Site: Medical-MEDICAL
Violation: THE MEDICAL DEPARTMENT DID NOT HAVE THE CLINICAL SPECIMEN CONTAINERS OR THE APPROPRIATE LABELS AND PACKAGING FOR BOTH VIRAL AND BACTERIAL SPECIMENS. THIS IS A NEW 2005 VSP REQUIREMENT.
Recommendation: Ensure the medical staff maintains a supply of at least 10 clinical specimen collection containers for both viral and bacterial agents ( 10 for each), as well as the proper shipping containers and labels for the same and provided in Annex 13.4 of this manual.

Site: Potable Water-
Violation: DAILY CALIBRATION OF THE FAR POINT ANALYZER/CHART RECORDER WAS NOT RECORDED BETWEEN 14 AUGUST AND 1 DECEMBER 2005. ALSO, THE CHARTS FOR 29 AND 30 SEPTEMBER WERE NOT DATED AND BETWEEN 13 AND 26 OCTOBER, THERE WERE 8 UNDATED AND 3 MISSING CHARTS.
Recommendation: Ensure that the halogen analyzer-chart recorder is properly maintained, operated, and calibrated daily in accordance with the manufacturer's instructions.

Site: Potable Water-
Violation: THERE WAS NO DOCUMENTATION OF THE RECOMMENDATIONS FOLLOWED FOR DRYING/CURING OF COATINGS USED IN THE POTABLE WATER TANKS. THIS IS A NEW REQUIREMENT OF THE 2005 VSP OPERATIONS MANUAL.
Recommendation: Ensure that interior coatings on potable water tanks are approved for potable water contact.

Violation: THERE WAS NO DOCUMENTATION THAT THE POTABLE WATER TANKS WERE FLUSHED TO BELOW 5 PPM BEFORE PLACING THEM BACK INTO SERVICE FOLLOWING DISINFECTION.
Recommendation: Ensure that the disinfected parts of the system are flushed with potable water until the free residual halogen is 5.00 mg/L (ppm).

Violation: SLOTTED FASTENERS WERE NOTED UNDER THE CUBER IN THE UPPER COMPARTMENT OF THE ICE MACHINE.
Recommendation: Remove the slotted fasteners and replace them with smooth low profile non slotted fasteners.

Violation: AN OPEN CELL INSULATING MATERIAL WAS NOTED ON THE FRONT PLASTIC COVER FOR THE WATER/ICE FALL AREA IN SOME OF THE CORNELIUS ICE MACHINES, LOCATED IN VARIOUS FOOD PREPARATION AREAS.
Recommendation: Ensure that nonfood-contact surfaces of equipment that are exposed to splash, spillage, or other food soiling or that require frequent cleaning are constructed of a corrosion-resistant, nonabsorbent, and smooth material.

Violation: NUMEROUS DOORS FOR THE UNDERCOUNTER REFRIGERATION UNITS WERE NOT TIGHT FITTING. THE BOTTOMS OF THESE DOORS DID NOT SEAL WELL.
Recommendation: Ensure that nonfood-contact equipment components such as doors, seals, hinges, fasteners, and kick plates are kept intact, tight, and adjusted in accordance with manufacturer's specifications.

Site: Galley-MAIN GALLEY - HOT SERVICE LINE
Violation: EXCESSIVE SEALANT WAS NOTED IN THE NON-FOOD CONTACT SURFACES OF THE FRYERS LOCATED ON THE FORWARD HOT SERVICE LINE, MAKING CLEANING DIFFICULT.
Recommendation: Ensure that nonfood-contact surfaces of equipment that are exposed to splash, spillage, or other food soiling or that require frequent cleaning are constructed of a corrosion-resistant, nonabsorbent, and smooth material.

Violation: A THIN GRADE OF ALUMINUM FOIL WAS USED TO WRAP THE PIPES AROUND KETTLES AND TILT PANS. THE FOIL WRAPPING WAS LOOSE AND DIFFICULT TO CLEAN.
Recommendation: Ensure that nonfood-contact surfaces of equipment that are exposed to splash, spillage, or other food soiling or that require frequent cleaning are constructed of a corrosion-resistant, nonabsorbent, and smooth material. Ensure that nonfood-contact surfaces are free of unnecessary ledges, projections, and crevices, and designed and constructed to allow easy cleaning and to facilitate maintenance.

Violation: THERE WAS NO DATA PLATE ON THE DISHWASHERS.
Recommendation: Ensure that warewashing machines are provided with an easily accessible and readable data plate affixed to the machine by the manufacturer that indicates the machine's design and operating specifications including the: (1) Temperatures required for washing, rinsing, and sanitizing; (2) Pressure required for the fresh water sanitizing rinse; and (3) Conveyor speed for conveyor machines or cycle time for stationary rack machines.

Violation: THE INTERIOR OF THE CTX WAS SOILED WITH FOOD DEBRIS. THE STAFF STATED THE CTX IS NOT USED.

Recommendation: Repair or replace the heat tape for the cool unit in pastry room 30.

Violation: THE ROOM SERVICE PANTRY WAS NOTABLY HOT AND HUMID.
Recommendation: Ensure that all food preparation, warewashing, and toilet rooms have sufficient ventilation to keep them free of excessive heat, steam, condensation, vapors, obnoxious odors, smoke, and fumes.
