Violation:
Since the last inspection on 3 January 2018, nine different 24-hour reports submitted by the ship prior to arrival in an U.S. port were never received by VSP. Staff stated that since the beginning of the year the new electronic medical database was supposed to automatically submit a 24-hour report prior to arrival in the U.S. The electronic medical database did have a record that these nine 24-hour reports were submitted, but upon further investigation it was determined that the email used by the electronic medical database was midrs_test@cdc.gov.test instead of midrs@cdc.gov. The specific 24-hour reports missing were from prior to arrival in Miami, Florida on 13 January 2018; in Key West, Florida on 22 January 2018; in Miami, Florida on 6 February 2018; in Miami, Florida on 16 February; in Key West, Florida on 25 February 2018; in San Juan, Puerto Rico on 1 March 2018; in St. Thomas, U.S. Virgin Islands on 5 March 2018; in Key West, Florida on 17 March 2018; and in Miami, Florida on 28 March 2018.

Recommendation:
Ensure the master, medical staff, or other designated staff of a vessel destined for a U.S. port from a foreign port submits at least one standardized AGE report based on the number of reportable cases in the AGE log to VSP no less than 24 hours-but not more than 36 hours-before the vessel's expected arrival at the U.S. port.

Recommendation:
Submit routine 24-hour and 4-hour update reports electronically and ensure that the vessel maintains proof onboard that the report was successfully received by VSP. Maintain the 24-hour, 4-hour, and special reports on the vessel for 12 months.

Violation:
Since the last inspection on 3 January 2018, the initial halogen value recorded at the start of the daily shock halogenation for the spa pool and both whirlpool spas was anywhere between 5.0 ppm and 7.0 ppm. The final halogen value at the end of the 1-hour shock halogenation was recorded as above 10.0 ppm.

Recommendation:
Increase the free residual halogen to at least 10.0 mg/L (ppm) and circulate for at least 1 hour every 24 hours. Test the free residual halogen both at the start and completion of shock halogenation. Superhalogenate the water in the entire RWF system to 10 ppm to include the whirlpool spa/spa pool tub, compensation tank, filter housing, and all associated piping before starting the 1-hour timing.

Ensure warewashing machines, drainboards, and the compartments of sinks, basins, or other receptacles used for washing and rinsing equipment, utensils, or raw foods, or laundering wiping cloths are cleaned: (1) before use; (2) throughout the day at a frequency necessary to prevent recontamination of equipment and utensils and to ensure that the equipment performs its intended function; (3) at least every 24 hours (if used).
