Violation: THE GASTROINTESTINAL ILLNESS LOG DID NOT COLLECT DATA ON ABDOMINAL CRAMPS, HEADACHE, OR MYALGIA. THIS IS A NEW REQUIREMENT IN THE AUGUST 2005 VSP OPERATIONS MANUAL.
Recommendation: Ensure the new log captures these additional symptoms.

Violation: THERE WAS NO MEDIA FOR COLLECTING CLINICAL SPECIMENS FOR BACTERIAL AGENTS. THIS IS A NEW REQUIREMENT OF THE AUGUST 2005 VSP OPERATIONS MANUAL.
Recommendation: The medical staff will be responsible for maintaining a supply of at least 10 clinical specimen collection containers for both viral and bacterial agents ( 10 for each), as well as the proper shipping containers and labels for same and provided in Annex 13.4 of this manual.

Violation: THE COFFEE MACHINE HAD A NON-CONTINUOUS PRESSURE BACKFLOW PREVENTION DEVICE INSTALLED ON THE POTABLE WATER SUPPLY LINE.
Recommendation: Ensure a continuous backflow prevention device is installed if there is a valve downstream of the device.

Recommendation: Install the appropriate continuous pressure backflow devices.

Violation: THE POTABLE WATER HOSES WERE NOT LABELED 'POTABLE WATER ONLY'. THIS IS A NEW 2005 REQUIREMENT.
Recommendation: Ensure that potable water hoses are labeled "POTABLE WATER ONLY" in 1/2 inch letters at each connecting end.

Violation: THE SERVING SPOONS FOR THE JAM WERE TO SHORT. THE HANDLES OF THESE UTENSILS WERE TOUCHING THE FOOD CONTACT SIDES OF THE SERVING CONTAINERS.
Recommendation: During pauses in food preparation or dispensing, ensure that food preparation and dispensing utensils are stored: (1) In the food with their handles above the top of the food and the container.

Recommendation: Ensure that nonfood-contact equipment components such as doors, seals, hinges, fasteners, and kick plates are kept intact, tight, and adjusted in accordance with manufacturer's specifications.

Recommendation: Ensure that nonfood-contact surfaces are free of unnecessary ledges, projections, and crevices, and designed and constructed to allow easy cleaning and to facilitate maintenance.

Violation: THERE WAS A GAP BETWEEN THE TILE GROUT AND THE METAL PLATE AT THE ENTRANCE TO FREEZER ROOM 11-11, MAKING CLEANING DIFFICULT.
Recommendation: Ensure that decks, bulkheads, and deckheads in food preparation, warewashing, pantries, and storage areas are constructed and maintained for easy cleaning.
