND a statement indicating that consuming raw or undercooked meats, seafood, shellfish, eggs, milk, poultry may increase your risk for foodborne illness, especially if you have certain medical conditions. Ensure the advisory is posted at the specific station where the food is served raw, undercooked, or cooked to order.

Site: Provisions-Parasite Destruction Record
Violation: The parasite destruction record for Capital Sales had unclear and conflicting information. The letter certified that the following raw-marinated herring products were parasite free: rollmops, gallon cream, gallon wine, 10# wine, and 10# cream. However, the letter also had a statement indicating that upon request, all other cream and wine marinated herring could be delivered with a certificate stating that the products were frozen at -4F for at least 7 days. There was no other record that indicated that these ready-to-eat products were parasite free.
Recommendation: If raw, raw-marinated, partially cooked, or marinated partially cooked fish are served in ready-to-eat form, ensure: (1) The person in charge records the freezing temperature and time to which the fish are subjected and retains the records on the vessel for 90 calendar days beyond the time of service or sale of the fish; or (2) If the fish are frozen by a supplier, a written letter from the supplier which specifies the fish species involved and both the temperature to which the fish was frozen and the total time period at that temperature. If the supplier provides any of the same species to the vessel in a fresh state, ensure there is a designation on the outer packaging for the parasite-free fish.

Violation: In the continental breakfast section, there was no serving utensil for a tray of bread. This was corrected during the inspection.
Recommendation: Provide food-dispensing utensils for each container displayed at a consumer self-service unit such as a buffet or salad bar.

Violation: There were eight slotted fasteners in the food splash zone of the sake warmer machine.

Violation: Fridge # 2 was a household refrigerator.
Recommendation: Ensure food-contact surfaces of food equipment complies with American National Standards Institute (ANSI), National Sanitation Foundation International (NSF International), or other internationally accredited food equipment sanitation standards for materials, design, and construction.

Violation: There were two slotted fasteners in each of the food splash zones by the coffee dispensing tubes of the two espresso machines.
Recommendation: Use only low profile, nonslotted, noncorroding, and easy-to-clean fasteners on food-contact surfaces and in splash zones.

Violation: The potable water line and electrical cord from the espresso machine were draped across the counter, making the counter difficult to clean.
Recommendation: Shorten and/or support the water line and the electrical cord above the counter to allow for easy cleaning.

Violation: The two deck-mounted wine coolers were not easily movable and were not sealed to the deck or elevated on legs to allow for easy cleaning.
Recommendation: Ensure deck-mounted equipment that is not easily movable is sealed to the deck or elevated on legs that provide at least a 150 mm (6-inch) clearance between the deck and the equipment.

Violation: The five service trolleys in front of the starboard omelet station were scratched and scored, making them difficult to clean.
Recommendation: Ensure nonfood-contact surfaces are free of unnecessary ledges, projections, and crevices, and designed and constructed to allow easy cleaning and to facilitate maintenance.

Site: Food Service General-Rack-Type Warewasher Data Plates
Violation: The data plates of the rack-type warewashers did not indicate the rack minimum transit times. There were six of these machines on the ship. New data plates were done during the inspection.
Recommendation: Ensure a warewashing machine is provided with an easily accessible and readable data plate affixed to or posted adjacent to the machine that indicates the machine's design and operating specifications including the minimum transit time for rack conveyor machines.

Violation: The drain line of the previously cleaned and not in use sake warmer machine was soiled with a brown material.

Violation: The pipe insulation in the technical compartment of upright refrigerator 2.32 A/B was soiled with a dark material that appeared to be mold. This was corrected during the inspection.

Violation: The handwashing station at the entrance of the dishwash area did not have a sign stating 'wash hands often,' 'wash hands frequently,' or similar wording posted over the handwashing sink. This was corrected during the inspection.
Recommendation: Post a sign over handwashing sinks stating "WASH HANDS OFTEN" in a language that the food employees understand.

Violation: The light intensity to the right of the counter-mounted coffee machine was less than 110 lux.
Recommendation: Ensure the light intensity is at least 110 lux (10 foot candles) behind and around mounted equipment, including counter-mounted equipment.

Site: Integrated Pest Management-Mooring Lines
Violation: The rodent guards on the mooring lines were ineffective in preventing rodents from entering the ship. The guards did not completely surround the lines and some of the guards had blown off due to the wind.
Recommendation: Protect entry points where pests may enter the food areas.
