Site: Medical-Specimen Collection Containers
Violation: The ship did not have the required viral specimen collection containers on board. Additionally, there are no specimen shipping containers on board.
Recommendation: Ensure the medical staff maintains a supply of at least 10 clinical specimen collection containers for both viral and bacterial agents (10 for each), as well as a shipping container that meets the latest shipping requirements of the International Air Transport Association (IATA) and U.S. Department of Transportation for Biological Substances, Category B designated shipments (or higher). Ensure the vessel maintains the appropriate labels and markings required for shipping Biological Substances, Category B shipments. Ensure the vessel maintains appropriate refrigerant materials to keep clinical specimens cool. (see Annex 13.4 of the VSP 2018 Operations Manual.)

Site: Provisions-Hotel Stores
Violation: Two stacks of 3-4 boxes each of bottled water were stored on the deck.
Recommendation: Protect food from contamination by storing the food: (1) Covered or otherwise protected; (2) In a clean, dry location; (3) Where it is not exposed to splash, dust, or other contamination; (4) At least 150 millimeters (6 inches) above the deck; (5) At least 127 millimeters (5 inches) for food in packages and working containers on pallets, skids, and racks; and (6) At least 150 millimeters (6 inches) above the deck on shelving racks.

Violation: The shelf inside the upper liquor cabinet was soiled with dust. The cabinet was full of previously opened bottles.
Recommendation: Protect food from contamination that may result from a physical, chemical, biological origin.

Violation: The soda gun had slotted fasteners on the food splash panel.
Recommendation: Use only low profile, nonslotted, noncorroding, and easy-to-clean fasteners on food-contact surfaces and in splash zones. Prohibit the use of exposed slotted screws, Phillips head screws, or pop rivets in these areas.

Violation: Three black trays exiting the rack-type dishwash machine were positioned on the rack in a way that prohibited all surfaces from being exposed to spray from all cycles.
Recommendation: Ensure soiled items to be cleaned in a warewashing machine are loaded into racks, trays, or baskets or onto conveyors in a position that (1) exposes the items to the unobstructed spray from all cycles; (2) allows the items to drain.

Violation: The light intensity measured less than 110 lux between the combination oven and the time control hot unit. The light bulb was not working during the inspection.
Recommendation: Ensure the light intensity is at least 110 lux (10 foot candles) at a distance of 75 centimeters (30 inches) above the deck when in use, in walk-in refrigerator units and dry storage areas, and in other areas and rooms during periods of cleaning. Ensure the light intensity is at least 110 lux (10 foot candles) behind and around mounted equipment, including counter-mounted equipment.

Site: Housekeeping-Outbreak Prevention and Response Plan (OPRP)
Violation: The OPRP did not state the concentration for the disinfectant used during outbreaks.
Recommendation: Ensure each vessel has a written OPRP that details standard procedures and policies to specifically address AGE onboard. Include the following in the written OPRP: (3) Disinfectant products or systems used, including the surfaces or items the disinfectants will be applied to, concentrations, and required contact times. Ensure the disinfectant products or systems are effective against human norovirus or an acceptable surrogate (e.g., caliciviruses).

Violation: The air intake filters were damaged, wet, and not properly seated. The crew stated that they will replace the filters with a new blue filter media.
Recommendation: Keep air handling units clean.
