Site: Medical-Acute Gastroenteritis (AGE) Reporting
Violation: The 24-hour AGE report submitted by the vessel did not match the AGE surveillance log. The AGE surveillance log for the 25-28 April 2025 voyage listed only one AGE reportable passenger case, but the 24-hour AGE report stated there was one reportable crew case. Medical confirmed the case was a passenger.
Recommendation: Ensure the AGE report contains the correct following information: (6) total numbers of reportable cases of AGE among passengers, including those who have disembarked because of illness?even if the number is 0 (zero reporting); (7) total numbers of reportable cases of AGE among crew members, including those who have disembarked because of illness?even if the number is 0 (zero reporting).

Site: Galley-Deck 3 - Rack-type Potwash
Violation: The atmospheric vacuum breaker (AVB) was continuously leaking. This was replaced, however, when the inspector asked if the new AVB was disinfected, the crew member stated 'yes, for 30 seconds.' This was the incorrect disinfection method. It was advised to disinfect with 50 ppm for 4 hours.
Recommendation: Disinfect after potential contamination by increasing the free residual halogen to at least 50 mg/L (ppm) throughout the affected area and maintain this concentration for 4 hours or by way of another procedure submitted to and accepted by VSP.

Violation: Water continuously leaked from the atmospheric vacuum breaker.

Violation: The walk-in refrigerator was out of order.
Recommendation: Food contact equipment must be maintained in good repair and proper adjustment: (1) Equipment must be maintained in a state of repair and condition that meets the materials, design, construction, and operating specifications of these guidelines.

Site: Buffet-Deck 9 - Beverage Station Aft Portside
Violation: The silicone was in disrepair around the legs of the counter-mounted hot chocolate machine.
Recommendation: Ensure nonfood-contact surfaces are free of unnecessary ledges, projections, and crevices, and designed and constructed to allow easy cleaning and to facilitate maintenance.

Violation: The hood-type glasswash machine was not operational due to a leak.

Violation: The rack-type potwash curtains in the wash compartment were heavily soiled and stained. Additionally, there were some areas where the curtains were ripped. This was replaced immediately.

Violation: The hood-type potwash machine was under maintenance during the time of the inspection. The crew stated there was a leak coming from a pipe.
Recommendation: Maintain warewashing equipment in good repair and proper adjustment, including: (1) warewashing equipment is maintained in a state of repair and condition that meets the standards of the materials, design, and construction of these guidelines.

Violation: An excessive amount of condensate was inside the technical compartment below the coffee machine.

Violation: The bulkhead in back of the service line had old, dried food debris that had accumulated for more than a day.
Recommendation: Ensure decks, bulkheads, deckheads, and attached equipment in food preparation, warewashing, pantries, and storage areas are cleaned as often as necessary.

Recommendation: Ensure decks, bulkheads, and deckheads in food preparation, warewashing, pantries, and storage areas are maintained in good repair.

Violation: The deckhead was heavily soiled with dust and was partially open to the plenum.

Violation: Pest were sighted in the following areas: Deck 10 - Steakhouse - three flies were near the handwashing station. Deck 5 - Alchemy Bar - four flies were near the handwashing station. Deck 3 - Main Galley Center - a crawling insect was near the soup kettles.
Recommendation: Effectively control the presence of insects, rodents, and other pests to minimize their presence in the food storage, preparation, and service areas and warewashing and utensil storage areas aboard a vessel.

Recommendation: Ensure the supervisor or person in charge of potable water operations on the vessel demonstrates to VSP?during inspections and on request-knowledge of potable water operations, is able to demonstrate this knowledge by compliance with Section 5 of the VSP 2018 Operations Manual or by responding correctly to the inspector?s questions as they relate to the specific operation, and properly trains employees to comply with Section 5 of the VSP 2018 Operations Manual as it relates to their assigned duties.
