Ensure the master, medical staff, or other designated staff of a vessel destined for a U.S. port from a foreign port submits at least one standardized AGE report based on the number of reportable cases in the AGE log to VSP no less than 24 hours?but not more than 36 hours?before the vessel?s expected arrival at the U.S. port.

During the 13-17 June voyage, a crew member's medical record indicated they had 3 episodes of diarrhea, but the AGE log stated zero; also, this crew member position stated N/A on the log. During the 23-30 May voyage, the AGE log stated two passengers did not receive anti-diarrheal medication and were classified as nonreportable cases; the medical records stated these passengers were reportable AGE cases and that anti-diarrheal medication was dispensed.

Ensure the information in the medical records matches the information on the AGE logs.

Recommendation:
Ensure the potable water distribution system is maintained free of cross-connections. Protect the potable water system against backflow or other contamination by backflow prevention devices or air gaps. Ensure the permeate lines and distillate lines directed toward the potable water system are also protected. Protect the following connections to the potable water system against backflow (backsiphonage or backpressure) with air gaps or mechanical backflow prevention devices: Black water or combined gray water/black water systems. An air gap is the only allowable protection for these connections.

Ensure crew members responsible for the whirlpool spas enforce the bather load. Exceeding the posted bather load can result in low disinfectant levels that can allow the spread of a variety of pathogens that cause AGE as well as skin and respiratory recreational water illnesses.

The crew member did not receive any disciplinary action, as management was not notified of the late reporting and working while experiencing AGE symptoms.

Ensure crew members immediately report to the medical center upon experiencing AGE symptoms.

Ensure multiuse food-contact surfaces are: (1) Smooth; (2) Free of breaks, open seams greater than 0.8 mm (1/32 inch), cracks, chips, inclusions, pits, and similar imperfections; (3) Free of sharp internal angles, corners, and crevices.

Where toilet stalls include handwashing facilities, ensure the bare-hands-free contact begins in the toilet stall. Ensure that toilet facilities with multiple exits, such as spa dressing rooms, have bare-hands-free contact at each exit.
