Site: Medical-Acute Gastroenteritis (AGE) Medical Log
Violation: The AGE log for voyage 16813 (3/31-4/8) indicated 11 reportable passenger cases in the log header when there were only 10 individual passenger entries marked 'Y' under the reportable column. It appeared the surveillance software added one passenger who was marked as non-reportable.
Recommendation: Ensure the AGE surveillance log includes a header containing the following information about the voyage: (1) Vessel name; (2) Voyage number; (3) Date from; (4) Date to; (5) Total number of passengers; (6) Reportable total number of passengers ill; (7) Total number of crew; (8) Reportable total number of crew ill. Total number of passengers and total number of crew must be the totals at the beginning of the voyage (i.e., totals on ?date from?).

Site: Potable Water-Cross-Connection Control Log - Recycling Center
Violation: The cross-connection control log only listed one non-testable backflow prevention device in this room. The inspector identified four vacuum breakers and one double check valve in this room. Staff planned to add the additional backflow prevention devices to the log.
Recommendation: Ensure the cross-connection control program includes at a minimum: a complete listing of cross-connections and the backflow prevention method or device for each, so there is a match to the plumbing system component and location.

Site: Potable Water-Engine Room - Mineralizer
Violation: The production potable water line after halogen injection and the mineralizer was striped blue/gray/blue.
Recommendation: Stripe or paint potable water lines either in accordance with ISO 14726 (blue/green/blue) or blue only. Stripe or paint distillate and permeate lines directed to the potable water system in accordance with ISO 14726 (blue/gray/blue). Ensure no other lines have the above color designations.

Site: Recreational Water Facilities-Swimming Pool Drain Covers
Violation: The two, field-fabricated drain covers in each of the three swimming pools (Main Pool Forward, Main Pool Mid-Ship, and Solarium) were not mounted flush to the bottom of the pool surface, which created an entrapment hazard. These field fabricated gravity drain covers were approximately 0.5 to 0.25 inches off the bottom surface along the edge facing the middle of the pool.
Recommendation: For RWFs with gravity drainage and a single blockable drain or multiple drains less than 3 feet apart, provide an ASME A112.19.8 compliant anti-entrapment/anti-entanglement drain cover and a gravity drainage system. Ensure the drain covers are properly installed so they do not create an entrapment hazard.

Site: Galley-Deck 5 Room Service
Violation: One large metal pan of sliced roast beef stored in walk-in freezer 5411.371-13F was not fully covered. Staff immediately replaced the plastic covering the pan.
Recommendation: Protect food from contamination by storing the food: (1) Covered or otherwise protected; (2) In a clean, dry location; (3) Where it is not exposed to splash, dust, or other contamination.

Site: Galley-Deck 4 Ice Machine
Violation: The left ice machine's deflector panel had a barcode sticker on the food-contact surface creating a difficult to clean surface. The area was clean at the time of the inspection.
Recommendation: Ensure multiuse food-contact surfaces are smooth.

Site: Galley-Deck 4 Hot Line
Violation: The stainless steel raised countertop in between the bar grill and flat top grill was in disrepair which created large gaps and difficult to clean surfaces.
Recommendation: Ensure nonfood-contact surfaces are free of unnecessary ledges, projections, and crevices, and designed and constructed to allow easy cleaning and to facilitate maintenance. Ensure nonfood contact equipment is maintained in good repair and proper adjustment including: (1) Equipment maintained in a state of repair and condition that meets the materials, design, construction, and operation specifications of these guidelines.

Site: Other-Engine Control Room - Coffee Station
Violation: The espresso machine was mounted on top of a laminated wood countertop that was in disrepair.
Recommendation: Ensure nonfood-contact surfaces are free of unnecessary ledges, projections, and crevices, and designed and constructed to allow easy cleaning and to facilitate maintenance. Design and construct nonfood-contact surfaces of equipment and utensils to be durable and to retain their characteristic qualities under normal use conditions.

Violation: The beverage station had four circular indents on the countertop near the coffee station and the coffee station's front left mounting plate had one circular indent which created difficult to clean surfaces.
Recommendation: Ensure nonfood-contact surfaces are free of unnecessary ledges, projections, and crevices, and designed and constructed to allow easy cleaning and to facilitate maintenance.

Site: Galley-Deck 4 Warewash
Violation: The left nozzle of the final sanitizing rinse's spray arm was partially blocked. Staff immediately removed the spray arm and unblocked the spray nozzle.
Recommendation: Maintain warewashing equipment in good repair and proper adjustment, including: (1) warewashing equipment is maintained in a state of repair and condition that meets the standards of the materials, design, and construction of these guidelines; (2) water pressure and water temperature-measuring devices are maintained in good repair and accurate within the intended range of use.

Site: Galley-Deck 4 Potwash
Violation: After breakfast, one large waste receptacle filled with various stainless-steel bowls and pans, eight stacks between two and four feet high of metal pans and plastic containers, and one moveable cart stacked with small stainless-steel pans were across from the trolley wash. The small pans and bowls on the moveable cart had shifted in between the nearby stack of large plastic pans and were not properly racked. Staff stated since they were not embarking guests, they were doing a thorough cleaning of all items.
Recommendation: Ensure drainboards, utensils, racks, or tables large enough to accommodate all soiled and cleaned items that may accumulate during hours of operation are provided for necessary utensil holding before cleaning and after sanitizing.

Violation: The ice machine's deflector panel was soiled on the food-contact surface. The inspector used an alcohol wipe, which removed the residue.
Recommendation: Ensure food only contacts surfaces of equipment and utensils that are cleaned and sanitized. Ensure food contact surfaces of equipment and utensils are clean to sight and touch.

Violation: The deckhead's center plate above the ice machine had a visible gap between the plate and deckhead with yellow/orange residue along the edge.

Violation: Condensate had collected on the deckhead above the upright combination oven and water was dripping out of the oven and pooling on the deck. Staff fully closed the oven door which immediately reduced the amount of steam in the area.

Violation: The light intensity measured 191 lux in front of the juice machine and 95 lux in front of the ice machine.
Recommendation: Ensure the light intensity is at least 220 lux (20-foot candles) on food preparation surfaces, and at a distance of 75 centimeters (30 inches) above the deck in food preparation areas, handwashing facilities, warewashing areas, equipment, and utensil storage, pantries, toilet rooms, and consumer self-service areas.
