Violation:
There was an 8 hour discrepancy in the report times to medical between the AGE logs and the actual report times annotated on the handwritten patient record. The medical staff was aware of this problem and had already reported this computer program issue to management and continued follow-up with discussion with corporate while the inspector was on board was on board.

Recommendation:
Ensure the AGE surveillance log entry for each passenger or crew member contain the following information in separate columns: (1) Date of the first medical visit or report to staff of illness; (2) time of the first medical visit or report to staff of illness; (3) case identification number; (4) person?s name; (5) person?s age; (6) person?s sex; (7) designation as passenger or crew member; (8) crew member position or job on the vessel, if applicable; (9) cabin number; (10) meal seating information; (11) date of illness onset; (12) time of illness onset; (13) illness symptoms, including the presence or absence of the following selected signs and symptoms, with a separate column for each of the following: (a) number of episodes of diarrhea in a 24-hour period; (b) number of episodes of vomiting in a 24-hour period; (c) bloody stools; (d) recorded temperature; (e) abdominal cramps; (f) headaches; (g) muscle aches; (14) entry (yes/no) for whether this was a reportable case; (15) entry (yes/no) for antidiarrheal medications sold or dispensed by designated medical staff; (16) presence of underlying medical conditions that may affect interpretation of AGE. Ensure the AGE surveillance log contains the above information in the exact order and is entered in the template in Annex 13.2.2. Ensure that the log data is exported in the exact order as in the example template in Annex 13.2.2 with analyzable formats such as Excel or Access. Ensure that any additional data fields are entered only outside of the form margins when exported to VSP.

The potable water line on the ice machine had a single check backflow preventer installed on the line. Since the ice machine is used over 12 hours in a day, a continuous pressure device is required.

Recommendation:
Install a continuous pressure-type backflow preventer when a valve is located downstream from the backflow preventer. Install a backflow prevention device for continous pressure on the water supply line.

Violation:
Many of the pool records showed that the halogen residual levels in the pools were below 1.0 ppm for 1-2 hours in the evenings between 1500 to 1900. Staff explained this happened when the pools shift from recirculation mode to sea-to-sea mode. Staff explained they switched to sea-to sea for a short time to change the water. The times for switchover from recirculation to sea-to-sea mode were not recorded on any logs or charts and no notations were documented to indicate why the halogen residual levels were below 1.0 ppm. Staff was extremely knowledgeable about when the pools were switched to sea-to-sea mode, but this was not documented. The halogen residual average over 4 hours was above 1.0 ppm during these periods when the pool was open.

Water was observed dripping onto food on the hot buffet line and the rain was so strong that it was blowing the water on the hot foods. A passenger was observed taking food from the line. A manager instructed the food employees to shut down the line prior to the passenger taking food, but the food employees did not shut down the line until another manager came up to approve shutting down the outlet. It was discussed if food employees observed food contamination, such as dripping onto food from a deckhead, rain, or other source of contamination, the food employees should take immediate action to discard the food and not wait for approval to discard food items or shut down a food line.

Recommendation:
During preparation, ensure unpackaged food is protected from environmental sources of contamination such as rain. Protect food from contamination by storing the food: (1) Covered or otherwise protected; (2) In a clean, dry location; (3) Where it is not exposed to splash, dust, or other contamination; and (4) At least 15 centimeters (6 inches) above the deck. Staff stated that they had replaced the breaker and the machine was working consistently. During the down time, staff made sure the soiled pots were stacked and stored appropriately.
