Item No.: 02
Site: Medical-Immediate Contacts of Symptomatic Crew
Violation: The medical records had no notes on whether crew AGE cases had immediate contacts other than cabin mates. Medical staff documented follow up interviews for cabin mates.
Recommendation: When any crew member (food or nonfood employee) meets the case definition for AGE, take the following actions: (1) restrict exposure to symptomatic crew member(s); (2) undergo a verbal interview with medical or supervisory staff, who will confirm their condition, provide facts and a written fact sheet about hygiene and handwashing, and instruct them to report immediately to medical if they develop illness symptoms; (3) complete a verbal interview daily with medical or supervisory staff until 48 hours after the ill crew members? symptoms began. Ensure that the first verbal interview is conducted within 8 hours from the time the ill crew member initially reported to the medical staff. If the asymptomatic immediate contact or cabin mate is at work, ensure that he or she must is contacted by medical or supervisory staff as soon as possible. Document the date and time of verbal interviews.

Site: Galley-Deck 3
Violation: Staff and inspector thought that the test strips being used by the crew were not accurate. In the potwash area, the sanitizing concentration was slightly below 50 ppm when checked by both staff and the inspector. Staff adjusted the concentration immediately and used new test strips.
Recommendation: Provide a test kit or other device that accurately measures the concentration in milligrams per liter (parts per million) of sanitizing solutions.

Site: Galley-Deck 3
Violation: In the potwash area, the sanitizing concentration was slightly below 50 ppm when checked by both staff and the inspector. Staff adjusted the concentration immediately and used new test strips. Staff and inspector thought that the test strips being used by the crew were not accurate.
Recommendation: Ensure sanitizing solutions are used with the following concentrations: (1) A chlorine solution with a concentration between 50 mg/L (ppm) and 200 mg/L (ppm).

Site: Galley-Deck 3
Violation: Both ice machines serving the main galley had an excess amount of pink slimy substance across the entire length of the water return arm and on the inside and outside of the ice thickness sensor. After the finding, the machines were cleaned and put back into service.
Recommendation: Ensure food contact surfaces of equipment and utensils are clean to sight and touch.

Site: Galley-Deck 3 Bain Marie Area
Violation: In the splash zone under the three shelves above the bain marie station, the inspector found an excess of hard food debris and grease. The bain maries were not in use at the time and no food was impacted. The area was cleaned after the finding.
Recommendation: Ensure food contact surfaces of equipment and utensils are clean to sight and touch.

Site: Buffet-Lido Deck 9
Violation: In the waiter station adjacent to quench beverage station, six plastic previously cleaned glasses were stored wet. These glasses were previously washed and in the clean area of the waiter station ready to be used for passengers. The glasses were removed and sent to be re-washed.
Recommendation: Store clean equipment and utensils in a self-draining position that allows air drying, and covered or inverted.

Site: Galley-Canaletto Restaurant Deck 9
Violation: The crew restroom door, just off the galley did not close tightly automatically. The bottom of the door hit the scupper and stopped about 50 mm (2 inches) short of being closed. Staff had the engineer fix the scupper during the inspection.
Recommendation: Ensure toilet rooms are completely enclosed and have tight-fitting, self-closing doors which are kept closed except during cleaning or maintenance.

Site: Galley-Canaletto Restaurant Deck 9
Violation: A trash can with food inside had no lid. The area was not in service at the time and would not be in service until dinner. It was approximately 12:30 pm when the area was visited by the inspector. Staff instructed crew to place a lid on the can during the down time between breakfast and dinner.
Recommendation: Cover receptacles and waste-handling containers when not in continuous use and after they are filled.

Site: Galley-Potwash Area
Violation: One fruit fly was noticed in the potwash area.
Recommendation: Effectively control the presence of insects, rodents, and other pests to minimize their presence in the food storage, preparation, and service areas and warewashing and utensil storage areas aboard a vessel.

Site: Integrated Pest Management-Training
Violation: The assistant shop manager inspecting and accepting incoming supplies on 18 October had no IPM training documented.
Recommendation: Document the training of the pest-control personnel.

Site: Children Area-Club HAL
Violation: The area for children under 6 years old had 17 fabric cushions that were difficult to clean. The material was absorbent. Staff stated they were steamed periodically.
Recommendation: Ensure surfaces of tables, chairs, and other furnishings that children touch with their hands are cleanable.
