Site: Medical-Close Contact Follow Up
Violation: A stateroom attendant presented with AGE symptoms during the 28 January 2023 to 2 February 2023 voyage. The stateroom attendant handled ice but was mislabeled as a nonfood worker. A follow-up interview with one close contact was conducted for 24 hours instead of the required 48 hours. The medical staff stated that the doctor will conduct additional review for future AGE cases.
Recommendation: When any crew member (food or nonfood employee) meets the case definition for AGE, take the following actions for the asymptomatic cabin mates or immediate contacts of symptomatic crew: (1) restrict exposure to symptomatic crew member(s); (2) undergo a verbal interview with medical or supervisory staff, who will confirm their condition, provide facts and a written fact sheet about hygiene and handwashing, and instruct them to report immediately to medical if they develop illness symptoms; (3) complete a verbal interview daily with medical or supervisory staff until 48 hours after the ill crew members? symptoms began. Ensure that the first verbal interview is conducted within 8 hours from the time the ill crew member initially reported to the medical staff. If the asymptomatic immediate contact or cabin mate is at work, ensure that he or she is contacted by medical or supervisory staff as soon as possible. Document the date and time of verbal interviews. An immediate contact is someone sharing a cabin either full-time or part-time or sharing a cabin?s bathroom facilities. This includes boyfriends, girlfriends, spouses, and significant others. Ensure to document if the symptomatic crew member has or not cabin mate(s) or other immediate contact(s).

Violation: The medical staff stated they identify cabin mates as immediate close contacts for AGE cases, but do not include individuals that use the shared bathroom as immediate close contacts. Up to 35 crew members may share a communal bathroom facility. VSP defines an immediate contact as someone sharing a cabin either full-time or part-time or sharing a cabin?s bathroom facilities. After reviewing records, this appeared to affect four crew AGE cases.
Recommendation: An immediate contact is someone sharing a cabin either full-time or part-time or sharing a cabin?s bathroom facilities. This includes boyfriends, girlfriends, spouses, and significant others. Ensure to document if the symptomatic crew member has or not cabin mate(s) or other immediate contact(s).

Site: Pantry-Deck 9 Coffee Shop
Violation: The backflow prevention device on the potable water line to the ice machine was covered with blue tape. Staff advised that the ice machine had an internal air gap. Remediation began immediately.
Recommendation: Maintain backflow prevention devices in good repair.

Site: Medical-Acute Gastroenteritis (AGE) Records
Violation: A stateroom attendant presented with AGE symptoms during the 28 January 2023 to 2 February 2023 voyage. The stateroom attendant handled ice but was mislabeled as a nonfood worker. The crew member was isolated for 24 hours instead of the required 48 hours.
Recommendation: When food employees meet the case definition for AGE, ensure the following actions are taken: (1) isolate in cabin or designated restricted area until symptom-free for a minimum of 48 hours; (2) follow-up with and receive approval by designated medical personnel before returning crew to work; and (3) document date and time of last symptom and clearance to return to work.

Site: Galley-Deck 8 Aft Beverage Station
Violation: The left juice machine was missing one fastener from the food splash zone. Remediation began immediately and was complete before the inspector left the area.
Recommendation: Ensure multiuse food-contact surfaces are: (1) Smooth; (2) Free of breaks and open seams greater than 0.8 mm (1/32 inch); and (3) Free of crevices. Use only low profile, nonslotted, noncorroding, and easy-to-clean fasteners on food-contact surfaces and in splash zones. Prohibit the use of exposed slotted screws, Phillips head screws, or pop rivets in these areas.

Violation: The second oven from the bottom of the left-side stacked ovens unit was missing two fasteners from the food-contact interior baking surface. Additionally, there were two slotted fasteners in the food-contact interior baking surface. Remediation began immediately and was complete before the inspector left the area.
Recommendation: Ensure multiuse food-contact surfaces are: (1) Smooth; (2) Free of breaks and open seams greater than 0.8 mm (1/32 inch); and (3) Free of crevices. Use only low profile, nonslotted, noncorroding, and easy-to-clean fasteners on food-contact surfaces and in splash zones. Site: Galley-Deck 3 Potwash
Violation: The hood-type potwash machine had been out of service since 14 January 2023. The work order indicated the repairs were waiting on spare parts to address the leaking booster. The crew potwash was using a three-compartment sink, but the area was overwhelmed with soiled pots and pans.
Recommendation: Maintain warewashing equipment in good repair and proper adjustment. Ensure warewashing equipment is maintained in a state of repair and condition that meets the standards of the materials, design, and construction of these guidelines. Ensure drainboards, utensils, racks, or tables large enough to accommodate all soiled and cleaned items that may accumulate during hours of operation are provided for necessary utensil holding before cleaning and after sanitizing.

Site: Pantry-Deck 11 Ice Machine
Violation: The recirculating ice machine reservoir had black debris inside of it. The crew member stated that the machine was cleaned three days ago. The machine was taken out of service, cleaned, and sanitized before the end of the inspection.
Recommendation: Ensure food contact surfaces of equipment and utensils are clean to sight and touch.

Site: Pantry-Deck 5 Ice Machine
Violation: The recirculating ice machine reservoir had black debris inside of it. The crew member stated that the machine was cleaned last week. The machines were taken out of service, cleaned, and sanitized before the end of the inspection.
Recommendation: Ensure food contact surfaces of equipment and utensils are clean to sight and touch.

Site: Galley-Deck 10 Blue Iguana Crew Handwash Sink
Violation: The temperature of the hot water for the handwashing sink measured 78.6F after several moments. The area was in operation. Remediation began immediately and was complete before the inspector left the area.
Recommendation: Ensure handwashing sinks are equipped to provide water at a temperature of at least 38C (100F) through a mixing valve or combination faucet.

Site: Buffet-Deck 10 Port Side Self-Pour Beer Dispenser
Violation: The deck underneath the dispenser was heavily soiled with more than a day's accumulation of debris. Although the unit was on wheels, two static bars had been affixed to the back to secure it to the bulkhead which prevented it from being moved for cleaning. There were 3 inches of clearance between the bottom of the unit and the deck. Staff responded to have the deck cleaned with a steam cleaner and advised that the two static bars would be changed to permit moving the equipment for routine cleaning underneath.
Recommendation: Ensure decks, bulkheads, deckheads, and attached equipment in food preparation, warewashing, pantries, and storage areas are cleaned as often as necessary.

Site: Buffet-Deck 10 Starboard Side Self-Pour Beer Dispenser

Violation: Underneath the stainless steel cabinets, the deck tiles around the stainless-steel posts were damaged and difficult to clean. Water had pooled on the deck around the back, far right-side post.
Recommendation: Ensure decks, bulkheads, deckheads, and attached equipment in food preparation, warewashing, pantries, and storage areas are cleaned as often as necessary. Violation: The tiled bulkhead of the counter was missing a round tile piece, which made the surface rough and difficult to clean. The area was not in operation, but still being cleaned and sanitized from breakfast service.
Recommendation: Ensure decks, bulkheads, and deckheads in food preparation, warewashing, pantries, and storage areas are maintained in good repair.

Violation: The decorative green pillar base was chipped on the crew service side, so the surface was rough and difficult to clean.

Recommendation: Ensure the supervisor or person in charge of medical operations related to AGE on the vessel demonstrates to VSP?during inspections and on request?knowledge of medical operations related to AGE, is able to demonstrate this knowledge by compliance with Section 4 of the VSP 2018 Operations Manual or by responding correctly to the inspector?s questions as they relate to the specific operation, and properly trains employees to comply with Section 4 of the VSP 2018 Operations Manual as it relates to their assigned duties.
