Item No.: 11
Site: Medical-Acute Gastroenteritis (AGE) Surveillance Log
Violation: A nonfood employee had an onset of acute gastroenteritis (AGE) symptoms on 6 September at 09:30 a.m. and reported to the medical center later that same day at 12:14 p.m. According to their time card, the employee worked from 09:25 a.m. to 12:00 p.m. A food employee had an onset of AGE symptoms on 3 September at 23:45 p.m. and reported to the medical center on 4 September at 00:15 a.m. According to their time card they worked from 22:03 p.m. to midnight on 3 September. A food assessment was completed.
Recommendation: When food employees meet the case definition for AGE, ensure the following actions are taken: (1) isolate in cabin or designated restricted area until symptom-free for a minimum of 48 hours; (2) follow-up with and receive approval by designated medical personnel before returning crew to work; (3) document date and time of last symptom and clearance to return to work. When nonfood employees meet the case definition for AGE, ensure the following actions are taken: (1) isolate in cabin or designated restricted area until symptom-free for a minimum of 24 hours; (2) follow-up with and receive approval by designated medical personnel before returning crew to work; (3) document the date and time of last symptom and clearance to return to work. Ensure the food employee's supervisor or person in charge conducts an assessment of food prepared or served by the food employee while symptomatic and take appropriate corrective actions. Document corrective actions taken as a result of the assessment. Exclude food employees suspected of, diagnosed with, or exposed to any communicable disease caused by Salmonella typhi, Shigella spp., E. coli O157:H7, hepatitis A virus, norovirus, or other communicable diseases that can be transmitted by food, from working in any food or food related areas or operations, including working with exposed food, warewashing, equipment, utensils, table linens, single-service articles and single-use articles. Do not allow the excluded individual to return to the above duties until they are symptom free for a minimum of 48 hours.

Site: Buffet-Deck 11 Windjammer Thirst Quenchers
Violation: The time control plans at the portside and starboard side Thirst Quenchers were swapped so they listed the incorrect undercounter refrigerator for the location.
Recommendation: Post a time control plan at each outlet where time control is used and ensure time control plan is accessible to all crew working at each outlet where time control is used. Ensure plan(s): (1) List all units (to include preparation counters, cabinets, compartments, and equipment) on time control, but only cabinets and compartments on time control must be physically labeled.

Site: Pantry-Deck 13 Vortex
Violation: The ice scoop's handle was touching the ice inside the ice machine.
Recommendation: During pauses in food preparation or dispensing, ensure food preparation and dispensing utensils are stored: (1) In the food with their handles above the top of the food and the container; (2) In food that is not potentially hazardous with their handles above the top of the food within containers or equipment that can be closed, such as bins of sugar, flour, or cinnamon; (3) On a clean portion of the food preparation table or cooking equipment only if the in-use utensil and the food-contact surface of the food preparation table or cooking equipment are cleaned and sanitized at least every 4 hours; (4) In running water of sufficient velocity to flush particulates to the drain (if used with moist food such as ice cream or mashed potatoes); (5) In a clean, protected location (if the utensils, such as ice scoops, are used only with a food that is not potentially hazardous); or (6)In a container of water (if the water is maintained at a temperature of at least 57C [135F] and the container is frequently cleaned and sanitized).

Site: Bar-Deck 11 Pool
Violation: The countertop edge along the entire length of the crew side was rough which made the surface difficult to clean.
Recommendation: Ensure nonfood-contact surfaces of equipment that are exposed to splash, spillage, or other food soiling or that require frequent cleaning are constructed of a corrosion-resistant, nonabsorbent, and smooth material. Ensure nonfood-contact surfaces are free of unnecessary ledges, projections, and crevices, and designed and constructed to allow easy cleaning and to facilitate maintenance.

Violation: The right undercounter cabinet door below the juice machine was in disrepair with a broken door hinge pin.
Recommendation: Ensure nonfood contact equipment is maintained in good repair and proper adjustment including: (1) Equipment maintained in a state of repair and condition that meets the materials, design, construction, and operation specifications of these guidelines; (2) Equipment components such as doors, seals, hinges, fasteners, and kick plates are kept intact, tight, and adjusted in accordance with manufacturer's specifications.

Site: Pantry-Deck 6 Schooner
Violation: The hood-type glasswash machine was in disrepair with the water leaking from the bottom of the machine and pooling on the deck.
Recommendation: Maintain warewashing equipment in good repair and proper adjustment, including: (1) warewashing equipment is maintained in a state of repair and condition that meets the standards of the materials, design, and construction of these guidelines; (2) water pressure and water temperature-measuring devices are maintained in good repair and accurate within the intended range of use.

Site: Buffet-Deck 11 Windjammer Show Galley
Violation: The chlorine sanitizing solution, in the red bucket near the middle entrance, measured greater than 200 ppm. Staff immediately replaced the solution.
Recommendation: Ensure sanitizing solutions are used with the following concentrations: (1) A chlorine solution with a concentration between 50 mg/L (ppm) and 200 mg/L (ppm).

Violation: The backflow prevention device for the soda machine was soiled with green and white residue.
Recommendation: Ensure nonfood-contact surfaces of equipment are kept free of an accumulation of dust, dirt, food residue, and other debris.

Violation: The deckhead throughout the area was soiled with a white crystallized residue. Staff stated it was from the pest control chemical used the previous evening.
Recommendation: Ensure decks, bulkheads, deckheads, and attached equipment in food preparation, warewashing, pantries, and storage areas are cleaned as often as necessary.

Violation: Condensate collected on the deckhead above the far-right combination oven and was dripping onto the deck.
Recommendation: Ensure decks, bulkheads, deckheads, and attached equipment in food preparation, warewashing, pantries, and storage areas are cleaned as often as necessary.

Violation: Condensate collected on the deckhead above the far-right combination oven and was dripping onto the deck.

Ensure ventilation hood systems and devices operate effectively to prevent grease and condensate from collecting on the bulkheads and deckheads and remove contaminants generated by equipment located under them.
