Site: Children Area-Child Center
Violation: Various previously cleaned small toys were being dried on towels in a playroom, and several of the small and large building blocks were still connected together, indicating they were not completely disassembled prior to cleaning.
Recommendation: Disassemble small toy pieces prior to cleaning. Ensure toys used in the child-activity center are maintained in a clean condition.

Site: Recreational Water Facilities-Records
Violation: The records for the cleaning and disinfection of the sand filter housing was not consistent, in that staff did not note the work was completed according to the procedure in the job order, or there was some history notation of only part of the job, such as brushing followed by disinfection at 50 ppm chlorine, with no wash/rinse step and no chlorine contact time.
Recommendation: Clean, rinse, and disinfect the filter housing before the new filter media is placed in it. Ensure disinfection is accomplished with an appropriate halogen-based disinfectant. At a minimum, use a 50-ppm solution for 1 minute, or equivalent CT value. Maintain records on all inspection and cleaning procedures. Include all steps in the cleaning and disinfection as part of the records and ensure contact times are provided for disinfectant concentrations.

Site: Potable Water-Records
Violation: The list of tested reduced pressure (RP) assembly backflow prevention devices along with many other types of preventers and air-gaps had future dates under the column inspection date or T date, such as 30 May, 2013, 3 September 2013, 11 October 2013, and 20 November 2013. Some of these data filled forms had the words, 'due to' handwritten into the form just below these dates, while others had no writing. In reviewing the pressure differential test results for the RP backflow preventers from the 2011 list, there were 49 RP's tested and the pressure values noted on test 1 and test 3 for every single one was an identical number to the same preventer test results for the 30 May 2013 testing. In comparison, the 2010 list of RP backflow preventers had mostly different values recorded for the same preventers under both the test 1 and test 3 than what was provided on the 2011 RP test log. Clearly the 2011 test data was simply copied over into the log form for RP backflow preventers dated 30 May 2013.
Recommendation: Ensure the supervisor or person in charge of potable water operations on the vessel demonstrates to VSP?during inspections and on request-knowledge of potable water operations, is able to demonstrate this knowledge by compliance with Section 5 of the VSP 2011 Operations Manual or by responding correctly to the inspector?s questions as they relate to the specific operation, and properly trains employees to comply with Section 5 of the VSP 2011 Operations Manual as it relates to their assigned duties.

Site: Potable Water-Records
Violation: In the backflow preventer inspection logs there were several backflow preventers listed under two log field columns titled device type and model as reduced pressure backflow preventers (RPZ). This included air-gaps and continuous pressure model backflow prevention devices, which were all titled 'RPZ' as device type and given model numbers such as RPZ - N9 or RPZ 1' under the model column.
Recommendation: Correctly describe either model or method type under description in the logs. If model number is given it should also match the model type in the manufacturers listing.

Site: Recreational Water Facilities-Spa Area Pool
Violation: The spa pool was open for use and the free chlorine concentration as measured by the inspector's test kit was 0.28 and 0.35 ppm for two consecutive tests. The ship used a different manual test kit and measured 0.64 and 0.76 ppm on two consecutive tests. Both tests from the ship and the inspector were from different parts of the pool. The spa pool was closed and netted immediately upon obtaining these test results.
Recommendation: Maintain a free residual halogen in recirculated RWFs in the following ranges: (1) swimming pools, between 1.0 and 5.0 mg/L (ppm); (2) children's pools, between1.0 and 5.0 mg/L (ppm); (3) wading pools, between 1.0 and 5.0 mg/L (ppm); (4) interactive RWFs (activity pools), between 2.0 and 5.0 mg/L (ppm); (5) baby-only water facilities, between 3.0 and 10.0 mg/L (ppm) free chlorine or between 4.0 and 10.0 mg/L (ppm) bromine; and (6) whirlpool spas and spa pools, between 3.0 and 10.0 mg/L (ppm) free chlorine or between 4.0 and 10.0 mg/L (ppm) bromine.

Site: Recreational Water Facilities-Neptune Pool
Violation: The Neptune pool was open for use and the free chlorine concentration of the water was measured at 0.61 and 0.86 ppm by the inspector. The ship's test kit measured 1.13 ppm and 1.09 ppm on consecutive tests. Both test kits were calibration checked using the secondary standards held by the ship onboard and both kits were verified thru their respective secondary standards to be accurate.
Recommendation: Maintain a free residual halogen in recirculated RWFs in the following ranges: (1) swimming pools, between 1.0 and 5.0 mg/L (ppm); (2) children's pools, between1.0 and 5.0 mg/L (ppm); (3) wading pools, between 1.0 and 5.0 mg/L (ppm); (4) interactive RWFs (activity pools), between 2.0 and 5.0 mg/L (ppm); (5) baby-only water facilities, between 3.0 and 10.0 mg/L (ppm) free chlorine or between 4.0 and 10.0 mg/L (ppm) bromine; and (6) whirlpool spas and spa pools, between 3.0 and 10.0 mg/L (ppm) free chlorine or between 4.0 and 10.0 mg/L (ppm) bromine.

Violation: The door from the front bar to the pantry was pinned open using a large free-standing metal sign holder.
Recommendation: Protect entry points where pests may enter the food areas.

Violation: The center handwash sink in the pantry was completely blocked by a large table filled with clean glasses set directly in front of it. A second handwash near the dishwash area was available for use.
Recommendation: Ensure handwashing facilities are used for no other purpose and are accessible at all times.

Site: Other-International Cafe - Pantry
Violation: The discard labels for several containers of potentially hazardous foods were stuck to the exterior of the reach-in refrigerator doors, rather than on the food containers being held under time as a public health control which were inside the refrigerator. The same method was used for potentially hazardous cold foods inside the front counter display refrigerator, where the discard labels were affixed to the exterior glass of the display refrigerator and not on the foods or even the shelf at the foods inside.
Recommendation: Label containers of potentially hazardous foods on counters even if set-up to service is less than 4 hours. Label foods inside refrigeration units and not the door to the unit.

Violation: Near the Chef's office, the light intensity was less than 110 lux on the right side of the ice machine.
Recommendation: Ensure the light intensity is at least 110 lux (10 foot candles) at a distance of 75 centimeters (30 inches) above the deck when in use, in walk-in refrigerator units and dry storage areas, and in other areas and rooms during periods of cleaning. Violation: Numerous food preparation areas had missing/recessed deck tile grout or broken deck tiles. Staff showed a progress report for these areas to be corrected in March and April.
Recommendation: Ensure decks, bulkheads, and deckheads in food preparation, warewashing, pantries, bars, and food and equipment storage areas are constructed and maintained for easy cleaning. Do not use carpet in these areas.

Violation: A Sous Chef with a full beard was preparing foods. He was not wearing a beard restraint.
Recommendation: Ensure food employees wear hair restraints and clothing that covers body hair (hats, hair nets, beard restraints and clothing). Ensure these items are designed and worn effectively to prevent hair from contacting exposed food; clean equipment, utensils, and linens; and unwrapped single-service and single-use articles. Violation: Excess condensation was on the deckhead and inside the hood overhang above the in-use potwash machine. Condensation was observed dripping onto the deck in front of the machine. No clean items were impacted.
Recommendation: Ensure ventilation hood systems and devices are sufficient in number and capacity to prevent grease or condensation from collecting on bulkheads and deckheads.

Violation: Excess condensation was on the deckhead above the potwash machine and above the sanitizing compartment of the three-compartment sink. The potwash machine was in operation, but the sanitizing compartment of the sink was not. Condensation was observed dripping onto the clean landing of the three-compartment sink. No clean items were impacted.
Recommendation: Ensure exhaust ventilation hood systems in food preparation and warewashing areas--including components such as hoods, fans, guards, and ducting--are designed to prevent grease or condensation from draining or dripping onto food, equipment, utensils, linens, and single-service and single-use articles.

Violation: A brown liquid was dripping from the water trap hatch and from the pipe at the hatch onto the deck. No clean items were impacted.

Violation: Between the last preparation counter and the bulkhead near the time control refrigerators, condensation was dripping from the deckhead vent onto the deck. The area was not in operation. No food or clean items were impacted.
Recommendation: Ensure all food preparation, warewashing, and toilet rooms have sufficient ventilation to keep them free of excessive heat, steam, condensation, vapors, obnoxious odors, smoke, and fumes.

Violation: Excess condensation was on the deckhead above the combination oven. The Ventilation Officer determined the exhaust in the galleys was on low speed, so he increased the speed. It was evident the steam coming from the oven was drawn into the exhaust system quicker.
