Item No.: 02
Site: Medical-Acute Gastroenteritis Log (AGE)
Violation: The food and beverage questionnaires stated for voyages commencing on 9/6/2022 and 7/12/2022, each 'food worker' AGE case had eaten in the crew mess for both lunch and dinner. However the staff verified that based on the onset and isolation times, the ill crew members were in their cabins under surveillance. It was determined that staff had not followed up to verify the information on this questionnaire was accurate.
Recommendation: Distribute questionnaires detailing activities and meal locations for the 72 hours before illness onset to all passengers and crew members who are AGE CASES. Ensure that the self-administered questionnaires contain all of the data elements that appear in the questionnaire found in Annex 13.2.2. Ensure that completed questionnaires are maintained with the AGE surveillance log.

Site: Potable Water-International Shore Connection Reduced Pressure Assemblies (RPZ)
Violation: The testable backflow assembly log stated the (RPZ) assemblies for the international shore connection located at FR6, FR19, FR268 portside and FR268 starboard side were tested on 2/24/2022, 5/25/2022, 5/19/2022, and 5/20/22 respectively. When the inspector asked staff how these devices are inspected, no standard operating procedure was presented, and no staff member knew how to energize the system to test these assemblies under pressure while under operation. It was determined by staff and the inspector that this test had not been conducted, yet testing dates were entered. Staff produced a procedure and pictures detailing how to test these assemblies and presented this information to the inspector by the end of the inspection. Staff agreed to test these assemblies immediately and update their testable backflow assembly log with the correct test cock pressures, differential pressures, and test dates accordingly.
Recommendation: Inspect and test backflow prevention devices requiring testing (e.g., reduced pressure backflow prevention devices and pressure vacuum breakers) with a test kit after installation and at least annually. Maintain test results showing the pressure differences on both sides of the valves.

Site: Medical-Acute Gastroenteritis Log (AGE)
Violation: On voyage dated 7/26 - 8/9/2022, a food worker who met the AGE case definition had illness onset on 8/1/2022, but did not report to medical until 8/2/2022 at 0530. The log noted 15 episodes of diarrhea had taken place between the first symptom and the time reported to medical. The inspector noted that the ship had reprimanded this crew member accordingly. Timecards for this crew member were verified that they had not worked during this time.
Recommendation: When nonfood employees meet the case definition for AGE, ensure the following actions are taken: (1) isolate in cabin or designated restricted area until symptom-free for a minimum of 24 hours; (2) follow-up with and receive approval by designated medical personnel before returning crew to work; (3) document the date and time of last symptom and clearance to return to work.

Site: Galley-Deck 1 - Cold Galley
Violation: On 12 and 13 October, Manfredi's cooling logs indicated Tiramisu was not properly cooled below 41°F. On 12 October the 'time in' was 1048 with a 'temperature in' at 68.7°F. At 1220 the temperature was 69°F. This was the final recorded time and temperature for this food item on the log. On 13 October the 'time in' was 1230 with a 'temperature in' at 68.7°F. At 1356 the temperature was 68.4°F. This was the final recorded time and temperature for this food item on the log. The logs were reviewed and signed by a supervisor.
Recommendation: Ensure cooked potentially hazardous food is cooled from 57°C (135°F) to 21°C (70°F) within 2 hours and from 21°C (70°F) to 5°C (41°F) or less within 4 hours.

Site: Bar-Deck 7 - Living Room
Violation: One small black fly was inside the pastry container. The station was in operation. No food was impacted at the time of the observation.
Recommendation: Protect food from contamination by storing the food: (1) Covered or otherwise protected; (2) In a clean, dry location; (3) Where it is not exposed to splash, dust, or other contamination.

Site: Galley-Deck 1 - Bakery / Pastry
Violation: Condensate formed around the outside of the left undercounter refrigerator door. It appeared the door gasket did not seal properly along the bottom.
Recommendation: Ensure nonfood contact equipment is maintained in good repair and proper adjustment including: (1) Equipment maintained in a state of repair and condition that meets the materials, design, construction, and operation specifications of these guidelines; (2) Equipment components such as doors, seals, hinges, fasteners, and kick plates are kept intact, tight, and adjusted in accordance with manufacturer's specifications.

Site: Galley-Deck 1 - Bakery / Pastry
Violation: Condensate formed around the outside of the left undercounter refrigerator door. It appeared the door gasket did not seal properly along the bottom.
Recommendation: Ensure nonfood-contact surfaces of equipment are kept free of an accumulation of dust, dirt, food residue, and other debris.

Site: Buffet-Deck 7 - World Cafe Port Side Beverage Station
Violation: Black pipes were on the bottom left side of the technical compartment below the coffee machine, which made it difficult to clean.
Recommendation: Install exposed utility service lines and pipes so they do not obstruct or prevent cleaning.

Site: Galley-Deck 1 - Cold Galley
Violation: In the far left undercounter technical compartment for cold counter F-3, a gap the pipe where it penetrated the back bulkhead. A reefer fitter quickly replaced the foam before the end of the inspection.
Recommendation: Ensure decks, bulkheads, and deckheads in food preparation, warewashing, pantries, and storage areas are maintained in good repair.

Site: Buffet-Deck 7 - Mamsen
Violation: The seam between the back bulkhead picture and white counter backsplash was soiled and in poor repair.
Recommendation: Ensure decks, bulkheads, and deckheads in food preparation, warewashing, pantries, bars, and food and equipment storage areas are constructed and maintained for easy cleaning.

Site: Preparation Room-Deck 1 - Meat, Poultry, Fish
Violation: The light intensity was less than 220 lux on the left side of the fish preparation counter.
Recommendation: Ensure the light intensity is at least 220 lux (20 foot candles) on food preparation surfaces, and at a distance of 75 centimeters (30 inches) above the deck in food preparation areas, handwashing facilities, warewashing areas, equipment, and utensil storage, pantries, toilet rooms, and consumer self-service areas.
