Site: Potable Water-Automatic Hood Cleaning System
Violation: The vent for the testable backflow prevention device located two of these units on this had a slight drip, approximately once every 10 seconds. Engineering explained that it was common in this type of RPZ and the connection was tested within the week.
Recommendation: Confirm correct operation of these units.

Site: Medical-24-Hour Report
Violation: The 24-hour report submitted prior to arrival in Port Canaveral, Florida, on 21 January 2018 listed a total of 5 reportable crew members. However, since a crew member was listed on the AGE log twice for the same symptoms, the actual number of reportable crew members was 4.
Recommendation: Ensure the AGE report contains the following: (1) name of the vessel; (2) port of embarkation; (3) date of embarkation; (4) port of disembarkation; (5) date of disembarkation; (6) total numbers of reportable cases of AGE among passengers, including those who have disembarked because of illness?even if the number is 0 (zero reporting); (7) total numbers of reportable cases of AGE among crew members, including those who have disembarked because of illness?even if the number is 0 (zero reporting); and (8) total number of passengers and crew members on the cruise.

Site: Medical-Acute Gastroenteritis (AGE) Log
Violation: A passenger reported to the medical center on 11 January after experiencing 5 episodes of vomiting with no other symptoms and was deemed a reportable case.
Recommendation: Ensure a reportable case of AGE is a case reported to the master of the vessel, the medical staff, or other designated staff by a passenger or a crew member and meets the case definition based on the following symptoms: (1) Diarrhea (three or more episodes of loose stools in a 24 hour period or what is above normal for the individual. See section 4.1.2.1.3, illness symptoms); or (2) vomiting and one additional symptom including one or more episodes of loose stools in a 24-hour period, or abdominal cramps, or headache, or muscle aches, or fever (temperature of greater than or equal to 38C [100.4F]).

Site: Medical-Acute Gastroenteritis (AGE) Log
Violation: A crew member was listed as a reportable case twice on the AGE log for the same symptoms during the voyage from 14 January to 21 January. Medical staff stated the original entry for the crew member on the AGE log was prematurely closed on accident. Since a closed case could not be reopened in the ship's electronic medical database, a new entry for the crew member was made on the AGE log with the same information.
Recommendation: List in the AGE surveillance log (1) the name of the vessel, cruise dates, and cruise number; (2) all reportable cases of AGE; and (3) all passengers and crew members who are dispensed antidiarrheal medication from the master of the vessel, medical staff, or other designated staff.

Site: Potable Water-Tank Maintenance Records
Violation: Tank maintenance was conducted on December 4, 2017 on potable water tank 3 port. The record stated the curing time for the paint was completed on December 8, 2017. The record then stated the entire tank as washed and sprayed with chlorine on June 12. Staff stated the date was a typo, left over from the template for tank maintenance.
Recommendation: Maintain accurate records of tank maintenance.

Site: Galley-Decks 4 and 5 Dishwash Station
Violation: The aft port and aft starboard atmospheric vacuum breakers (AVB) on both decks 4 and 5 warewash machines were rusted and corroded.
Recommendation: Maintain backflow prevention devices in good repair.

Site: Recreational Water Facilities-Aqua Theatre
Violation: During the previous inspection, this facility, which was only used by crew members, did not have a safety sign posted. Crew provided documentation stating the safety sign was ordered January 4, 2018. The sign was scheduled to be delivered on February 18, 2018.
Recommendation: Provide safety signs for all RWFs, except for baby-only water facilities. Ensure the signs, at a minimum, include the following words: (1) do not use these facilities if you are experiencing diarrhea, vomiting, or fever; (2) no children in diapers or who are not toilet trained; (3) shower before entering the facility; and (4) bather load #. For children?s RWF signs, include the exact wording ?TAKE CHILDREN ON FREQUENT BATHROOM BREAKS? or ?TAKE CHILDREN ON FREQUENT TOILET BREAKS.?

Site: Provisions-Fresh Vegetables
Violation: Two heads of broccoli were growing mold on the surface. Staff explained that this material was provisioned the day of the inspection. He explained that each day the produce on hand is evaluated for the days use. The broccoli was discarded.
Recommendation: Ensure food is safe and unadulterated.

Site: Dining Room-Deck 3 Main Galley
Violation: Six bar trays stored in the waiter stations had tattered corners. The plastic coating was coming off of the trays, making them difficult to clean.
Recommendation: Ensure nonfood-contact surfaces of equipment that are exposed to splash, spillage, or other food soiling or that require frequent cleaning are constructed of a corrosion-resistant, nonabsorbent, and smooth material.

Site: Galley-Deck 3 Dishwash Station
Violation: The portside dishwash machine had a wash tank curtain that was torn in many locations. The machine was not in service at the time of the inspection. The curtain was replaced during the inspection.
Recommendation: Maintain warewashing equipment in good repair and proper adjustment, including: (1) warewashing equipment is maintained in a state of repair and condition that meets the standards of the materials, design, and construction of these guidelines; (2) water pressure and water temperature-measuring devices are maintained in good repair and accurate within the intended range of use.

Site: Galley-Windjammer
Violation: All three buckets of the wash, rinse, and sanitize station located in the galley by the tilt kettles were cloudy and contained an oily sheen. The area was open for breakfast at the time.
Recommendation: Ensure the wash, rinse, and sanitize solutions are maintained clean.

Site: Galley-Deck 4 Dishwash Area
Violation: Food debris was found in the wash tank area. It was removed and the machine was cleaned during the inspection.
Recommendation: Ensure food debris on equipment and utensils is scrapped over a waste disposal unit, pulper, or garbage receptacle or removed in a warewashing machine with a prewash cycle.

Site: Galley-Deck 3 Dishwash Station
Violation: At least 60 white coffee mugs were stored clean with coffee residue inside the cup. Staff and the inspector took alcohol wipes and rubbed the inside of the cups to see if they were stained or soiled. When the residue came off onto the alcohol pad, it was determined that they were soiled.
Recommendation: Ensure food only contacts surfaces of equipment and utensils that are cleaned and sanitized.

Violation: The grey container of blue artificial sweetener contained a buildup old food and sweetener debris greater than one day's accumulation.
Recommendation: Ensure nonfood-contact surfaces of equipment are kept free of an accumulation of dust, dirt, food residue, and other debris.

Violation: At least 15 highball glasses on the bar for service contained sticker residue on the bottom of the glasses. The glasses were not nested and standing alone.
Recommendation: Ensure nonfood-contact surfaces of equipment are kept free of an accumulation of dust, dirt, food residue, and other debris.

Violation: A shard of glass was observed under the drain mat on the bar containing clean ready to use glasses. The bartender remarked he recently broke a glass. All of the glasses were removed to rewash.
Recommendation: Store cleaned equipment and utensils, laundered linens, and single-service and single-use articles: (1) in a clean, dry location; (2) in a location where they are not exposed to splash, dust, or other contamination; (3) at least 150 millimeters (6 inches) above the deck.

Violation: Fifteen dirty glass storage racks were resting against the ice machine adjacent to the dishwash machine.
Recommendation: Store cleaned equipment and utensils, laundered linens, and single-service and single-use articles: (1) in a clean, dry location; (2) in a location where they are not exposed to splash, dust, or other contamination; (3) at least 150 millimeters (6 inches) above the deck.

Violation: The aft, mid-ship waiter station had 40 coffee cups stored upright in the clean side. The area was not in service at the time. Staff stated that they were stored there until the next service period, which would be dinner time.
Recommendation: Store clean equipment and utensils in a self-draining position that allows air drying, and covered or inverted.

Violation: In the aft waiter station, two carts stacked with soiled dishes was stored resting against the clean side counter. The station was not in use at the time, however clean dishes were on the clean side counter and breakfast had ended approximately 45 minutes earlier.
Recommendation: Maintain adequate separation between clean and soiled items.

Site: Galley-Deck 3 Hot Station
Violation: On the hot counter waiter side, multiple plate covers were stored on a soiled counter. The plate covers were taken back to be re-washed.
Recommendation: Store cleaned equipment and utensils, laundered linens, and single-service and single-use articles: (1) in a clean, dry location; (2) in a location where they are not exposed to splash, dust, or other contamination; (3) at least 150 millimeters (6 inches) above the deck.

Site: Galley-Deck 4 Hot Station
Violation: Twenty plate covers were stored with water inside the rim. The inspector and staff discussed a plan to raise the bottom cover so it would allow an air-dry opportunity for the bottom covers. These covers are stored in a pyramid type shape.
Recommendation: Store clean equipment and utensils in a self-draining position that allows air drying, and covered or inverted.

Violation: The handwash sink in this pantry was blocked by a large grey trash can while open during embarkation.
Recommendation: Ensure handwashing facilities are used for no other purpose and are accessible at all times.

Violation: A trash can full of breakfast food was full and stored without a lid. This trash can was stored just outside of the potwash area the hot galley joining bulkhead.
Recommendation: Cover receptacles and waste-handling containers when not in continuous use and after they are filled.

Violation: In a technical department below the bain marie station, the hot water supply pipe was leaking water that pooled in the technical compartment. This was fixed during the inspection.
Recommendation: Ensure decks, bulkheads, and deckheads in food preparation, warewashing, pantries, and storage areas are maintained in good repair.

Site: Buffet-Windjammer
Violation: Water was leaking from the drain line on a large mobile ice container at the starboard beverage station in front of the ice machine. A significant amount of water had accumulated on the deck in this area while the area was open for breakfast.
Recommendation: Ensure decks, bulkheads, deckheads, and attached equipment in food preparation, warewashing, pantries, and storage areas are cleaned as often as necessary.

Violation: A broom and dust pan were resting against the counter outside the cleaning materials locker.
Recommendation: Ensure maintenance tools such as mops, brooms, and similar items are stored in a designated locker so they do not contaminate food, food-contact surfaces of utensils, and equipment, linens, and single-service and single-use articles.

Site: Integrated Pest Management-Windjammer
Violation: One fruit fly was observed above the hand wash station on the port side during breakfast.
Recommendation: Effectively control the presence of insects, rodents, and other pests to minimize their presence in the food storage, preparation, and service areas and warewashing and utensil storage areas aboard a vessel.

Violation: The interior doors to the pool bar pantry and adjacent pantry were pinned open. The exterior doors leading to the pool area were self-closing. However the positive air pressure was holding the door open allowing a direct open path into these pantries.
Recommendation: Protect entry points where pests may enter the food areas.

Violation: The lid on the previously cleaned diaper waste receptacle at the diaper changing station was soiled
Recommendation: Clean and disinfect diaper changing, handwashing facilities, and toilet rooms daily and when soiled during use.
