Site: Medical-Acute Gastroenteritis (AGE) Surveillance Log
Violation: A nonfood employee had AGE symptom onset at 0200 on 13 February 2026, but did not report to medical until 0843. The employee was in their cabin with their roommate. They received training and disciplinary action.
Recommendation: When nonfood employees are symptomatic and meet the case definition for AGE, ensure the following actions are taken: (1) Isolate in cabin or designated restricted area until symptom-free for a minimum of 24 hours; (2) Follow-up with and receive approval by designated medical personnel before returning crew to work; (3) Document the date and time of last symptom and clearance to return to work.

Site: Recreational Water Facilities-Deck 14 Hydra Room
Violation: The safety sign did not include the verbiage 'no children in diapers or who are not toilet trained.'
Recommendation: Provide safety signs for all RWFs. Ensure signs are in a prominent and unobstructed location for all users of the RWF. At a minimum, include the following words: (1) Do not use these facilities if you are experiencing diarrhea, vomiting, or fever. (2) No children in diapers or who are not toilet trained; (3) Shower before entering the facility; (4) Bather load #. For children?s RWF safety signs, include the exact wording ?TAKE CHILDREN ON FREQUENT BATHROOM BREAKS? or ?TAKE CHILDREN ON FREQUENT TOILET BREAKS.?

Site: Buffet-Deck 14 - Oceanview Cafe - Bar Store
Violation: The light intensity at the beverage storage rack, near the electrical panel, measured less than 110 lux.
Recommendation: Ensure the light intensity is at least 110 lux (10 foot-candles) at 750 millimeters (30 inches) above the deck in walk-in refrigerator units, dry storage areas, linen storage, waiter stations, scrapping stations, and handwashing stations in bars.

Violation: A bartender was wearing a cloth wristband on their arm during food operations. This was corrected immediately.
Recommendation: Ensure food employees, including bartenders, do not wear jewelry on their arms or hands while preparing food.

Violation: An accumulation of oily residue was on the bottom half of the fire shutter and around the deckhead housing.
Recommendation: Ensure decks, bulkheads, deckheads, and attached equipment in food preparation, warewashing, pantries, and storage areas are cleaned as often as necessary.

Violation: Two previously cleaned and sanitized plates were stored soiled on the serving line plate holder. This was corrected immediately.
Recommendation: Ensure food only contacts surfaces of cleaned and sanitized equipment and utensils. Ensure food-contact surfaces of equipment and utensils are clean to sight and touch.

Violation: The glasswash machine's final sanitizing rinse bottom spray arm had a partially blocked nozzle that did not produce a fan-like spray pattern. This was corrected immediately.
Recommendation: Maintain warewashing equipment in good repair and proper adjustment, including: (1) Warewashing equipment is maintained in a state of repair and condition that meets the standards of the materials, design, and construction; (2) Water pressure and water temperature-measuring devices are maintained in good repair and accurate within the intended range of use.

Violation: The chlorine solution in the sanitizing bucket measured 25 ppm. This was corrected immediately.
Recommendation: Ensure sanitizing solutions are used with the following concentrations: (1) A chlorine solution with a concentration between 50 mg/L (ppm) and 200 mg/L (ppm).

Violation: A customer receipt was stored with previously cleaned and sanitized glasses in an overhead cabinet. This was corrected immediately.
Recommendation: Store cleaned equipment and utensils, laundered linens, and single-service articles and single-use articles: (1) In a clean, dry location; (2) In a location where they are not exposed to splash, dust, or other contamination; (3) At least 150 millimeters (6 inches) above the deck unless in packages on pallets, skids, or racks, which may be 127 millimeters (5 inches) above the deck

Violation: An excessive amount frozen condensate was on the deckhead.
